Treatment of fractures and dislocations of the thoracic and lumbar spine by fusion and Harrington instrumentation.

Thirty-three patients with fractures of the thoracolumbar spine were treated by fusion and Harrington instrumentation after early reduction and stabilisation by postural reduction or halo-bifemoral traction. The rod-long-fuse-short technique was used. Postoperatively, all patients were mobilised with an external support. The mean follow-up was 6 years (range 1-13 years). Twenty-eight patients were pain-free, three patients needed sporadic pain medication. One patient was not working due to pain in his leg. One patient had constant pain. Twenty-three patients returned to work. The mean kyphosis

Fracture of the posterior process of the talus associated with subtalar dislocation: report of a case.

Although subtalar dislocation is not common in foot trauma, fracture of the entire posterior process of the talus is even rarer. The concomitant injury of a subtalar dislocation associated with a fracture of the posterior process of the talus has not yet been reported in the medical literature. A 52-year-old woman involved in a traffic accident had her right foot twisted into an inversion deformity. An X-ray revealed a medial subtalar dislocation associated with a fracture of the entire posterior process of the talus. Tomography demonstrated that the posterior process was incongruent to the an

Anterior decompression and plate fixation in fracture dislocations of the lower cervical spine.

Surgical treatment of unstable traumatic injuries of the cervical spine can be carried out by a posterior or anterior approach, with different advantages and disadvantages. Twenty patients were treated with anterior decompression, interbody fusion with autogenous iliac bone graft, and osteosynthesis with a Louis anterior plate. The screws were inserted in the vertebral body without reaching the posterior vertebral wall. There were 18 male and 2 female patients, aged between 18 and 66 years (average 36 years). The osteoarticular lesion was in 8 cases a tear-drop fracture and in 12 a fracture-di

[Injuries of major blood vessels of the lower extremities as a complication of fractures and dislocations].

Surgical treatment was applied in 46 patients with simultaneous injury of great vessels of the lower extremities and fractures of the long bones or dislocations of the knee joint. The authors have determined the best in their opinion sequence of surgical procedure. They think that external fixation gives better bone stabilization and is more favorable than internal stabilization. They also recommend lavage of the peripheral vascular bed of the ischemic lower extremity, which prevents post-traumatic insufficiency of the kidneys. They think that early fasciotomy of the crus or fibulectomy is imp

Open reduction of the dislocated, fractured condylar process: indications and surgical procedures.

The subject of this paper is the evaluation of the results after surgical management of 29 dislocated fractures of the condylar process. Only fractures with total dislocation of the condyle out of the articulate fossa were surgically treated. The surgical procedure, making a broad exposure necessary, is detailed. The low rate of complications as well as the satisfactory function of the joints in spite of severe dislocation seems to confirm the surgical treatment of this type of fractures.

Posterior acetabular fracture-dislocations: fragment size, joint capsule, and stability.

In acetabular fractures, the size of a significant posterior wall fragment remains undefined as it affects joint stability. The purpose of this study was to quantitatively evaluate fragment size and hip stability in cadaveric specimens after serial osteotomies. Also, the role of the posterior capsule, in the various osteotomies, was evaluated for changes in hip stability. We found that fragments involving 25% or less of the acetabulum are insignificant, i.e., do not affect joint stability, while fragments involving 50% or more are significant. The significance of transitional fragments (25-50%

Anterior perineal dislocation of the hip with fracture of the femoral head. A case report.

Traumatic anterior perineal hip dislocation with an associated fracture of the femoral head is a rare entity. A 26-year-old man injured in a motorcycle accident was treated by closed reduction of the dislocation within three hours after admission. However, several reports of patients with anterior hip dislocation with associated femoral head fractures were treated nonoperatively and had unfavorable results when treatment failed to achieve anatomical position of the fragments. Consequently, this patient was treated by open reduction and internal fixation of the fractured fragment. Follow-up exa

Traction bow for acute reduction of fracture and/or dislocation of the thoracic or thoracolumbar spine.

