The treatment of complex dorsal hand lesions involving skin and subcutaneous tissues, extensor tendons, and bone remains a difficult problem for reconstructive surgeons. Traditional treatment of these defects uses staged reconstruction, first obtaining soft tissue cover and then performing bone and tendon grafts. The purpose of this study was to compare a series of seven patients who underwent staged reconstruction with seven patients who had immediate reconstruction involving primary bone and tendon grafting. All procedures were performed to correct similar severe dorsal hand defects. Patient

Improvement of long-term treatment with hemodialysis requires an optimized radiological differential diagnosis of bone changes associated with long-term hemodialysis. 85 patients with chronic renal failure on maintenance hemodialysis were examined. In a conventional radiographic skeletal survey of the whole spine, 22 patients (26%) had involved intervertebral discs. The site of predilection was the cervical spine followed by the lumbar spine; oligosegmental involvement was seen in nine patients. Eighteen patients with destructive non-infectious spondyloarthropahy had juxtaarticular cystic bony

This article examines the most common problematic hand and wrist injuries in the pediatric athlete. Hand and wrist injuries in the growing skeleton pose a different diagnostic and therapeutic challenge than in the mature skeleton. Ligaments are stronger than bone, and unossified cartilaginous sections of the skeleton are yet more susceptible to injury than bone. Although remodeling can correct for even moderate deformities if sufficient growth potential exists, remodeling cannot return the child to normal anatomy in many cases. Remodeling depends on intact periosteum, a nearby growing physis,

Gorham-Stout disease is a rare idiopathic syndrome with distinctive clinical, pathologic and radiologic features. It is a variant form of osseus angiomatosis associated with massive osteolysis of bone. Usually appearing after trauma, the disease is described to occur at any age. Especially in case of thoracic involvement (chylothorax), lethal outcomes are reported. In Medline, about 200 cases have been described. A patient with osteolysis of the right hand following contusion at the age of two years is reported. Despite radiotherapy and repeated bone grafting, the osteolysis progressed until t

Forty six patients (41 female and 5 male) with progressive systemic sclerosis were analysed through X-ray photographs of feet. Age ranged from 33 to 74 (mean: 48 +/- 13) years old, and durations of the disease were one to 28 (mean: 9.8 +/- 8) years. Abnormal radiological findings of feet were noted as following; bone absorption of the terminal tufts of the first phalanx (6 out of 46 cases: 13%), bone absorption of metatarsals and/or mid, basal phalanges (2 cases: 4.3%), bone erosion (6 cases: 13.0%), calcification (7 cases: 15.2%), diffuse osteoporosis (2 cases: 4.3%) and cystic lesion (one ca

Brown tumor or osteoclastoma is a lytic bone tumor, which is common in secondary hyperparathyroidism (1.5-13%) in chronic dialysis patients, mainly in those with untreated renal osteodystrophy. Brown tumor appears as a result from excess osteoclast activity and consists of collections of osteoclasts intermixed with fibrous tissue and poorly mineralized woven bone. It can be manifested as a single or multiple bone lesions. Although invasive, it has no malignant potential and should be distinguished from giant cell tumors of the bone. Two unusual cases of brown tumor in dialysis patients are rep

OBJECTIVE: To investigate whether rheumatoid arthritis (RA) and psoriatic arthritis (PsA) can be differentiated in the early stages of the disease (duration of symptoms ≤1 year) on the basis of magnetic resonance imaging (MRI) features of the hand and wrist.
MATERIAL AND METHODS: Twenty early RA and 17 early PsA patients with symptomatic involvement of the wrist and hand joints and inconclusive radiographic studies were examined prospectively with contrast-enhanced MRI. Images were evaluated in accordance with the Outcome Measures in Rheumatology Clinical Trials recommendations.
RESULTS: Cer

UNLABELLED: Early identification of osteoporosis promotes timely treatment. This retrospective study demonstrated that bone cortical percentage (2MCP) measured on hand X-ray in patients with inflammatory arthritis correlates significantly with bone density, fracture risk and fracture incidence. Routine hand X-ray 2MCP reporting could support identification of patients at risk of osteoporotic fractures.
PURPOSE: Effective osteoporotic screening facilitates earlier identification and treatment of patients at risk. Bone mineral density (BMD) alone has limited sensitivity and may not always be av

Closing wedge osteotomy is an accepted technique for correcting bony malalignment. This study reports the results of a novel osteotomy technique used in children's hands. Excision of a partial wedge generates a 'greenstick' type of fracture which can then be reduced and stabilised using only intraosseous wires. Eight consecutive patients who underwent surgery of this type between March 2003 and January 2008 were reviewed retrospectively. Union was obtained in all cases and there was good bone alignment and range of movement. No significant technique-related complications were encountered. This

