Treatment of mallet finger deformity with a modified palmaris longus tendon graft through a bone tunnel.

OBJECTIVE: To investigate the clinical effect of treating mallet finger deformity using a modified palmaris longus tendon graft through a bone tunnel. METHODS: Altogether, 21 patients with mallet finger deformity (16 men, 5 women; average age 31 years, range 19-47 years) were treated with a modified palmaris longus tendon graft through a bone tunnel during 18 months (2014-2016). Four index fingers, seven middle fingers, eight ring fingers, and two little fingers were treated for four cutting injuries, eleven finger sprains, four crush injuries, and two twist injuries (7 open and 14 closed inj

Vascularized autogenous whole joint transfer in the hand--a clinical study.

Nine vascularized autogenous whole joint transfers were employed in the reconstruction of traumatized joints of six male patients ages 6 to 38 years. The proximal interphalangeal joint was involved in four patients, the thumb metacarpophalangeal (MP) joint in two, and the small finger MP joint in one. In five patients, the donor joint came from the foot, and in one patient an MP joint was transferred from a digit that had sustained a more distal amputation. The mean follow-up was 24 months (range 13 to 38). Bony union as well as full radiographic preservation of the articular space has occurre

Metacarpophalangeal joint arthroscopy: indications revisited.

Arthroscopic surgery has become the gold standard for the diagnosis and treatment of major joint disorders. With advancement in arthroscopic technique, arthroscopy has become feasible in most human joints, even those as small as the finger joints. The metacarpophalangeal joint (MCPJ) can become spacious with simple traction, the intra-articular anatomy is simple, and its major structures can be easily visualized and identified. However, MCPJ arthroscopy has never been popular. This article describes our experience with MCPJ arthroscopy and seeks to establish its role in clinical practice.

The use of acellular dermal matrix for coverage of exposed joint and extensor mechanism in thermally injured patients with few options.

INTRODUCTION: One of the most devastating complications of deep burn injuries to the hand and finger is the exposure of joint, tendon, and neurovascular structures. The inevitable consequence of such injuries is severe deformity, often requiring joint fusions and digital amputations. Complicating this scenario is the anatomic limitation of few local and reliable soft tissue flaps available for this intricate distal distribution. This is particularly true for the patient who has suffered very large and deep thermal injuries. METHODS: This series of cases describes the use of thin and meshed ac

Kinesiological study of push-up motion using a three-dimensional floor reaction on a force plate.

To find an effective way to handle wheelchairs, 3-dimensional floor reactions of the hand and angular deviation of the elbow and wrist joints during push-up motion were studied in 10 healthy men. The push-up was carried out using 3 hand positions (fist, finger and palm) and a push-up device. In all hand positions, anteroposterior force (Fx) and the mediolateral force (Fy) appeared after the vertical force (Fz). The end point of Fx and Fy was observed before that of Fz. Among the 4 different hand positions, Fx and Fy appeared first in the palm, followed by the finger and fist positions, and las

[Soft tissue changes of fingers in rheumatoid arthritis. Results of low k.v. radiographs in three views (author's transl)].

Based upon a large series of low k.v. radiographs of fingers in three views, the X-ray anatomy of the soft tissue of fingers is presented and soft tissue changes in joints, tendons and tendon sheaths due to rheumatoid arthritis as well as perifocal effects of the inflammatory process upon the subcutaneous tissue and the skin are described. Because of the resolution of even minute soft tissue changes it is possible to recognize the incipient stage of rheumatoid arthritis and of relapses devoid of clinical symptoms.

Dupuytren's Disease of the Distal Interphalangeal Joint: A Systematic Review of Case Reports and Case Series.

Background and Objectives: Dupuytren's disease (DD) most commonly affects the palm and metacarpophalangeal/proximal interphalangeal joints; distal digital involvement at the distal interphalangeal joint (DIPJ) is uncommon and incompletely characterised. This systematic review summarises reported cases of DD involving the DIPJ, with or without proximal interphalangeal joint (PIPJ) involvement, focusing on clinical patterns, management, and reported outcomes. Materials and Methods: A systematic search of PubMed, Embase, and Scopus from database inception to July 2025 identified published case re

Wrist-driven prehension prosthesis for amputee patients with disarticulation of the thumb and index finger.

Partial hand amputations may leave significant functional limitations that are difficult to ameliorate by either orthoses or prostheses. Disarticulation of the thumb and index finger at the metacarpophalangeal joint level and the range of motion of the three remaining fingers would be insufficient to provide any type of grasp. In cases like this, a cosmetic hand usually seems to be the best solution. This report describes the design and use of a wrist-driven prehension prosthesis that was applied to a patient and provided a considerable improvement in function and cosmesis.

The articular manifestations of progressive systemic sclerosis (scleroderma).

The articular manifestations of progressive systemic sclerosis (PSS) were studied in 38 patients. Of these, 66% experienced joint pain and 61% had signs of joint inflammation. Limitation of joint movement was seen in 45%. Radiological abnormalities included periarticular osteoporosis (42%), joint space narrowing (34%), and erosions (40%). Erosive disease did not correlate with disease duration, presence of rheumatoid factor, antinuclear antibodies, distal tuft resorption, or the extent of the scleroderma skin changes. Calcinosis was seen more frequently in those patients with articular erosion

Increased variability in finger position occurs throughout overarm throws made by cerebellar and unskilled subjects.

