Acetabular growth potential in congenital dislocation of the hip and some factors upon which it may depend.

The factors which determine the rate and amount of acetabular growth in congenital dislocation of the hip are ill-understood. A prospective radiological study has been devised in order to assess the influence of the age of congruity, femoral anteversion and neck/shaft angles on acetabular growth. To be included, the children must have reached at least 8 years of age. Those with associated congenital abnormalities and subluxation were excluded, as were those who had operations for acetabular reconstruction. Seventy-four hips were suitable for analysis, and the age range at follow-up was from 8

Biomechanical comparison of a 2 and 3 proximal screw-configured antegrade piriformis intramedullary nail with a trochanteric reconstruction nail in an unstable subtrochanteric fracture model.

OBJECTIVE: The objective of this study was to test the stiffness and ultimate load to failure of new intramedullary (IM) nail proximal screw configurations as compared to a trochanteric reconstruction nail. METHODS: Twenty-one synthetic composite femurs were mounted on a Material Testing System and tested in axial compression 5 times. The femurs had an 1 of 2 IM nail types inserted with 1 of 3 proximal screw configurations: a 3-screw configuration with 2 transverse screws and a screw angled into the femoral neck; a 2-screw design with a single transverse screw and a single screw angled into t

Rare progression of cholangiocarcinoma: distal femoral metastasis.

Cholangiocarcinoma (CCA) is a rare cancer of the bile duct epithelial cells, with metastases to long bones rarely reported. We present the case of a woman aged 61 years, diagnosed with CCA 6 years previously. She presented with right thigh discomfort, rapidly progressing to immobility with severe pain. Radiological investigations revealed a lytic lesion in her right distal femur, most likely a metastatic deposit, at high risk of pathological fracture. This was managed with an en bloc resection and implantation of a distal femoral replacement. Pathology specimens from theatre confirmed metastat

Amputation through the hip joint during the pre-anaesthetic era.

Even to the present day, hip disarticulation is one of the most radical orthopaedic operations. First carried out on the Continent in 1774 and in Britain in 1779, this procedure was only deemed necessary in circumstances where death was otherwise inevitable. George James Guthrie claimed to have undertaken the first amputation through the hip joint in which the patient survived. We describe some of these cases from the pre-anaesthetic surgical literature in which amputation through the hip joint was undertaken, and we comment on the surgical techniques used. This information is complemented by

Intramedullary nails: some design features of the distal end.

Intramedullary nails are used to stabilise fractures of the proximal femur. The nail acts by transferring loads from the proximal fraction to the rest of the femoral shaft. The way in which this occurs depends to a large extent on the design of the distal end of the nail. This is not dissimilar to the situation with regard to load shedding (or load transfer) from the femoral component of a total hip replacement. A finite element model of a fractured femur with either a neck or a subtrochanteric fracture is set up to investigate the effects of nail length, nail distal stiffness and material sti

["Prognostic" analysis of late results of trochanteric femoral osteotomy following post-traumatic malalignment].

Post-traumatic axial malalignment of the lower limbs causes osteoarthrosis of the adjacent joints. Whether or not correction osteotomy is indicated depends upon the degree and the location of the deformity. Malalignment of the proximal femur should be corrected if the axis of the leg does not pass through the knee joint. If the axis is outside the central half of the tibial head, the indication for correction osteotomy is a relative one: varus deformities are more difficult to compensate and it is more urgent that these be corrected. Of 103 patients who underwent osteotomies of the proximal fe

Spontaneous proximal migration of Kuntscher nail: an uncommon complication.

The Kuntscher nail (K-nail) was initially designed with an axial section in the letter 'V' shape. Still, it was later reintroduced in a cloverleaf shape to achieve additional strength and easier use. Migration of the K-nail proximally or distally in the femur is a well-documented complication, with distal migration reported as one of the complications in the 1940s. However, spontaneous extrusion of the K-nail proximally is a rare complication. The common causes of the extrusion include disuse/senile osteoporosis, infection, inappropriate K-nail size, delayed union with shortening and premature

Geometrical values of the normal and arthritic hip and knee detected with the EOS imaging system.

PURPOSE: EOS 2D/3D is an integrated, low-dose orthopedic digital radioimaging solution, which, due to its groundbreaking properties, has recently shown an increasing application in scoliosis surgery. Its integrated sterEOS 3D software allows creation of patient-specific three-dimensional (3D) lower limb models, and can produce geometrical parameters in 3D. Currently there are a limited number of reports on EOS for lower limb applications. METHODS: Three-dimensional reconstructions of 256 hip and knee joints of 128 healthy subjects, as well as 53 hips and 46 knees of 69 patients with hip or kn

Bone mineral density of the spine and femur in healthy Saudis.