In the wide variety of maxillofacial deformities resulting from congenital malformation, trauma, or excision of malignant tissue, restoration of function and of facial form is achieved by the replacement of missing soft tissue, repositioning of displaced bone, bone grafting of osseous defects, and restoration of adequate contour. In these cases, the surgeon and prosthodontist must work hand in hand. Prostheses may be temporary or permanent, depending on the extent of deformity, the age of the patient, and the long-term needs. Prostheses can be made from various materials, including methyl meth

BONE: and bone-marrow associated cancers, including osteosarcoma, Ewing sarcoma, multiple myeloma, and various leukemias, are highly aggressive tumors with limited therapeutic options and poor outcomes. Viruses play a paradoxical role in these diseases, acting both as pathogenic drivers and as potential therapeutic agents. On one hand, oncogenic viruses such as Epstein-Barr virus (EBV), human T-cell leukemia virus type 1 (HTLV-1), and hepatitis viruses have been implicated in the initiation and progression of certain hematologic and skeletal malignancies. On the other hand, oncolytic viruses h

PURPOSE OF REVIEW: The term osteomyelitis covers a wide range of inflammatory bone disorders caused by microbial invasion or due to autoinflammatory processes, but furthermore osteomyelitis also occurs at different ages and at preferred localizations in the human skeleton. This article aims to give an overview of the current literature focussing on pathognomonic aspects of osteomyelitis because of microbial invasion.
RECENT FINDINGS: Outlining the chronological sequence of osteomyelitis originating from the invasion of microbes finally leading to destruction of bone tissue, the formation and

Proximal symphalangism is a rare autosomal dominant affection responsible for multiple joint ankylosis, it results from NOG gene mutation whose locus is on 17q21-22. This gene may be involved in secretory functions of glands such as ovarian function. Premature ovarian failure is idiopathic in 80% of cases but may be secondary to multiple etiologies including genetic. We report the case of a patient with proximal symphalangism and premature ovarian failure, and we discuss the possibility of genetic relationship between the two anomalies.

Various bisphosphonates and the selective estrogen receptor modulator (SERM) raloxifene are approved treatments of postmenopausal osteoporosis. They increase bone mineral density (BMD), decrease bone turnover, and reduce vertebral fracture incidence through different cellular mechanisms. We investigated the bone cellular activities, architecture, mineral content/density, and strength of ovariectomized (ovx) rats on a long-term bisphosphonate or SERM treatment, at doses of either agent correcting bone strength. Eleven weeks postovariectomy, 6-month-old rats were treated with the SERM MDL 103,32

INTRODUCTION: Metacarpal bone fractures often lead to significant shortening in the phalanges; it can be caused by variable conditions, ranging from idiopathic, and infective to traumatic causes. There are very rare reports of metacarpals shortening in idiopathic primary hypoparathyroidism cases in the literature. Traumatic metacarpal shortening differs from congenital agenesis [2]. This condition leads to both functional and cosmetic limitations which affect the patient's quality of life.
CASE REPORT: A 22-year-old male patient was admitted to our outpatient clinic with a traumatic left-hand

For crushing injury of the hand that involves 4-digit amputations sparing the thumb, lengthening of amputated stump and covering the defect are important for hand function. From March 2007 to September 2011, the authors performed "reverse on-top plasty" in 5 patients. The fifth metacarpal bone was disarticulated, turned over 180 degrees, and embedded in reverse direction so that the metacarpal base becomes the tip of new finger. Soft tissue defects were covered using anterolateral thigh free flap or reverse radial forearm flap. The mean follow-up period was 21.2 months, and major complications

Tumoral calcinosis rarely occurs in the hand. It is an uncommon familial disease suggesting an autosomal recessive gene pattern, often with laboratory findings of hyperphosphatemia and normocalcemia. Our patient had bilateral hand involvement, including local calcinosis cutis of the skin and bone changes not previously reported in the literature. At a 2-year follow-up after two lesions were excised from her left hand, one lesion had completely resolved and the other recurred. Two large symptomatic lesions and one area of local calcinosis cutis in her right hand have recently been excised.

Enchondroma is a common bone tumor of hand. It is rarely localized in scaphoid. We report a new case of this tumor revealed by fracture. The diagnosis was suggested by standard radiography and confirmed then by anatomic pathology examination. Enucleation and bone graft with osteosynthesis yielded good result.

We report on a 36-year-old male patient with a congenital arterio-venous malformation (type C) of the hand. Symptoms onset was three years ago with an ulcer on the tip of the left middle finger, with the fingers and the palm of the hand turning increasingly livid. The shunt-volume in the area of an arterio-venous fistula was 70 ml/min, there was also involvement of the third metacarpal bone. Increasing pain from the index to the ring fingers with no tendency of healing in the middle finger were indications for surgical intervention. In view of the diffuse expansion to all the tissue layers and
