Fetal, infant, and child mortality in a context of alcohol use.

Alcohol is an important element in the causal chain of risk for fetal, infant, and childhood mortality. Mortality risk is influenced by inte … e risk of mortality from abuse of mother/fetus, can cause other adverse outcomes, and can result in fetal alcohol spectrum disorders (FASDs). In infancy alcohol use can increase risk from impairment of arousal in adults who use alcohol. Infants with gestational exposure can be behaviorally difficult with sleep disturbance, irritability, and colic. This can increase the risk of ha …

Abnormal fetal heart rate pattern associated with severe intrapartum maternal ethanol intoxication. A case report.

Acute intrapartum ethanol intoxication occurred in a chronic ethanol abuser. Continuous fetal heart rate monitoring revealed a pattern of fetal tachycardia, decreased variability and late and variable decelerations in association with a maternal serum ethanol level of 300 mg/dL. This case gives evidence of an association between very high maternal serum ethanol levels and acute fetal distress.

Choline supplementation following third-trimester-equivalent alcohol exposure attenuates behavioral alterations in rats.

Despite the known adverse consequences of prenatal alcohol exposure, some pregnant women continue to drink alcohol, making it imperative to identify treatments for children with fetal alcohol spectrum disorders. The authors recently reported that perinatal choline supplementation can reduce some fetal alcohol effects (J. D. Thomas, M. Garrison, & T. M. O'Neill, 2004), and the present study examined whether choline supplementation is effective when administered after third-trimester-equivalent ethanol treatment. Rat pups were exposed to 6.0 g/kg/day ethanol during the neonatal brain growth spur…

Combined transcriptome analysis of fetal human and mouse cerebral cortex exposed to alcohol.

Fetal exposure to environmental insults increases the susceptibility to late-onset neuropsychiatric disorders. Alcohol is listed as one of such prenatal environmental risk factors and known to exert devastating teratogenetic effects on the developing brain, leading to complex neurological and psychiatric symptoms observed in fetal alcohol spectrum disorder (FASD). Here, we performed a coordinated transcriptome analysis of human and mouse fetal cerebral cortices exposed to ethanol in vitro and in vivo, respectively. Up- and down-regulated genes conserved in the human and mouse models and the bi

Paternal drinking, intimate relationship quality, and alcohol consumption in pregnant Ukrainian women.

OBJECTIVE: Maternal alcohol consumption during pregnancy and fetal alcohol spectrum disorders (FASDs) represent a significant public health problem. The influence of the male partner's alcohol consumption patterns and the quality of the partner's intimate relationship might be important factors to consider in the design of successful FASD prevention programs. METHOD: As part of the Collaborative Initiative on Fetal Alcohol Spectrum Disorders, 166 pregnant women in two regions in Ukraine participated in an in-person interview at an average gestational age of 18-19 weeks. Subjects were classifi…

Alcohol and the Developing Brain: Why Neurons Die and How Survivors Change.

The consequences of alcohol drinking during pregnancy are dramatic and usually referred to as fetal alcohol spectrum disorders (FASD). This condition is one of the main causes of intellectual disability in Western countries. The immature fetal brain exposed to ethanol undergoes massive neuron death. However, the same mechanisms leading to cell death can also be responsible for changes of developmental plasticity. As a consequence of such a maladaptive plasticity, the functional damage to central nervous system structures is amplified and leads to permanent sequelae. Here we review the literatu…

Heat shock factor 2 is a stress-responsive mediator of neuronal migration defects in models of fetal alcohol syndrome.

Fetal alcohol spectrum disorder (FASD) is a frequent cause of mental retardation. However, the molecular mechanisms underlying brain development defects induced by maternal alcohol consumption during pregnancy are unclear. We used normal and Hsf2-deficient mice and cell systems to uncover a pivotal role for heat shock factor 2 (HSF2) in radial neuronal migration defects in the cortex, a hallmark of fetal alcohol exposure. Upon fetal alcohol exposure, HSF2 is essential for the triggering of HSF1 activation, which is accompanied by distinctive post-translational modifications, and HSF2 steers th

Committee opinion no. 496: At-risk drinking and alcohol dependence: obstetric and gynecologic implications.

