The epidemiology of multidrug-resistant tuberculosis in the United States.

There has been a significant increase in the number of cases of MDR-TB in the United States. Although cases of MDR-TB have been reported from many areas of the country, the majority of the cases are concentrated in large urban areas. MDR-TB is difficult and expensive to treat. CDC has developed a National Action Plan to Combat Multidrug-Resistant Tuberculosis. The main elements of this plan include (1) greater surveillance and epidemiologic studies of drug-resistant TB; (2) initiatives to make the laboratory diagnosis of MDR-TB more rapid, sensitive, and reliable; (3) education of health care

Six-year trends in health care-associated infections in Chilean hospitals: A national surveillance analysis (2017-2022).

BACKGROUND: Health care-associated infections (HAIs) remain a major patient safety challenge in hospital settings. Surveillance systems provide monitoring, but few studies integrate long-term trends in infection risk and outcomes with measures of system capacity and emergency investment. METHODS: We conducted a descriptive analysis of national HAI surveillance data from Chilean public hospitals spanning 2017 to 2022, reported in accordance with STROBE guidelines. Indicators included HAI prevalence, device utilization ratios, incidence densities for ventilator-associated pneumonia (VAP), centr

Prevention of cardiovascular diseases in women planning pregnancy and during pregnancy. Expert opinion of the Section of Prevention and Epidemiology of the Polish Cardiac Society, the Polish Society of Gynecologists and Obstetricians, the Polish Society of Family Medicine, the Polish Lipid Association, and the Polish Lifestyle Medicine Society.

Pregnancy is a period of change in a woman's life and health. Adaptive mechanisms also involve the cardiovascular system. Preventing cardiovascular disease (CVD) before and during pregnancy requires a multifaceted approach, including lifestyle modification, early identification and treatment of risk factors, and comprehensive postpartum care. Key strategies include proper diet, adequate regular physical activity, maintaining a healthy weight, coping with stress, getting adequate sleep, and avoiding smoking. In addition, it is crucial to address pregnancy-related complications such as pregnancy

Pulmonary and tricuspid regurgitation after Tetralogy of Fallot repair: A case report.

BACKGROUND: Tetralogy of Fallot (TOF) is one of the most common congenital heart defects, and surgery is the primary treatment. There are no precise guidelines on the treatment protocol for tricuspid regurgitation (TR) as a common complication of TOF repair. The timing for treatment in patients presenting with valve regurgitation after TOF repair is often difficult to determine. Here, we report the first case of sequential treatment of pulmonary and TR using interventional therapy. CASE SUMMARY: We present the case of a 52-year-old female patient, who had a history of TOF repair at a young ag

Frequently asked questions about medical abortion.

The development of methods of inducing abortion medically (nonsurgically) has created alternative options to make abortion available to women in a variety of health-care settings. Medical abortion is induced with a regimen of mifepristone followed by a prostaglandin analogue. Since its first introduction in the late 1980s, the regimen has undergone some modifications based on research evidence, and, in many countries, clinicians are using regimens that may differ from the one that has been licensed. This causes confusion among providers, also because only a few countries have developed nationa

A Conceptual Framework for Individualized Patient Education in Healthcare: A Scoping Review.

INTRODUCTION: Individualized patient education has received increasing attention as a strategy to improve patient engagement, self-management, and health outcomes. However, its definition, components, and implementation remain conceptually unclear. This scoping review aimed to synthesize existing evidence, clarify the concept of individualized patient education, and develop a conceptual framework by identifying its core dimensions and implementation approaches. METHODS: This scoping review followed the Joanna Briggs Institute methodology and PRISMA-ScR guidelines. A comprehensive search was p

Mental Health Polypharmacy in "Non-Coded" Primary Care Patients: The Effect of Deprescribing.

Background: Mental health (MH) polypharmacy, defined as prescribing multiple mental health medications for the same condition, presents significant challenges in clinical practice. With varying prevalence rates and an increasing trend, particularly in the UK, this deprescribing prospective quality improvement project aimed to address the complexities and risks associated with MH polypharmacy. Patients and Methods: A large primary care centre in London was selected for this project. Electronic records of 667 patients (non-coded in mental health lists) were analysed as a result of the absence of

Counting every stillbirth and neonatal death through mortality audit to improve quality of care for every pregnant woman and her baby.