Treatment of acute fractures and/or fracture dislocations of the thoracic or thoracolumbar spine has traditionally involved bedrest or the use of traction devices with external hanging weights, until surgical correction can be accomplished. A fiberglass tubular traction bow with continuous adjustable elastic tension has been designed for the application of skeletal traction. When used to treat thoracic or thoracolumbar fractures and/or dislocations, it can maintain distraction forces in an uninterrupted fashion. Ten patients with acute fractures and/or dislocations of the thoracic or thoracolu…

[Fractures of the upper end of radius and ulna and recent dislocations of the elbow in children].

In this report concerned with luxation of the elbow and fractures of the upper end of two forearm bones in children emphasis is placed on the importance of the initial treatment, since sequelae are not corrected during growth. Correct therapy requires firstly a precise diagnosis, this depending on the obtaining of images of very good quality, if necessary taken during sleep if the child has to be anesthetized. Therapy should be as atraumatic as possible, particularly in the region of the head of radius, to avoid its subsequent necrosis.

Fracture dislocations of the tarsometatarsal joints: end results correlated with pathology and treatment.

Seventy-two patients with 76 fracture-dislocations of the Lisfranc tarsometatarsal joint complex were evaluated. Fifty-eight (81%) were polytrauma patients and the remainder suffered isolated injuries. Sixty of the original 72 patients were available for long-term study. Eight of these had an amputation at or shortly after the original admission, leaving 52 patients with 55 Lisfranc injuries for analysis. The average length of follow-up was 4.2 years (range, 20 months to 11 years). According to the Painful Foot Center scoring system, 27 feet (49%) achieved an excellent or good result and 28 (5

Revascularization of the humeral head. A report of two cases of fracture-dislocation of the shoulder.

Two cases of fracture-dislocation of the shoulder are reported in which the humeral heads were found at open reduction to be without soft tissue attachments. The patients were followed up clinically and radiologically and by technetium-99m scintigraphy for at least two years. In the case of a three-part fracture-dislocation, revascularization of the humeral head was complete three months after surgery, whereas the humeral head in the four-part fracture-dislocation did not appear totally revascularized scintigraphically until nine months after surgery. Radiologic signs of avascular necrosis of

[New trends in treating fractures and dislocations of the cervical spine (author's transl)].

Critical analysis of 50 patients suffering from fractures and dislocations of the cervical vertebral column proved that in most cases conservative as well as surgical treatment have satisfactory results. As long as no contraindications exist priority is given more to surgical procedures today.

Intravenous regional anesthesia for closed treatment of fractures and dislocations of the upper extremities.

Intravenous regional anesthesia for the treatment of fractures and dislocations of the upper extremity is a very effective, consistent and safe form of analgesia which requires low doses of lidocaine and can be performed in an emergency room using a regular blood pressure cuff. Dosage should be related to body weight and the blood pressure cuff should be maintained at higher than systolic pressure for a minimum of 15 minutes after the lidocaine is injected. Release of the tourniquet should be staged as described. Ninety-one per cent of 77 patients had excellent analgesia following the IVRA. Ei

Traumatic dislocation of hip joint with fracture of shaft of femur on the same side.

Four cases of traumatic dislocation of the hip joint with fracture of the shaft of femur on the same side, and one case of bilateral hip joint dislocation with a fracture of shaft of femur on one side are reported. Pitfalls in diagnosis and hazards of delayed treatment are emphasized. Methods of treatment are outlined. Complications such as avascular necrosis of the head of the femur and sciatic nerve palsy are discussed. Avascular necrosis of the head of the femur is not inevitable even after reduction of the joint has been delayed for several days.

[Traumatic dislocations of the hip with fracture of the femur head. Apropos of 28 cases].

Having reviewed 28 cases of fractures of the femoral head complicating traumatic dislocation of the hip the authors point out that the problems to be solved are the reduction of the dislocation (close or surgical) and the fate of the femoral head fragment. Close reduction succeeded in 12 cases, failed in 12 other cases, was not attempted 4 times. Open reduction was performed 16 times. When the femoral fragment was small, close reduction was attempted and the femoral head fragment neglected (10 cases). In three cases close reduction was attempted despite a big femoral fragment. These cases were
