Although the optimal treatment for femur shaft fracture is intramedullary nailing fixation, nonunion still occurs. We determined the oligotrophic nonunion rate among femur fractures managed operatively and identified risk factors for reoperation. This was a retrospective clinical study. The data of the patients between 40 and 70 years old with diaphyseal femur fracture who have received reamed and interlocked intramedullary nailing fixation in our hospital from February 2014 to April 2018 were collected. They were followed at regular intervals for at least 1 year after the operation. The prima

BACKGROUND: Fractures in the proximal or distal third of the femoral shaft or those with severe comminution are known to be less suitable for intramedullary nailing with Kuntscher nail. With the introduction of interlocking intramedullary nailing the problems of rotational malalignment and shortening that accompany such fractures are now less common. At the National Orthopaedic Hospital Igbobi, Interlocking Intramedullary Nailing was introduced as a form of surgical treatment for fractures of the femur, our initial experience in this type of intramedullary nailing form the basis of this report

Most clavicle shaft fractures in children are nondisplaced, minimally shortened or angulated, and are well treated with nonoperative management. Indications for operative fixation include open fractures and impending open fractures. Relative indications for operative fixation include fractures in multi-trauma patients, floating shoulder injuries, comminuted fractures, and shortened fractures in older adolescents. Controversies exist in preferred methods of fixation and include superior or anteroinferior plating, locked or unlocked plating, and intramedullary fixation. Retrospective studies of

When stabilizing fractures with large soft tissue and bone defects, the primary concern is to avoid additional vascularization damage. Therefore, external fixation is still the standard method. In metaphysical fractures, joint transfixation should be avoided if possible. Concerning closed comminuted fractures of femoral and tibial shaft fractures, interlocking nailing shows the lowest complication rate. The introduction of unreamed nailing of open fractures shows the same low infection rate as external fixation, so it can be considered an alternative method. Early plate fixation is applied for

A 31-year-old motorcyclist presented with an open contaminated comminuted fracture of the shaft of the femur. The extruded diaphyseal segments were stored in a subcutaneous pouch in the thigh and subsequently replaced in the fracture site as a composite graft with iliac bone. The interval of in vito storage provided time for evaluation of infection and apparently allowed the bone to possess a limited degree of viability and function as a fresh autogenous graft.

BACKGROUND: Lateral locking plate fixation serves as an alternative to intramedullary nailing for femoral shaft fractures in patients with narrow medullary canals or existing implants. However, the optimal screw configuration—particularly working length and screw positioning—remains uncertain, especially in comminuted fractures where construct stiffness and interfragmentary strain are critical for healing. METHODS: Finite element femur models were developed based on Sawbones model. A total of 27 models were constructed by combining three bone quality conditions (high, medium, and low) with nin

INTRODUCTION: In cases of significantly displaced or comminuted mandible fractures, surgical guide splints can be developed to create the normal preinjury occlusion rather than placing patients in maxillomandibular fixation. Standard fracture fixation involves bending of plates intraoperatively based on surgeon-constructed dental splints which is prone to error.
METHODS: A 38-year-old male experienced a gunshot wound to the face that resulted in mandibular angle fractures bilaterally causing severely deranged occlusion and free-floating anterior mandible segment. Virtual surgical planning (VS

Fat embolism syndrome (FES) is a rare but serious multisystem syndrome that occurs after 0.9% to 2.2% of fractures, with long bone and pelvic fractures being the most common. The classic triad of FES consists of neurological impairment, respiratory insufficiency, and petechial rash, which develops 12-72 hours after the initial incident. We hereby present a case of a patient who developed persistent altered consciousness, seizures, and hypoxia secondary to a comminuted sacral fracture. Although the patient could not survive owing to multiple factors, imaging played a pivotal role in expediting

The reshaping of the nasal pyramid has evolved significantly over the past 20 years. Mechanical instruments have been refined to minimize the bone trauma of osteotomies. However, these instruments can inadvertently cause radiated fractures and comminuted fractures, which compromise bone stability and can create surface defects. Electric and piezoelectric instruments (PEI) have been developed to address these issues. PEI instruments are selective unlike electric instruments. These instruments allow for precise rhinosculpture and osteotomies under direct visual control. The article reviews the a

The blocking osteosynthesis is preferable in the case of open forearm fractures of the I degree. The treatment of patients with open comminuted complicated forearm fractures should be reasonable to perform in two stages: at the first stage, healing of soft tissues with fixation in the apparatus of external fixation should be obtained. At the second stage, the conversion of external fixator on interlocking nail with single-stage performance of osteoplasty as needed should be carried out. The presented experience of treatment of healing failure, pseudoarthroses and forearm bone defects by develo

