The survey was carried out between January 1988 and June 1990. It consisted in a clinical and allergological study of 1489 housewives, 880 of which had contact dermatitis at their hands. (group of "cases"). The other 609 represented the control group. 139 workers from 3 ceramics factories underwent dermatological and allergological examination, using standard and occupational patch test series, in order to evaluate the prevalence of dermatitis and contact sensitization, to identify the most important sensitizing substances in the ceramics industry, and to correlate the results with possible risk factors such as atopy. Hand dermatitis had affected 37% of the subjects examined. 27% of the workers were sensitized to 1 or more allergens. Of 52 subjects with dermatitis, 37% had allergic contact dermatitis and 63% irritant contact dermatitis. 18

Mycetomas of the hand are a rare site of an uncommon disease, particularly in temperate climates. They are non-purulent inflammatory pseudotumours of mycotic origin. They are generally seen in Tropical Africa with a slow course progressing towards a phase of cutaneous fistula formation producing black, white or red grains depending on the fungus responsible. Ten cases are reported together with a review of the literature. A 61-year-old man is described with sclerodactyly of the hands and syndactyly of the second and third toes. Hereditary sclerodactyly is a rare condition, beginning in early youth with flexion contracture of the fingers. In this patient the skin of the fingers was sclerotic and thickened, and the dorsal skin of the hands was atrophic and dry. A mycological and immunological survey was performed in 26 patients with clinical signs of dermatophytid. Only 10 patients fulfilled the main criteria of dermatophytid reactions, i.e. positive delayed skin test to trichophytin and dermatophyte isolated by culture. Trichophyton mentagrophytes--especially the zoophilic variant--was found in 9 patients of 10. In the majority of cases, inflammatory tinea pedis caused the dermatophytid reaction which appeared as vesicles localized to the palms. A few days after a spine of one of his indoor cacti had pricked the fourth finger of his left hand, a 56-year-old man developed multiple inflammatory suppurating nodules and infiltrates and a cord-like lymphangitis on the left forearm and upper arm. Nocardia brasiliensis was cultivated from aspirated pus. Subsequently, both Nocardia brasiliensis and Nocardia asteroides were isolated from the cactus earth. This is the first report of an infection by Nocardia brasiliensis acquired in Germany.

Combined application of clinical, genetic and histological criteria in general allows a definite diagnosis of autosomal dominant ichthyosis vulgaris and of X-linked recessive ichthyosis. For differential diagnosis, the following rare syndromes should be considered: ichthyosis bullosa: Refsum syndrome; Jung-Vogel syndrome; ichthyosis with corneal opacity, pili torti and alopecia; ichthyosis with deafness, pili torti and dental anomalies; and ichthyosis with hepatosplenomegaly and cerebellar degeneration.
