Performance goals for an adjunct diagnostic test to reduce unnecessary biopsies after screening mammography: analysis of costs, benefits, and consequences.

PURPOSE: Because benign biopsies resulting from false-positive mammographic findings are a known harm of breast cancer screening, physicians and test manufacturers are searching for ways to reduce their frequency. The aim of this study was to estimate potential costs and consequences associated with using an adjunct diagnostic test for triaging women with suspicious mammographic findings before biopsy. METHODS: A decision model was developed to compare the use of an adjunct test before biopsy to the current standard of care for suspicious mammographic findings. The decision analysis was perfo

Identifying Unmet Needs of Families With Children and Youth With Special Health Care Needs in a Preparedness Intervention Connected to the Medical Home.

BACKGROUND: Families with children and youth with special health care needs (CYSHCN)-especially those with complex or multiple morbidities-have additional considerations when planning for emergencies. These families can also have health-related social needs that make achieving household emergency preparedness especially challenging. Our objective was to identify and address the unmet social needs and emergency preparedness needs in a diverse sample of families of CYSHCN recruited from a network of medical homes across Pennsylvania as part of a home-focused, virtual emergency preparedness inter

Surgical tactics in revision operations of the hip acetabulum.

Surgical tactics in acetabulum revisions is based on 2 elements. The first of these is the assessment of the patient's general health (age, clinical status, and concomitant diseases). Adequate planning makes it possible to avoid further complications, such as problems with bone transplant incorporation and subsequent mobility of the acetabulum. The second element is the assessment of bone stock deficits, especially in the acetabular ring. Although it is possible to determine the extent of bone deficits intraoperatively, in preoperative planning we use conventional X-rays, CT scans, and - in ca

Integrating HIV/AIDS services into financial protection systems to increase sustainability of the HIV/AIDS response.

Begun in 2014, the Sustainable Financing Initiative (SFI) was dedicated to mobilizing domestic resources for the HIV response. Among its three programmatic pillars was a focus on ensuring financial protection for people living with HIV (PLHIV). SFI's activities were founded on a strong understanding of and alignment with partner government priorities, as well as costing and actuarial analyses, which allowed for the development of context-specific approaches for improving financial protection for PLHIV. SFI implemented financial protection activities in a total of nine countries; the five count

Spinal cord injury management and rehabilitation: highlights and shortcomings from the 2005 earthquake in Pakistan.

Recent natural disasters have highlighted the lack of planning for rehabilitation and disability management in emergencies. A review of our experience with spinal cord injury (SCI) after the Pakistan earthquake of 2005, plus a review of other literature about SCI after natural disasters, shows that large numbers of people will incur SCIs in such disasters. The epidemiology of SCI after earthquakes has not been well studied and may vary with location, severity of the disaster, available resources, the expertise of the health care providers, and cultural issues. A lack of preparedness means that

Hospital recruitment for a pragmatic cluster-randomized clinical trial: Lessons learned from the COMPASS study.

BACKGROUND: Pragmatic randomized clinical trials are essential to determine the effectiveness of interventions in "real-world" clinical practice. These trials frequently use a cluster-randomized methodology, with randomization at the site level. Despite policymakers' increased interest in supporting pragmatic randomized clinical trials, no studies to date have reported on the unique recruitment challenges faced by cluster-randomized pragmatic trials. We investigated key challenges and successful strategies for hospital recruitment in the Comprehensive Post-Acute Stroke Services (COMPASS) study

Planned mode of birth after previous cesarean section: A structured review of the evidence on the associated outcomes for women and their children in high-income setting.

In many high-income settings policy consensus supports giving pregnant women who have had a previous cesarean section a choice between planning an elective repeat cesarean section (ERCS) or planning a vaginal birth after previous cesarean (VBAC), provided they have no contraindications to VBAC. To help women make an informed decision regarding this choice, clinical guidelines advise women should be counseled on the associated risks and benefits. The most recent and comprehensive review of the associated risks and benefits of planned VBAC compared to ERCS in high-income settings was published i

Understanding patient perspectives on the quality of family planning care: a qualitative study.

BACKGROUND: In 2024, the United States (US) Office of Population Affairs updated federal guidelines for the provision of quality family planning (QFP) services, outlining six guiding principles-person-centered, evidence-based, inclusive, accessible, sex- and body-positive, and trauma-informed care-to promote equitable, high-quality sexual and reproductive health (SRH) services. This study explores patients' SRH care preferences and the extent to which asserted preferences are aligned with the systems-level definition of quality advanced in the QFPs. METHODS: We conducted a secondary qualitati