Locked plating and retrograde nailing are two accepted methods for treatment of periprosthetic distal femur fractures. Each has relative benefits and potential pitfalls. Appropriate patient selection and knowledge of the specific femoral component geometry are required to optimally choose between these two methods. Locked plating may be applied to most periprosthetic distal femur fractures. The fracture pattern, simple or comminuted, will dictate the specific plating technique, compression plating or bridge plating. Nailing requires an open intercondylar box and a distal fragment of enough siz

The authors report their experience of the treatment of 31 gunshot open fractures in civilian practice. The fractures, comminuted in two thirds of cases, were produced by various fire-arms. Two vascular injuries were observed. Conservative treatment was performed in 80% of cases. 13 septic complications were observed (6 superficial and 7 deep infections). The were also 7 cases of delayed union, 3 mal unions and 7 cases of joint stiffness. To improve the prognosis of these lesions, the authors emphasize that surgeons must be familiar with ballistic wounds and carry out an early initial and appr…

Spiral or comminuted fractures of the femoral shaft in the vicinity of a prosthesis in the elderly patient have been encountered with increasing frequency and may be very difficult to treat. The use of a combination of multiple nylon cerclage straps and plates as described by Partridge is a satisfactory alternative to either conservative treatment in traction or a metallic internal fixation. The authors report their experience with this method in a series of 6 patients aged 74 to 91 years. The surgical trauma was minimal, early mobilization was possible, and the fractures united at a normal ra

Closed and Grade-I open multi-fragment, comminuted, and segmental fractures of the femoral and tibial shafts are well established indications for surgical stabilisation, using interlocking nailing. Eighty-two fractures of those categories in 78 patients were handled by the authors, between 1976 and 1987. High efficiency of the approach has been confirmed by early functional treatment, soon consolidation of fragments, within half a year, less complications as compared to other techniques as well as good functional end results which had been recorded in follow-up checks from 88 per cent of the p

Intramedullary fixation is the latest in a variety of techniques that have been developed to manage distal radius fractures. Intramedullary nailing of these fractures combines the soft-tissue advantages of a less invasive surgical approach with the biomechanical advantages of locking screw technology. These features may enable an accelerated postoperative rehabilitation and quicker return to function. Disadvantages associated with the intramedullary technique include the necessity of a closed or percutaneous reduction and the inability of the implant to adequately stabilize intraarticular or h

Subtrochanteric fractures of the femur comprise only 5% to 7% of hip fractures, but are important because of the difficulty in management. They combine the problem of instability to varus deformation common to comminuted intertrochanteric fractures and the problem of delayed union common to diaphysial fractures of the femur. While the benefits of open reduction and internal fixation in decreasing morbidity and mortality have been well established, formidable operative complications have occurred when this fracture is treated like an intertrochanteric fracture. Improved results have been obtain

The patella plays a crucial role in the extensor mechanism to increase the mechanical advantage of the quadriceps. Fractures can be classified based on displacement, comminution, and fracture pattern, which often guide treatment. Modern treatment options include internal fixation using tension bands with Kirschner wires or cannulated screws, lag screw fixation, partial patellectomy, and rarely total patellectomy. Nondisplaced, closed patellar fractures or fractures with less than 2-mm articular steps can be successfully treated conservatively. Open fractures, articular step of 2 mm or greater,

The successful use of external fixation to stabilize severe open fractures elsewhere in the body has led to its adaptation in the hand. It is particularly useful when extensive wounds prohibit internal fixation and when comminution and bone loss render the fracture difficult to control by other methods. External fixation permits simultaneous wound access and fracture management. It can be used alone or adjunctively with other methods of fixation such as Kirschner wires, screws, or plates for definitive fracture management. Alternatively, external fixation can be used for provisional fracture m

BACKGROUND: Even though the radiological and functional outcomes following femoral head fractures have been reported earlier, Quality Adjusted Life Years (QALY) after fixation of these fractures has not yet been studied. This study aims at a subgroup analysis of the Pipkins classification of fractures, radiological and functional outcomes, and the burden of femoral head fractures by QALY.
METHODS: A retrospective analysis of 101 patients with femoral head fractures was performed between 2008 and 2022 with a minimum follow-up of 2 years and maximum of 15 years. Serial radiographs were studied

BACKGROUND: Osteoporosis is a complex multifactorial disease characterized by reduced bone mass and microarchitectural deterioration of bone tissue linked to an increase of fracture risk. Fragility fractures occur in osteoporotic subjects due to low-energy trauma. Osteoporotic patients are a challenge regarding the correct surgical planning, as it can include fixation augmentation techniques to reach a more stable anchorage of the implant, possibly lowering re-intervention rate and in-hospital stay.
METHODS: The PubMed database and the Google Scholar search engine were used to identify articl
