The results of 57 A-O type A or C supracondylar femur fractures treated by open reduction and internal fixation using indirect reduction techniques are reported. No bone grafting or dual plating was used. All patients were placed in a continuous passive motion (CPM) machine postoperatively. Patients were followed at 4-week intervals until fracture healing had occurred. All patients were followed for at least 1 year after injury. All fractures were treated by a single surgeon. The average time for fracture healing was 10.7 weeks (range 8-16). Hardware failure did not occur in this series. Outco

Since absorbable internal fracture fixation devices made of polyglycolide have been increasingly used clinically, a peculiar type of complication has emerged. After an initially uneventful course, a local nonbacterial inflammatory reaction appears two to four months after the operation, resulting in a copiously discharging sinus on the skin. A series of 286 patients with unimalleolar or bimalleolar fractures were treated by open reduction and internal fixation using cylindrical rods made of polyglycolide. Among these there occurred 18 nonbacterial inflammatory tissue responses (6.3% of the tot

We report a new proximal to distal screw fixation technique for proximal pole fractures and non unions of the carpal scaphoid (type I of the Schernberg, Elzein and Gerard classification). Seven cases of non union with a live proximal polar fragment were operated from 1984 to 1986 with 4 excellent and 2 fair results and one failure. Three recent displaced fractures were also treated this way, with 3 excellent results. This useful technique is worth knowing and deserves a place in the treatment of scaphoid fractures, especially type I proximal pole fractures which represent a challenging problem

Thirty-eight patients in whom a fracture of the posterior malleolus was shown to comprise 25 per cent or more of the articular surface on the lateral radiograph were followed for an average of forty-four months (range, twenty-four to ninety-nine months). All injuries were judged to be a result of external rotation or abduction of the talus. Fifteen patients had fixation of the posterior malleolus, and twenty-three did not. Open reduction and internal fixation was carried out on all associated fractures of the medial and lateral malleoli. Satisfactory reduction of the posterior malleolus was of

Interfragmentary lag screw internal fixation was combined with external fixation to treat five difficult oblique lower extremity fractures. Uncomplicated primary bone healing was achieved in all five cases in spite of crush injury, devascularization, or local infection. This method represents a treatment option for the difficult oblique fracture and reconfirms the role of fixation in fracture biology.

Out of a series of 207 consecutive fractures of the tibial shaft, 102 were treated conservatively, 64 fractures were treated by AO compression plate osteosynthesis and 41 by internal fixation using Eggers or Lane plates. The choice of method was independent of the extent of soft tissue damage. A follow-up examination of 199 fractures, with a mean observation time of 3.4 years, revealed residual malalignment in 21 per cent of conservatively treated cases and in 8 per cent after conventional plate fixation, while the AO method resulted in anatomical restoration of the axis of the tibia in all ca

BACKGROUND: Paediatric pelvic and acetabular fractures are rare and usually the consequence of high-energy trauma, often associated with life-threatening injuries. The majority are managed non-operatively; however, open reduction and internal fixation (ORIF) is indicated in selected, complex, or displaced, acetabular fractures. The modified Stoppa approach is well established in adults, but has been rarely reported in skeletally immature patients, and evidence guiding surgical approach and fixation in children remains limited.
METHODS: We report the case of an 11-year-old girl who sustained a

INTRODUCTION: Radial head (RH) fractures are relatively uncommon injuries in children, typically resulting from a fall onto an outstretched hand, and can pose significant challenges in diagnosis and management, particularly when they are displaced or involve the articular surface.
CASE REPORT: A 12-year-old female sustained a comminuted displaced partial-articular fracture of the RH following a fall onto her arm during a netball game. Open reduction and internal fixation were performed using headless compression screws. Post-operative recovery was uneventful with a full range of motion at 3-m

UNLABELLED: An 89-year-old man presented with right elbow pain after a low-energy fall. At age 2 years, he sustained an untreated distal humeral fracture dislocation, which resulted in progressive remodeling and formation of a functional pseudoarticulation. Despite permanent deformity, he maintained stable functional motion for decades and performed heavy manual labor. Imaging revealed a displaced distal humeral fracture through the chronically remodeled elbow. Open reduction and internal fixation with a variable-angle locking plate was performed, while the pseudoarticulation was intentionally

