Posterior tibial nerve stimulation for faecal incontinence.

Faecal incontinence is a common multifactorial condition with a range of invasive treatment options, all of which may be associated with significant complications. Posterior tibial stimulation by continence nurses is an established treatment for urinary incontinence; however, its use in faecal incontinence, while rapidly evolving, is limited to eight small and differing studies. In this article, the background of current management options for faecal incontinence is discussed, as are the physiology and evidence underlying neuromodulation. The evidence base for posterior tibial nerve stimulatio

Evaluation and treatment of anal incontinence, constipation, and defecatory dysfunction.

Posterior compartment disorders include anal incontinence, constipation, and defecatory dysfunction. These disorders cause considerable morbidity, and are typically underreported by patients and undertreated by providers. The purpose of this article is outline the approach to diagnosis and treatment of anal incontinence, constipation, and defecatory dysfunction with a brief description of the nature of the problem and approaches to evaluation and diagnosis, as well as medical and surgical management.

Incontinence rates after cutting seton treatment for anal fistula.

OBJECTIVE: To determine the incidence of anal incontinence after the use of cutting seton treatment for anal fistula. METHOD: Literature searches were performed on PubMed, MEDLINE and Google Scholar using the words 'cutting seton(s)', 'seton(s)' and 'anal fistula'. An analysis of the data in the collected references was performed. RESULTS: The average rate of incontinence following cutting seton use was 12%. The rate of incontinence increased as the location of the internal opening of the fistula moved more proximally. In the studies that described the types of incontinence, liquid stool was

Faecal incontinence: what influences care and management options?

Since the development of guidelines for faecal incontinence in adults by the National Institute for Health and Clinical Excellence guidelines in June 2007, there now appears to be more of a focus in primary and secondary care on the management patients with faecal incontinence. Faecal incontinence has a devastating effect on peoples quality of life but they often do not seek help due to the embarrassing and taboo nature of the problem. Therefore, it is essential that the healthcare professional who they eventually approach have the skills, knowledge and experience to undertake a thorough clini

Tapered terminal ileum conduit for antegrade continence enemas.

Antegrade continence enemas can be used to control fecal incontinence caused by neurogenic bowel. Creating a conduit can become a problem when the appendix is unsuitable for use. This paper describes a procedure using a tapered terminal ileum conduit for use with antegrade continence enemas in place of the appendix. This procedure has been attempted and successfully completed on two pediatric patients with good results at 6-month follow-up. The tapered terminal ileum conduit is an option for creating a conduit for antegrade continence enemas in patients without a suitable appendix.

[Technical aspects and indications of anorectal manometry].

Anorectal manometry is an important tool in testing anorectal disorders. Water-filled balloons is the most commonly used. The step-by-step anorectal manometry using a small balloon tube is easy to perform, well standardized and reproducible. Parameters studied by anorectal manometry are the rectoanal inhibitory reflex, anal resting pressure, sustained voluntary contraction of anal canal and rectal sensation. Anorectal manometry is also useful for diagnosis of anismus. The most important indications are anal incontinence, distal constipation and preoperative evaluation before sphincteroplasty o

Hemorrhoidectomy: indications and risks.

Hemorrhoids are a common cause of perianal complaints and affect 1-10 million people in North-America and with similar incidence in Europe. Symptomatic hemorrhoids are associated with nutrition, inherited predisposition, retention of feces with or without chronic abuse of laxatives or diarrhea. Increased pressure and shearing force in the anal canal may lead to severe changes in topography with detachment of the hemorrhoids from the internal sphincter and fibromuscular network resulting in bleeding, itching, pain and disordered anorectal function, even incontinence. The significance of hemorrh

Colorectal symptoms in patients with neurological diseases.

Several neurological diseases cause constipation or faecal incontinence restricting social activities and influencing quality of life. As several new treatment modalities have become available within the last few years, doctors treating patients with neurological diseases should be aware of the symptoms and have a basic knowledge of relevant treatment options. Constipation and faecal incontinence are common symptoms in patients with traumatic spinal cord injuries, spina bifida, multiple sclerosis, diabetic polyneuropathy, Parkinson's disease, stroke, and cerebral palsy. New treatment modalitie

Surgical repair of vulvar anus in adults.

Three adult women with previously unoperated vulvar anus underwent surgical treatment for faecal incontinence which developed in adulthood. The obvious cause of incontinence in these patients was the weakening of pelvic floor musculature by aging and pregnancy. Two of the patients had several associated anomalies. The anus was transposed to the normal perineal position through the well-developed external sphincter found posterior to the vulvar anus. Faecal continence improved markedly in all patients.

[Physiopathology of incontinence following treatment of anorectal malformations. Non-surgical treatments].

Attainment of continence after treatment of supra-levatorian ano-rectal malformations is frequently hampered by difficult problems. Cinedefecography and particularly conventional manometry along with myography allow to analyse the different factors involved in incontinence, in connection on the one hand with the type of gut pulled-down, and on the other hand with the sphincteric system. This study allows to propose, under specific circumstances, suitable techniques aiming at modifying either the gut contents, or the properties of the pulled-down bowel, or to reinforce the spincters, particular

Functional Gastrointestinal Disorders: Functional Lower Gastrointestinal Disorders in Adults.

Functional lower gastrointestinal disorders include irritable bowel syndrome (IBS), functional constipation, functional fecal incontinence, and functional anorectal pain. These disorders are common and have significant medical and social effects. They also can be challenging to manage. Patients with mild symptoms may benefit from lifestyle modification. IBS is classified into two subtypes: diarrhea-predominant and constipation-predominant. Depending on the IBS subtype and its likely etiology, patients may benefit from treatment with antispasmodics, antidepressants, guanylate cyclase-C agonists…

The results and complications of perineal rectosigmoidectomy for complete rectal prolapse in 114 patients have been reviewed. Most patients … rectal prolapse, six of them underwent repeat perineal rectosigmoidectomy. Sixty-seven patients had fecal incontinence prior to surgery. Eleven patients underwent concomitant levatoroplasty; 10 of them either improved or regained full continence of feces postoperatively. Twenty-six of the 56 patients who underwent perineal rectosigmoidectomy alone improved or regain …
