Fourth-Generation Minimally Invasive Hallux Valgus Surgery With Guided Percutaneous System: Step-by-Step Surgical Technique.

Hallux valgus is a common condition in foot and ankle surgery practice. Nonoperative treatment is the first line of management in painful cases. Once these actions have failed, surgical treatment is indicated. Minimally invasive surgery has gained popularity owing to its favorable clinical and radiologic outcomes. Nevertheless, the learning curve of these procedures may be difficult in the initial cases. To solve this problem, some guided devices have been developed. The objective of this technical note is to describe 3-dimensional hallux valgus deformity correction surgery through a guided pe

Application value of metagenomics next-generation sequencing in the diagnosis of respiratory virus infection after congenital heart surgery.

BACKGROUND: Timely and accurate pathogen diagnosis can be challenging in children who contract a respiratory virus following congenital heart surgery (CHS). This often results in suboptimal drug use and treatment delays. Metagenomics next-generation sequencing (mNGS) is a swift, efficient, and unbiased method for obtaining microbial nucleic acid sequences. This technology holds promise as a comprehensive diagnostic tool, especially for pathogens undetectable by traditional methods. However, the efficacy of mNGS in the context of congenital heart disease infections remains uncertain. This study

Distinct profiles of cerebral oxygenation in focal vs. secondarily generalized EEG seizures in children undergoing cardiac surgery.

OBJECTIVES: Seizures are common in children undergoing cardiopulmonary bypass (CPB). Cerebral oxygen saturation (ScO2) by near-infrared spectroscopy is routinely monitored in many centers, but the relations between the levels and changes of ScO2 and brain injuries remain incompletely understood. We aimed to analyze the postoperative profiles of ScO2 and cerebral blood flow velocity in different types of EEG seizures in relation to brain injuries on MRI. METHODS: We monitored continuous EEG and ScO2 in 337 children during the first 48 h after CPB, which were analyzed in 3 h periods. Cerebral b

Techniques of parathyroid exploration at North American endocrine surgery fellowship programs: what the next generation is being taught.

BACKGROUND: Minimally invasive techniques are now often used to treat primary hyperparathyroidism but with uncertain conformity and some controversy. Endocrine surgery fellowships (ESFPs) have recently proliferated. METHODS: The directors of the 19 ESFPs recognized by the American Association of Endocrine Surgeons were polled to identify the approaches currently taught to trainees. RESULTS: With 100% participation, all ESFPs obtain ≥1 imaging study, and 95% use ultrasound to assess for concurrent thyroid nodules that require care. For an apparent single adenoma, all ESFPs minimize dissection

[Is the observation of patients with sleep-apnea-syndrome after surgery of the upper airway in an intensive care unit generally necessary?].

BACKGROUND: Although it is known that after surgery of the nose and/or the paranasal sinuses serious complications can arise for patients suffering from Sleep-Apnea-Syndrome (SAS), there exists no general recommendation for postoperative care of these patients. This retrospective analysis is dealing with the question whether it is generally necessary to observe SAS-patients after nasal surgery including intubation in an Intensive Care Unit (ICU). PATIENTS AND METHODS: 24 Patients of the ORL-Dept., Marienkrankenhaus Hamburg, suffering from SAS underwent surgery of the nose, the paranasal sinus

The impact of dexmedetomidine on postoperative delirium and early postoperative cognitive impairment in elderly patients undergoing general surgery: a systematic review and meta-analysis.

Anesthetic management in general surgery poses substantial risks in elderly patients because age-related physiologic decline and preexisting comorbidities increase vulnerability to peri-operative complications. Older adults have reduced physiological reserve, impaired organ function, and slower recovery from surgical and anesthetic stress, placing them at increased risk for postoperative delirium and early postoperative cognitive impairment. Dexmedetomidine, a selective α2-adrenergic receptor agonist, has sedative, anti-inflammatory, and opioid-sparing properties that may influence early posto…

Development and internal validation of a nomogram to predict perioperative hypothermia in patients undergoing laparoscopic gynecologic surgery under general anesthesia: a retrospective cohort study.

BACKGROUND: Perioperative hypothermia remains a common complication among patients undergoing laparoscopic gynecologic surgery under general anesthesia and is associated with adverse postoperative outcomes. Early identification of patients estimated to be at high risk may facilitate targeted preventive strategies. However, practical and individualized prediction tools for perioperative hypothermia in laparoscopic gynecologic surgery remain limited. METHODS: This single-center, retrospective observational cohort study included adult patients who underwent laparoscopic gynecologic surgery under

Effect of low-dose remifentanil infusion on emergence agitation after general anaesthesia in patients undergoing intracranial surgery: study protocol of a randomised controlled trial.

INTRODUCTION: Emergence agitation (EA), defined as acute postoperative restlessness after general anaesthesia, is a common complication in the post-anaesthesia care unit (PACU). The reported incidence of EA is nearly 30% in neurosurgical surgery, which bears tremendous risks for neurosurgical patients. Although current evidence suggests that remifentanil may reduce EA risk in non-cardiac settings, its preventive efficacy in patients undergoing intracranial surgery remains unclear. METHODS AND ANALYSIS: In this single-centre, randomised, double-blind, parallel-group prospective clinical trial,

Continuous versus bolus norepinephrine administration and arterial blood pressure stability during induction of general anaesthesia in high-risk noncardiac surgery patients: a randomised trial.

BACKGROUND: Hypotension after induction of general anaesthesia is common in high-risk patients having noncardiac surgery. Anaesthesiologists often give manual boluses of vasopressors repeatedly to maintain blood pressure during induction of general anaesthesia, including the fast-acting vasopressor norepinephrine which has a short half-life. We tested the hypothesis that giving norepinephrine continuously during induction of general anaesthesia, compared with giving it as repeated manual boluses, improves blood pressure stability in high-risk noncardiac surgery patients. METHODS: In this sing