BACKGROUND: Fracture-related infection (FRI) and subsequent persistent infections significantly affect fracture surgeries. We aimed to develop a precise, personalized risk calculator to assist orthopedic surgeons in evaluating perioperative FRI risk.
METHODS: Data from 36,087 patients across two medical centers and four regional hospitals were analyzed. We assessed 29 risk factors, including patient characteristics, comorbidities, fracture location, and surgical variables using multivariable logistic regression. Each factor was weighted based on its regression coefficient. Discrimination and

BACKGROUND: Surgical site infections are estimated to occur in 1.4 % - 5.5 % of all patients who undergo ankle fracture open reduction with internal fixation.
PURPOSE: While other studies only report on infection rates after ankle ORIF, this study evaluated the association between infection rates and operative factors.
STUDY DESIGN: We performed a retrospective cohort study of adults at Kaiser Permanente Northern California who underwent ankle fracture ORIF between January 1, 2010 and September 30, 2022.
METHODS: Patients were followed up to 6 weeks after initial surgery for return to the o

The clinical management of traumatic chest incisions accompanied by rib fractures presents the formidable challenge. The study was carried out to compare the outcomes of auscultatory triangle internal fixation (ATIF) and external fixation (EF) in such injuries. From June 2019 to June 2022, 105 patients with multiple rib fractures participated in the cohort study in which they were divided into two groups: 53 patients underwent ATIF and 52 patients underwent EF. The incidence of surgical site infection, wound healing time, incidence of wound dehiscence, number of dressing changes, pain as measu

Background Fractures of the proximal femur are amongst the most common injuries in the elderly population. While there is a clear consensus regarding the management of displaced femoral neck fractures, the management of non-displaced fractures is less clear. Both fixation and arthroplasty are valid treatment options. Internal fixation is a less invasive procedure, but it carries the risks of non-union and avascular necrosis (AVN) of the femoral head. The literature describes varying complication rates associated with these risks. We aim to describe a series of elderly patients above the age of

BACKGROUND AND PURPOSE: Fracture comminution occurs in 83.9%-94% of vertical femoral neck fractures (VFNFs), the majority of which were located in posterior-inferior region, and poses a clinical challenge in fixation stability. We conducted a subject-specific finite element analysis to determine the biomechanical features and optimal fixation selection for treating VFNF with posterior-inferior comminution.
PATIENTS AND METHODS: Eighteen models with three fracture types (VFNF without comminution [NCOM], with comminution [COM], with comminution + osteoporosis [COMOP]) and six internal fixation

BACKGROUND: The quality of reduction is an important factor affecting clinical outcomes for displaced femoral neck fractures (FNFs). However, concerns remain about the invasiveness of open reduction and internal fixation (ORIF) as compared to that of closed reduction and internal fixation (CRIF), and the choice between ORIF and CRIF as an optimal treatment strategy for displaced pediatric FNF remains controversial.
MATERIALS AND METHODS: MEDLINE, Embase, and the Cochrane Library were systematically searched for studies published up to December 22, 2022, that compared ORIF and CRIF techniques

Background: Schaztker II fracture is the most common type of the tibial plateau fractures (TPF). There has been a large number of cadaveric biomechanical studies and finite element simulation studies to explore the most stable fixation methods for this type of fracture, which were based on a single fracture morphology. But differences among fracture morphologies could directly affect the stability of internal fixation systems. In this sense, we verified the stability of existing internal fixation modalities by simulating Schatzker II fractures with different fracture morphologies. Objectives:

INTRODUCTION: Open reduction with internal fixation (ORIF) and intramedullary nailing (IMN) have similar union rates for treating humerus shaft fractures, but IMN leads to increased incidence of shoulder impingement and reoperation. The difference in 30-day readmission rate and length of stay (LOS) between these procedures is unknown. The objective of the study was to compare 30-day unplanned readmissions and length of stay between humeral shaft fractures fixed with either ORIF or IMN.
METHODS: The nationwide readmissions database (NRD) was queried for patients with humeral shaft fractures tr