BACKGROUND: While there is widespread acknowledgment of the need for improved quality and quantity of information on births and deaths, there has been less movement towards systematically capturing and reviewing the causes and avoidable factors linked to deaths, in order to affect change. This is particularly true for stillbirths and neonatal deaths which can fall between different health care providers and departments. Maternal and perinatal mortality audit applies to two of the five objectives in the Every Newborn Action Plan but data on successful approaches to overcome bottlenecks to scali

The Lililwan Project: study protocol for a population-based active case ascertainment study of the prevalence of fetal alcohol spectrum disorders (FASD) in remote Australian Aboriginal communities.

INTRODUCTION: Anecdotal reports suggest that high-risk drinking in pregnancy is common in some remote Australian communities. Alcohol is teratogenic and may cause a range of lifelong conditions termed 'fetal alcohol spectrum disorders' (FASD). Australia has few diagnostic services for FASD, and prevalence of these neurodevelopmental disorders remains unknown. In 2009, Aboriginal leaders in the remote Fitzroy Valley in North Western Australia identified FASD as a community priority and initiated the Lililwani Project in partnership with leading research organisations. This project will establis

Increasing identification of foot at risk of complications in patients with diabetes: a quality improvement project in an urban primary health centre in India.

The majority of foot amputations are preventable in people with diabetes. Guidelines recommend that people with diabetes should receive a foot examination for risk assessment, at least annually. In an audit at a primary health centre (PHC) in Mumbai, India, no patient with diabetes was offered preventive foot assessment in preceding 12 months. Problem analysis identified a lack of clinic policy, training and equipment for foot assessment. There was no standardised referral pathway for patients identified with foot at risk of diabetes complications. Furthermore, limited data review, high patien

Integrating multiple programme and policy approaches to hepatitis C prevention and care for injection drug users: a comprehensive approach.

BACKGROUND: New York State is home to an estimated 230,000 individuals chronically infected with hepatitis C virus (HCV) and roughly 171,500 active injection drug users (IDUs). HCV/HIV co-infection is common and models of service delivery that effectively meet IDUs' needs are required. A HCV strategic plan has stressed integration. METHODS: HCV prevention and care are integrated within health and human service settings, including HIV/AIDS organisations and drug treatment programmes. Other measures that support comprehensive HCV services for IDUs include reimbursement, clinical guidelines, tra

"No One Needs to be Forced": Qualitative Insights on Competing Priorities between Antiretroviral Therapy and Reproductive Health Planning during the Dolutegravir Rollout.

Potential associations between periconception dolutegravir (DTG) exposure and neural tube defects (NTDs) reported in 2018 caused shifting international and national antiretroviral treatment (ART) guidelines. They sometimes required women to use contraception prior to initiating DTG. To better understand the tensions between ART and family planning (FP) choices, and explore the decision-making processes of women living with HIV (WLHIV) and their healthcare providers (HCPs) employed, we conducted interviews with WLHIV exposed to DTG and their providers in western Kenya from July 2019 to August 2

The health burden and economic costs averted by ambient PM2.5 pollution reductions in Nagpur, India.

National estimates of the health and economic burdens of exposure to ambient fine particulate matter (PM2.5) in India reveal substantial impacts. This information, often lacking at the local level, can justify and drive mitigation interventions. Here, we assess the health and economic gains resulting from attainment of WHO guidelines for PM2.5 concentrations - including interim target 2 (IT-2), interim target 3 (IT-3), and the WHO air quality guideline (AQG) - in Nagpur district to inform policy decision making for mitigation. We conducted a detailed assessment of concentrations of PM2.5 in 9

Clinicians' perspectives on race-specific guidelines for hypertensive treatment.

Despite the general consensus that there is no biological basis to race, racial categorization is still used by clinicians to guide diagnosis and treatment plans for certain diseases. In medicine, race is commonly used as a rough proxy for unmeasured social, environmental, and genetic factors. The American College of Cardiology's Eighth Joint National Committee's (JNC 8) guidelines for the treatment of hypertension provide race-specific medication recommendations for Black versus non-Black patients, without strong evidence for race-specific physiological differences in drug response.
