[Treatment of pararectal fistulas].

We observed 120 patients with acute and chronic paraproctitis. In 75 of them, the pararectal fistulas were revealed. The most frequent causes of a pararectal fistula recurrence were the incomplete excision of the necrotized fat, inadequate wound drainage and incorrect definition of the internal fistulous opening.

Management of enterovesical fistulas.

The records of 51 patients diagnosed with enterovesical fistulas at Virginia Mason Medical Center from 1974 to 1988 were reviewed. Diverticulitis (41%), Crohn's disease (17%), and colorectal cancer (16%) were the major causes. In 50 of 51 patients, the diagnosis was made on the basis of the clinical history and the urine culture. Radiologic and endoscopic studies failed to identify the fistula in 20%, though all were confirmed at operation or autopsy. In four of eight patients with fistulas secondary to colorectal cancer, malignancy was not diagnosed preoperatively. Operation was performed in …

Corneal fistulas and their management.

We reviewed three representative cases of chronic corneal fistula formation and provide a systematic approach to the assessment and therapeutic alternatives for this problem. In two of our patients, the fistula was managed surgically. The third patient developed endophthalmitis, which resulted in loss of light perception. Chronic corneal fistulization is a rare clinical entity resulting from malapposition of corneal tissue after traumatic, surgical, or infectious perforation. Fistulas may result in prolonged or recurrent hypotony, peripheral anterior synechia formation, or endophthalmitis. Acc…

Graft appendiceal fistulas.

We report the fifth case of a graft appendiceal fistula. The four previously reported cases are reviewed. This is the first report of the utility of the tagged red blood cell study in the evaluation of a patient with a graft appendiceal fistula.

Bronchobiliary fistulas in adults.

Reported herein are 16 cases of nontraumatic bronchobiliary fistula in adults observed over 32 years. The fistulas were due to either biliary obstruction or infectious diseases involving the liver. All patients had biloptysis and other respiratory symptoms, and most had cholangitis, jaundice, and/or sepsis. The diagnosis was established by fistulography or cholangiography. All patients except one were treated surgically by relief of biliary obstruction and drainage of intra-abdominal abscesses. All 15 surgically treated patients had a successful outcome. Rapid diagnosis and early treatment of …

Complex anal fistulae.

Complex anal fistulas challenge the judgment and operative skills of the surgeon in effecting a cure without compromise of anorectal function. This chapter outlines the classification, pathogenesis, methods of investigation and alternatives of management of a variety of complex fistulas. Also reviewed are the advantages and disadvantages of both traditional and newer methods of management, with special reference to staged fistulotomy and advancement rectal flap procedures.

[Treatment of anal fistulas].

It is generally accepted that laying-open constitutes the only effective cure for anal fistulae. The sacrifice of at least some part of the anal sphincter apparatus is therefore the inevitable consequence of every operation for fistula. In the case of an intersphincteric or low-trans-sphincteric fistula, only the internal sphincter or the lower half of the external sphincter need to be incised. This may usually be performed without endangering postoperative faecal continence. High trans-sphincteric or suprasphincteric fistulae, however, involve the entire external sphincter and/or the puborect…

CT of aortoenteric fistulas.

We compared CT findings with endoscopic, angiographic and surgical results in ten patients suspected of having an aortoenteric fistula (AEF) because of gastrointestinal bleeding (seven) or recurrent sepsis (three). CT correctly diagnosed AEF in six patients and excluded it in the other four. CT findings of AEF consisted of perigraft fluid (PGF) (5/6) and/or gas within the bed of the graft (4/6) later than three months after graft surgery. All six patients with AEF had perigraft infections; PGF with gas was found in 50%, PGF alone in 33%, and in one patient perigraft gas alone was found. Angiog…

Anterior horseshoe fistulae.

A series of anal fistulae is presented in which a high proportion were anterior horseshoe fistulae. The incidence of this type of fistula in this series is higher than that previously reported. It can be recognised by a study of the position of the track by palpation, and the distance from the anal verge of the external opening. A meticulous dissection at operation is required. In order to get good results it is important that this clinical entity be recognised and managed with the same care and attention that is required for a posterior horseshoe fistula.

Colovesical and rectovesical fistulas.

Colovesical or rectovesical fistulas develop because of a congenital abnormality, a traumatic incident, or an underlying disease, such as inflammation or cancer. Inflammation, either diverticulitis or Crohn's disease, was the cause of fistula in 73 (67%) of 109 patients with colovesical or rectovesical fistulas seen at the Mayo Clinic, Rochester, Minn, between 1965 and 1980. Sixty-six of the 73 patients had colovesical fistulas, and 64% of them underwent resection and anastomosis in one stage. Cancer of the cervix, prostate, bladder, colon, or rectum was responsible for 27 fistulas, equally di…

Pulmonary arteriovenous fistulas.

During the 20-year-period 1959-78, 7 patients with pulmonary arteriovenous fistula were treated at the Department of Thoracic and Cardiovascular Surgery. There were 5 men and 2 women, with a mean age of 26 (14-47) years. Cyanosis with elevated haematocrit was present in 4, dyspnoea in 4, neurological signs in 3 (including one brain abscess and one hemiplegia). Systolic hum was audible in 3 cases. Three patients had the hereditary type of the disease (Rendu-Osler-Weber) with telangiectasiae also elsewhere in the body. The calculated right-to-left shunt varied from 14 to 56 per cent of the cardi…

[Complex vesicovaginal fistulas].

The author reviews the therapeutic indications (it being necessary to abstain from repair of certain fistulae) in vesico-vaginal fistulae. Fistulae following pelvic irradiation, african fistulae of obstetric origin and the various difficulties (and their possible solutions) which may arise when the fistula passes through an extensive zone of fibrous tissue, is close to a ureteric orifice, at the neck of the bladder or associated with a loss of the urethra or with a recto-vaginal fistula, are studied. In the opinion of the author, classification between simple and complex vesico-vaginal fistula…