We investigated the ability of cerebellar patients and unskilled subjects to control finger grip position and the amplitude of finger opening during a multijoint overarm throw. This situation is of interest because the appropriate finger control requires predicting the magnitude of back forces from the ball on the finger throughout the throw and generating the appropriate level and rate of change of finger flexor torque to oppose the back force. Cerebellar patients, matched controls, and unskilled subjects threw tennis balls and tennis-sized balls of different weights. In all cases angular pos

Dorsal carpometarcarpal dislocation of ulnar fingers. A case report.

Multiple dorsal dislocations of the carpometacarpal joints are rare. Diagnosis requires a high index of suspicion, careful examination and good radiography. Treatment is controversial and is based on the presence or not of associated fractures. This paper presents a case of combined fifth and fourth finger carpometacarpal joint dorsal dislocation, successfully treated with closed reduction and percutaneous pinning. A discussion of this pattern of injury is also presented.

Swan-neck deformity as a complication of the Agee technique.

Unstable dorsal fracture dislocations of the PIP joint of a finger commonly result in joint stiffness following immobilization or open reduction and internal fixation (Green and Rowland, 1984). The Agee dynamic external fixator, or force couple splint (Agee, 1978; 1987), was introduced in an attempt to avoid this complication and maintains a concentric reduction whilst allowing a full range of joint movement. The splint is constructed from three Kirschner wires and is activated by a single rubber band. A force couple is created across the proximal interphalangeal joint levering the base of the

Diagnosis and clinical presentation of osteoarthritis.

Osteoarthritis (OA), the commonest arthropathy, targets the knees, hips, finger interphalangeal joints, thumb bases, first metatarsophalangeal joints, and spinal facet joints, and displays marked heterogeneity of clinical presentation. Signs of OA include coarse crepitus, bony enlargement, reduced range of movement, and joint-line tenderness. Muscle wasting and joint deformity occur with severe OA. Painful periarticular disorders often coexist with OA. Inflammation is absent or only modest, although mild-moderate effusions are common at the knee. The diagnosis of OA may be made without recours

Hemophilic pseudotumor in the hand: a case report and literature review.

We describe the case of an 11-year-old boy with hemophilia B who presented with swelling and pain in the left index finger. The patient was referred to our hospital after first presenting to another hospital. The patient experienced slight difficulty making a fist because of limited flexion of the proximal interphalangeal joint. Radiography of the left index finger revealed an expansile remodeling osteolytic lesion with a well-defined sclerotic border and cortical thinning. The patient had prolonged activated partial thromboplastin times and decreased factor IX levels. We performed curettage o

Thumb reconstruction by digital transposition.

In traumatic loss or congenital aplasia of the thumb, transposition of a digit on its neurovascular pedicle is the preferable method for thumb reconstruction. The index finger is the digit of choice, but any other finger or a portion of a finger can be used, especially if it is damaged. The operative techniques differ considerably, depending on the level of thumb loss. In total amputation or total aplasia all the parts of the new thumb have to be constructed with the structures of the transposed index finger. The skeletal readjustment consists of shortening in the metacarpal region, rotation o

Open Reduction and Internal Fixation of Dorsal Fracture-Dislocation of the Distal Interphalangeal Joint Using a Low-Profile Mini-Plate: A Report of Three Cases.

Although dorsal fracture-dislocations of the distal interphalangeal (DIP) joints of the fingers are relatively rare injuries, if left untreated, functional disabilities of the DIP joints persist. Thus, reducing the dislocation and reconstructing the articular surface at the base of the distal phalanx is crucial to allow early DIP joint motion. This study evaluates the outcomes of three patients with dorsal fracture-dislocations of the DIP joints. Two patients had a split-depression type fracture and one had an impaction type fracture, and articular involvement averaged 55.7%. They were treated

Case Report: Treatment of a complex hand disfigurement injury by fully shaped reconstruction of a severed finger combined with ectopic banking.

One of the primary challenges in hand microsurgical reconstruction lies in addressing severe hand injuries, particularly those involving multiple finger amputations, as autologous replantation might not fully restore hand functionality. In such scenarios, fully shaped reconstruction of a severed finger combined with Ectopic banking could yield superior reconstructive outcomes and enhance the amputated limb's function. This case report presents a unique approach that combines ectopic replantation of an amputated finger with interphalangeal reconstruction methods to restore both the form and fun

Hemihamate Arthroplasty Revisited: A Retrospective Review of 11 Cases.

PURPOSE: Proximal interphalangeal joint (PIPJ) fracture dislocations are complex injuries that can result in persistent pain, stiffness, and angulation. Hemihamate arthroplasty (HHA) can be used to reconstruct the base of the middle phalanx in cases of unstable PIPJ fracture dislocations. Despite previous case series describing good outcomes with HHA, it has not gained widespread use. The purpose of this study is to describe our straightforward, reproducible technique and to demonstrate the benefit in motion after the procedure in chronic unstable PIPJ fracture dislocations. METHODS: All pati
