Primary spontaneous pneumothorax occasionally occurs in young healthy individuals. Most of the time, it is diagnosed before it progresses to acute respiratory failure. We present a healthy young girl who developed acute respiratory failure secondary to primary spontaneous pneumothorax. Prompt evaluation and management are essential because this entity can lead to fatal outcome if not diagnosed and treated immediately.

Fungal infections occur in patients who are severely immunocompromised with profound and prolonged neutropenia. We report a patient of acute lymphoblastic leukemia who developed nonspecific abdominal symptoms within two days after the onset of neutropenia in the early induction phase of chemotherapy, which was later found to be due to intestinal mucormycosis and resulted in a fatal outcome.

Hunter's syndrome is due to recessively inherited mucopolysaccharide storage diseases. The anaesthetic complications in this syndrome are related to the organs involved in the disease, but pertain especially to upper airway obstruction problems. We report a patient with Hunter's syndrome who, while under general anaesthesia for a simple procedure (the introduction of bilateral grommets), suffered a fatal outcome.

A case of diffuse neonatal haemangiomatosis involving the skin, liver, lungs, adrenals, gums, diaphragm, skull, and testes is reported. Intra-uterine onset of bleeding led to bloody amniotic fluid, severe anaemia, congestive heart failure, and hydrops fetalis. Intractable coagulopathy and renal failure resulted in persistent bleeding, anuria, metabolic acidosis, and hyperkalaemia, leading to a fatal outcome.

We report a case of adult respiratory distress syndrome in a 65 yr old man under inappropriate steroid therapy who developed both Herpes simplex virus 1 (HSV) and Mycobacterium tuberculosis pneumonia; we conclude from clinical and histological data that HSV 1 played a major role in the fatal outcome of this immunocompromised patient, in spite of acyclovir treatment.

A 58-year-old Chinese woman was admitted to hospital with a presumed hypersensitivity reaction to allopurinol. Her illness was characterised by high fever, eosinophilia, exfoliative dermatitis and jaundice. She developed fulminant hepatic failure and septicemia with a fatal outcome. Clinical details are presented and the possible relationship of allopurinol hypersensitivity to renal impairment is discussed.

An analysis of lethality in cholecystitis has been carried out. The main complications, which had resulted in the death of 238 patients, were analysed. Purulent complications served as the most common cause of fatal outcome. The authors came to the conclusion on the necessity to broaden indication for an early surgical intervention in acute cholecystitis in elderly patients.

The clinical course and post mortem findings of a 57 year old woman stuffering from polymyositis are reported. Besides the symptoms of the inflammatory musculoskeletal process the myocardial involvement finally gaines importance leading to the fatal outcome. Therefore the indication to early preventive pacemaker implantation is recommended in all cases of myocardial involvement with symptoms of conduction defects.

A 29-year-old female suffering from acute terminal ileitis was subjected to laparotomy undertaken on the tentative diagnosis of acute appendicitis. The appendix was found to be normal and was removed. The course was complicated with segmental gastro-intestinal gangrene with fatal outcome. The involvement of histotoxic clostridial infection as a causative factor is discussed.

Colonoscopy is a common procedure but splenic injury after colonoscopy is quite uncommon. Its presentation can be atypical and its outcome possibly fatal. In this case report, we highlight the importance of prompt recognition of the incident, by discussing etiological factors, risk factors and also by underlining the significance of maintaining a clinical awareness of this complication.

In rheumatic diseases the possibility of pulmonary manifestation must always be considered and checked. Interstitial lung disease can often be fatal in these cases. In the presented case, the link between progressive dyspnea and newly occurring skin irritation is to be seen as particularly important. A good outcome can be achieved by immediate therapy with immunosuppression and plasmapheresis.

We report a case of severe COVID-19 pneumonia complicated by fatal co-infection with a multi-triazole resistant Aspergillus fumigatus and highlight the importance of recognising the significance of Aspergillus sp. isolation from respiratory samples. Early diagnosis and detection of triazole resistance are essential for appropriate antifungal therapy to improve outcome in patients with coronavirus associated invasive aspergillosis.

We describe a patient with compensated cirrhosis and portal hypertension who underwent continuous flow LVAD implantation. Shortly after LVAD implantation, the patient developed new onset bleeding esophageal varices and ultimately had a fatal outcome. Our experience suggests that even well-compensated cirrhotic patients with significant portal hypertension are at risk of variceal bleeding after LVAD placement.

The purpose of this report is to describe two cases of human rabies in Senegal that illustrate possible diagnostic and therapeutic pitfalls even in an endemic area. Although outcome is almost always fatal and interhuman transmission is uncommon, prompt diagnosis of rabies is important since delay increases the risk of exposure to the virus for the entourage.

Elective transcatheter closure of congenital septal defects has emerged as a valuable method, but the clinical experience on occlusion of ventricular septal rupture after myocardial infarction is very limited. We report a case of fatal outcome in a patient with inferior myocardial infarction and cardiogenic shock despite technically successful transcatheter closure of a large complex ventricular septal defect.

Diabetic ketoacidosis (DKA) is a medical emergency with a potentially fatal outcome if not recognized and treated appropriately. Infective processes are a common precipitant of DKA. We report two cases of dentoalveolar infections in patients with type I diabetes mellitus who presented with DKA. The management of such cases requires both specialist surgical and medical intervention.
