Oral phenotype and scoring of vascular Ehlers-Danlos syndrome: a case-control study.

OBJECTIVE: Vascular Ehlers-Danlos syndrome (vEDS) is a rare genetic condition related to mutations in the COL3A1 gene, responsible of vascular, digestive and uterine accidents. Difficulty of clinical diagnosis has led to the design of diagnostic criteria, summarised in the Villefranche classification. The goal was to assess oral features of vEDS. Gingival recession is the only oral sign recognised as a minor diagnostic criterion. The authors aimed to check this assumption since bibliographical search related to gingival recession in vEDS proved scarce. DESIGN: Prospective case-control study. …

A Dual-Layer Flap Preparation Approach Utilizing the External Coronally Advanced Flap and the Internal Trapdoor Periosteal Flap for Endodontic Microsurgery in the Esthetic Zone: Technique and Case Report.

Endodontic microsurgery (EMS) is often utilized to surgically treat teeth with persistent endodontic lesions when conventional nonsurgical endodontic treatments have been unsuccessful. However, EMS with conventional techniques may result in esthetic concerns, such as gingival recession, papilla loss, and soft tissue scarring, especially in patients with high esthetic demands. This manu-script describes a dual-layer flap preparation approach that utilizes an external coronally advanced flap (CAF) and an internal trapdoor periosteal flap (TPF) for EMS in the esthetic zone. The external CAF is a …

Clinical Management of Type 3 Recession Defects With Immediate Implant and Provisional Restoration Therapy: A Case Report.

Type 3 extraction sockets present a unique challenge in that they possess gingival recession indicative of facial hard- and soft-tissue loss. When teeth present with prior disease requiring removal and implant replacement, the treatment strategy incorporates palatal implant positioning as well as proper restorative contour management to compensate for the recession defect, thereby allowing the gingival tissues to heal in the correct 3-dimensional position. This article describes the case of a patient with a nonrestorable maxillary right central incisor with internal resorption. The case demons…

Kinetics of Phenotype Modification Therapy with VISTA: A Retrospective Case Series Using 3D Digital Analysis.

Phenotype modification therapy has emerged as one of the main treatment objectives of periodontal plastic surgery. However, long-term data on the stability of gingival thickness gains are not available. This study examined the kinetics of mucosal thickness gain as a measure of phenotype modification therapy following treatment of multiple gingival recession defects with vestibular incision subperiosteal tunnel access (VISTA). Six patients with 14 recession type (RT) II teeth were treated using VISTA and subepithelial connective tissue grafts (SCTG). Scanned images of study casts at pre- and po…

A New Matrix for Soft Tissue Management.

Gingival recession is a mucogingival defect defined as the apical shifting of the gingival margin in relation to the CEJ. The use of connective tissue autografts allows for the obtention of very satisfactory results but is associated with undoubted disadvantages. The aim of the present work is to carry out a systematic review of the literature using a meta-analysis to investigate the clinical efficacy of xenogeneic collagen matrix (XCM) in the treatment of gingival recessions. This revision was carried out strictly following the guidelines published in the Cochrane Handbook. Thus, a meta-analy…

Periodontal plastic pioneering procedures revisited.

BACKGROUND: Various periodontal plastic procedures are available for treatment of localized gingival recessions. However, in the mirage of newer techniques and materials available, it is difficult for clinicians to decide on the treatment modality. Pioneering techniques provide a cost effective and effective means of maintaining esthetics in recession cases. MATERIALS AND METHODS: Twenty-two subjects with localized gingival recessions were divided in equal groups and treated with lateral pedicle graft (Group A) and Free mucosal graft (Group B). Clinical parameters of length and width of reces…

Efficacy of the supraperiosteal envelope technique: a preliminary comparative clinical study.

The purpose of this clinical study was to compare the effectiveness of the supraperiosteal envelope technique as performed by three surgeons with different amounts of professional experience during a 6-month observation period. Sixty-eight gingival recessions in 35 healthy patients were selected and included in clinical recordings. They were statistically evaluated at baseline and 6 months later. First, clinical parameters (attachment level, pocket depth, recession height, recession width, and gingival keratinized tissue height) were calibrated between the three operators. Then, single or mult…

Ascertaining the regenerative potential of the "gold standard" grafts: Achieving 100% root coverage in Miller's Class III recession with periosteal pedicle graft and autogenous bone.

Recession of the gingiva is defined as the stripping of a portion of the dental root surface as a result of gingival margin shifting apically. Various techniques have been advocated for root coverage. The practice of utilizing periosteal pedicle graft for covering gingival recession defects is a contemporary development. Utilizing bone grafts for hard tissue regeneration has also been implemented. This case report assesses the effectiveness of the surgical approach utilizing autogenous bone and periosteum for recession coverage. A participant with Miller's Class III gingival recession in #23 a…

Combination of orthodontic movement and periodontal therapy for full root coverage in a Miller class III recession: a case report with 12 years of follow-up.

One of the main purposes of mucogingival therapy is to obtain full root coverage. Several treatment modalities have been developed, but few techniques can provide complete root coverage in a class III Miller recession. Thus, the aim of this case report is to present a successful clinical case of a Miller class III gingival recession in which complete root coverage was obtained by means of a multidisciplinary approach. A 17-year-old Caucasian female was referred for treatment of a gingival recession on the mandibular left central incisor. The following procedures were planned for root coverage …

Coronally positioned flap with subepithelial connective tissue graft for root coverage: various indications and flap designs.

Aesthetic concerns and functional abnormalities, such as dentin hypersensitivity, are often associated with gingival recession defects. Root coverage procedures aim to restore both gingival aesthetics and function in recession defects. The coronally positioned flap combined with the subepithelial connective tissue graft is one of the most widely used root coverage procedures. The present report illustrates four different indications where this procedure has been successfully employed. An isolated Miller class II recession defect associated with frenum pull, multiple adjacent Miller class I def…

Creeping attachment after free gingival grafts.

Free gingival grafts were placed in the lower anterior vestibular region of 30 patients each of whom had a gingival recession. According to the width of the recession and to the surgical procedure, the patients were divided into four groups. Group 1 showed a "narrow" (less than 3 mm) type of recession, while Group 2 had a "wide" type. In both of these groups the graft was placed over the denuded root. In Group 3 (narrow recession) and Group 4 (wide), a collar of vestibular gingiva was left in place on the lower margin of the denuded root at the moment of grafting. Measurements and photographs …