Compared with men, at-risk alcohol use by women has a disproportionate effect on their health and lives, including reproductive function and pregnancy outcomes. Obstetrician–gynecologists have a key role in screening and providing brief intervention, patient education, and treatment referral for their patients who drink alcohol at risk levels. For women who are not physically addicted to alcohol, tools such as brief intervention and motivational interviewing can be used effectively by the clinician and incorporated into an office visit. For pregnant women and those at risk of pregnancy, it is

Report from the CDC. Physician and allied health professionals' training and fetal alcohol syndrome.

Maternal prenatal alcohol use is one of the leading preventable causes of birth defects and developmental disabilities. On the severe end of the spectrum of conditions related to drinking during pregnancy is fetal alcohol syndrome (FAS). Physicians and other health practitioners play a critical role in diagnosing FAS and in screening women of childbearing age for alcohol use during pregnancy. The Fetal Alcohol Syndrome Prevention Team at CDC's National Center on Birth Defects and Developmental Disabilities awarded funds to four medical school partners (Meharry and Morehouse Medical Colleges, S

Defects of Cortical Microcircuits Following Early Exposure to Alcohol.

The interplay between excitatory pyramidal neurons and GABAergic interneurons is the basic building block of neocortical microcircuits and plays a critical role in carrying out higher cognitive functions. Cortical circuits are deeply and permanently disrupted by exposure to alcohol during brain development, the main non-genetic cause of intellectual disability. Here, I review experimental studies of fetal alcohol spectrum disorders, dealing with permanent cellular and molecular alterations of neocortical neurons and their connections.

[Effect of teturam on the development of the offspring of alcoholized animals].

During experiments on rats it was found that in alcoholized animals teturam on the whole did not potentiate and in some cases even attenuated the toxic effect of alcohol on the offspring development. The data confirm the idea about necessity of studying toxicity of drugs under the conditions corresponding to their clinical use.

[Penetration and accumulation of 14C-ethanol in the brain and liver of female rats exposed to long-term alcohol and in the tissue of the fetus].

In their experiments on female rats which had been given 10% ethanol for 10 months the authors showed the characteristics of 14C-ethanol accumulation in the structures of the brain and liver of the animals and changes in these parameters during pregnancy. The penetration and accumulation of 14C-ethanol in the brain and liver of fetuses of chronically alcoholized animals were found to be different.

Zinc status of pregnant alcoholic women: a determinant of fetal outcome.

The zinc status of 25 alcoholic and 25 non-alcoholic pregnant women was measured and correlated with fetal outcome. The volume index of alcohol consumption and the Michigan Alcoholic Screening Test were used to define alcoholic women. The alcoholic women had significantly lower plasma-zinc levels that the non-alcoholic women (50.7 versus 72.2 micrograms/dl). Fetal cord-plasma zinc levels were also lower in the offspring of alcoholic women (65.5 versus 81.3 micrograms/dl). Manifestation of fetal dysmorphogenesis appears to relate to zinc status.

A mouse model of prenatal ethanol exposure using a voluntary drinking paradigm.

BACKGROUND: The incidence of fetal alcohol spectrum disorders is estimated to be as high as 1 in 100 births. Efforts to better understand the basis of prenatal ethanol-induced impairments in brain functioning, and the mechanisms by which ethanol produces these defects, will rely on the use of animal models of fetal alcohol exposure (FAE). METHODS: Using a saccharin-sweetened alcohol solution, we developed a free-choice, moderate alcohol access model of prenatal alcohol exposure. Stable drinking of a saccharin solution (0.066%) was established in female mice. Ethanol then was added to the sacc…

Impact of ethanol on the developing GABAergic system.

Alcohol intake during pregnancy has a tremendous impact on the developing brain. Embryonic and early postnatal alcohol exposures have been investigated experimentally to elucidate the fetal alcohol spectrum disorders' (FASD) milieu, and new data have emerged to support a devastating effect on the GABAergic system in the adult and developing nervous system. GABA is a predominantly inhibitory neurotransmitter that during development excites neurons and orchestrates several developmental processes such as proliferation, migration, differentiation, and synaptogenesis. This review summarizes and br…
