Analysis of the Outcome of Reoperative Face Lifts Using Sub-SMAS Techniques.

BACKGROUND: Reoperative face lift can be technically more demanding than primary face lift because of altered anatomy and scarred tissue plane. This study was carried out to analyze the outcome of reoperative face lift using sub-superficial musculoaponeurotic system (SMAS) techniques, including facial nerve injury. METHODS: Seventy reoperative face lifts using sub-SMAS techniques were studied. Data analysis includes the patients' demographic information, the interval between the previous face lift and the reoperative face lift, intraoperative findings of the SMAS-platysma anatomy, complicatio

Facial Nerve Repair: Bioengineering Approaches in Preclinical Models.

Injury to the facial nerve can occur after different etiologies and range from simple transection of the branches to varying degrees of segmental loss. Management depends on the extent of injury and options include primary repair for simple transections and using autografts, allografts, or conduits for larger gaps. Tissue engineering plays an important role to create artificial materials that are able to mimic the nerve itself without extra morbidity in the patients. The use of neurotrophic factors or stem cells inside the conduits or around the repair site is being increasingly studied to enh

Common rugby league injuries. Recommendations for treatment and preventative measures.

Rugby league is the main professional team sport played in Eastern Australia. It is also very popular at a junior and amateur level. However, injuries are common because of the amount of body contact that occurs and the amount of running that is required to participate in the game. Injuries to the lower limbs account for over 50% of all injuries. The most common specific injuries are ankle lateral ligament tears, knee medial collateral and anterior cruciate ligament tears, groin musculotendinous tears, hamstring and calf muscle tears, and quadriceps muscle contusions. Head injuries are common

Implications of Facial Fracture in Airway Management of the Adult Population: What Is the Most Effective Management Strategy?

PURPOSE: Facial fractures are a harbinger when it comes to airway management. Facial fractures can cause airway obstruction or preclude the use of intubation. We aim to examine the etiologies, types of facial fractures, and the risk factors that may lead to requirement of an advance airway. METHODS: A retrospective chart review was performed of all facial fractures in the adult population in a level 1 trauma center in an urban environment (University Hospital in Newark, NJ). Patient demographics were collected, as well as location of fractures, concomitant injuries, and course of hospital sta

Facing the facts on prophylactic antibiotics for facial fractures: 1 day or less.

BACKGROUND: To evaluate the role of initial prophylactic antibiotics on facial fractures, outcomes were compared between a short course (≤24 hours) of antibiotics to those who received an extended course (>24 hours). METHODS: Adults admitted (2010-2015) to a Level I trauma center intensive care unit with at least one facial bone fracture and major injuries isolated to the head and neck were included. Our primary analysis compared infectious complications of the head or neck (H/N infection) between patients given short or extended courses of antibiotic prophylaxis. Multivariate logistic regres

The usefulness of transcatheter arterial embolization for patients with blunt polytrauma showing transient response to fluid resuscitation.

BACKGROUND: This study aimed to determine whether nonsurgical management using transcatheter arterial embolization (TAE) is safe for patients with blunt multiple trauma who transiently respond to the initial fluid resuscitation. METHODS: Contrast computed tomography was performed for patients with blunt abdominal injuries, excluding those who did not respond to initial fluid resuscitation. Angiography was performed for patients with injuries showing contrast extravasation or solid organ injury classified, according to the American Association for the Surgery of Trauma, as grade 3 or higher on

Skull Base Trauma: Clinical Considerations in Evaluation and Diagnosis and Review of Management Techniques and Surgical Approaches.

Traumatic injuries to the skull base can involve critical neurovascular structures and present with symptoms and signs that must be recognized by physicians tasked with management of trauma patients. This article provides a review of skull base anatomy and outlines demographic features in skull base trauma. The manifestations of various skull base injuries, including CSF leaks, facial paralysis, anosmia, and cranial nerve injury, are discussed, as are appropriate diagnostic and radiographic testing in patients with such injuries. While conservative management is sometimes appropriate in skull

The impact of COVID-19 on facial laceration trends: a comparative analysis of pre- and intra-pandemic data.