The reference values of bone mineral density (BMD) were determined in healthy Saudis of both sexes and compared with US / northern European and other reference data. BMD was determined by dual-energy X-ray absorptiometry (DXA) at the lumbar spine and femur including subregions: trochanter, Ward's triangle, and neck, in 1,980 randomly selected Saudis (age range 20-79 years; 915 males and 1,065 females) living in the Jeddah area. Age-related changes in BMD were similar to those described in US / northern European and Lebanese reference data. Decreases in BMD of males were evident (% per year): 0

[Research progress of caput femoris posterior tilt and its impact on prognosis in nondisplaced femoral neck fractures].

There are still many unresolved problems in the treatment and prognosis of nondisplaced femoral neck fractures, such as nonunion and avascular necrosis of the caput femoris .In order to reduce the risk of various complications after non-displaced femoral neck fractures, the caput femoris posterior tilt of femoral neck fractures and its impact on prognosis have attracted more and more attention. A large number of scholars' studies have found that when the posterior tilt exceeds 20°, the risk of internal fixation failure increases significantly. Based on this concept, we can choose to use primar

Novel slide compression anatomic plates of the femoral neck for treating unstable femoral neck fracture: A biomechanical study.

To compare the biomechanical stability of slide compression anatomic plates of the femoral neck, cannulated compression screws and dynamic hip screws with derotation screws for stabilizing unstable femoral neck fractures (Pauwels angle = 70°). Pauwels III femoral neck fractures were created on 45 Sawbones femurs and randomly assigned to three implant groups (1:1:1). The biomechanical stability of all Sawbones in each treatment group was evaluated with three tests. First, in the static loading test, the load-displacement curve, vertical stiffness (load/vertical displacement [N/mm]) and 5 mm fai

Atraumatic Displaced Femoral Neck Insufficiency Fracture Because of Severe Hypocalcemia in a Pediatric Patient: A Case Report.

CASE: This is a case of a 14-year-old autistic boy who presented with an atraumatic transcervical femoral neck fracture in the setting of significant hypocalcemia and vitamin D deficiency. We discuss his surgical and medical management and metabolic derangements associated with atraumatic femoral neck fractures. CONCLUSION: Pediatric femoral neck fractures in the absence of trauma are uncommon and often have underlying metabolic abnormalities. In addition, autism poses unique challenges in caring for these patients who are at an increased risk of complications. Interdisciplinary care is integ

[Femoral neck stress fractures].

Femoral neck stress fractures are relatively rare and caused by repetitive high pressure on the bone with insufficient time to recover. These fractures are often seen in fanatic runners or military personnel, who cover great distances. Patients with a femoral neck stress fracture present with mild pain at the front of the thigh or groin. Radiological imaging includes a plain X-ray and/or MRI-scan. Differentiation between a tension- and compression type stress fracture is important, since their treatment strategies differ. Generally, tension type fractures are unstable and demand operative fixa

Treatment of Valgus-Impacted and Nondisplaced Femoral Neck Fragility Fractures in the Elderly.

As the life expectancy of the worldwide population increases, the number of hip fractures in the elderly cohort is expected to grow. It is important for surgeons to critically analyze available treatment options for these injuries, with the goal of optimizing outcomes and minimizing complications. Femoral neck fractures make up approximately half of all hip fractures. Nonoperative treatment of valgus-impacted and nondisplaced (Garden I and II) femoral neck fractures has high rates of secondary displacement, osteonecrosis, and nonunion; only patients with notable risk for perioperative complica

Comparison of femoral neck shortening and outcomes between in situ fixation and fixation after reduction for severe valgus-impacted femoral neck fractures.

INTRODUCTION: Severe valgus-impacted femoral neck fractures are associated with femoral neck shortening (FNS). However, no study has focused on the effect of reduction for severe valgus impaction in terms of healing and restoration of the femoral neck length. This study aimed to compare FNS and treatment outcomes of in situ fixation and fixation after reduction for severe valgus-impacted femoral neck fractures in patients aged 65 years or younger. METHODS: This retrospective study included 55 patients who underwent internal fixation with three parallel screws for femoral neck fractures with v