BACKGROUND: This study explores the impact of the COVID-19 pandemic on the patterns, mechanisms, and clinical management of facial lacerations. By comparing data from pre-pandemic and intra-pandemic periods, it aims to elucidate how COVID 19 influenced maxillofacial trauma care. MATERIALS AND METHODS: A retrospective analysis was conducted on 747 patients treated for facial lacerations at a maxillofacial trauma center. Data were collected for two periods: pre-COVID (February 2019-January 2020) and intra-COVID (February 2020-January 2021). Demographics, injury characteristics, mechanisms, and

Autonomic Nerve Fibers Aberrantly Reinnervate Denervated Facial Muscles and Alter Muscle Fiber Population.

The surgical redirection of efferent neural input to a denervated muscle via a nerve transfer can reestablish neuromuscular control after nerve injuries. The role of autonomic nerve fibers during the process of muscular reinnervation remains largely unknown. Here, we investigated the neurobiological mechanisms behind the spontaneous functional recovery of denervated facial muscles in male rodents. Recovered facial muscles demonstrated an abundance of cholinergic axonal endings establishing functional neuromuscular junctions. The parasympathetic source of the neuronal input was confirmed to be

T cell memory in the injured facial motor nucleus: relation to functional recovery following facial nerve crush.

T cells have the ability to mount a memory response to a previously encountered antigen such that re-exposure to the antigen results in a response that is greater in magnitude and function. Following facial nerve transection, T cells have been shown to traffic to injured motor neurons in the facial motor nucleus (FMN) and may have the ability to promote neuronal survival and functional recovery. Previously, we demonstrated that early exposure to neuronal injury on one side of the brain during young adulthood elicited a T cell response that was greater in magnitude following exposure to the sam

Anatomical variation in the cervical branch of the facial nerve: a systematic review.

BACKGROUND: There is a paucity of literature on the anatomical variations of the cervical branch of the facial nerve (CBFN). Poor awareness of its anatomy and variations risks intraoperative damage, misdiagnosis of facial nerve injury and undesirable functional and aesthetic outcomes for patients. PURPOSE: To review anatomical studies on the CBFN and synthesise data on its course, innervations and common variations. METHODS: A PRISMA-compliant systematic review of anatomical studies of the cervical branch of the facial nerve was conducted. PubMed, Embase, Scopus, Cochrane Library, Web of Sci

Novel clinical classification system for peripheral facial nerve defects after oncological resection.

Peripheral facial nerve defects resulting from oncological resection necessitate early repair, and underscore the need for accurate intraoperative assessment of their extent. This study proposes a span-based classification system for peripheral facial nerve defects, based on the length and location of the defect. The system delineates specific defect characteristics and provides practical guidance for intraoperative decision-making and personalised repair plans.

[Postoperative facial and vestibular nerve palsy: experimental study of its pathophysiological mechanisms].

The 7th and 8th cranial nerves were shifted in the cerebellopontine (CP) angle of dogs by cerebellar retractions that were similar to those performed in humans with monitoring of auditory evoked brainstem responses (ABR). Postoperatively, the vestibular, facial nerves, and brainstem were histologically examined. Caudal-to-rostral shifts of the nerves could induce vestibular and/or facial nerve damages. The most vulnerable portion of the vestibular nerve was located between the vestibular ganglions and the area vestibularis-the most lateral end of the internal auditory canal. This indicated tha

The clinical diagnosis of cranial nerve injuries at operations on the carotid arteries.

Operation on the carotid arteries (CA) prevent strokes in patients with occlusions and stenoses of these arteries. We have worked out the measures for preventing perioperative strokes. During interventions on the CA, iatrogenic injuries to the cranial nerves (CN) are paid little attention. 1186 patients underwent 1362 operations. There were 26 lethal outcomes and 38 strokes. 146 (0.7%) cases of CN injuries were documented. Of these, there were 2 (0.1%) injuries to the accessory nerve, 14 (1%) to the glossopharyngeal nerve, 28 (2%) to the hypoglossal nerve, 46 (3.4%) injuries to vagal nerve bra

Vertical suture plication of the orbicularis oris muscle: a simple procedure for the correction of unilateral marginal mandibular nerve paralysis.

Injury to the marginal mandibular nerve is present as an adverse outcome in many surgical procedures. The resultant cosmetic deficit, manifesting lower lip asymmetry and imbalance, is readily noticeable especially during opening of the mouth. Multiple techniques to correct this deformity have been described, but most involve obvious scars or major procedures. A simple, one-step outpatient procedure, which can be performed under local anesthesia with a camouflaged scar, is described to correct the lip malposition.
