[Emergency craniocerebral hypothermia in the management of open mitral commissurotomy].

Craniocerebral hypothermia was used during open-heart surgery in 69 patients with mitral stenosis (hospital mortality rate 2.9%). Indications for open-heart surgery (left atrial thrombosis, mitral valve calcinosis, onset of traumatic mitral insufficiency) were determined intraoperatively during closed mitral commissurotomy. Some pathophysiological peculiarities of craniocerebral hypothermia were studied. A decrease in brain temperature and total oxygen consumption (up to 27.7% of the basic metabolism level) has been observed. Prolongation (up to 25 min) of major vessel occlusion had no negativ

Utilization of Cardiac Rehabilitation Among Cardiac Intensive Care Unit Survivors.

Cardiac rehabilitation (CR) is associated with decreased mortality and rehospitalization rates for patients with a variety of cardiac conditions. Although CR referral rates for STEMI patients have improved, rates for heart failure have remained low. Many of these patients are admitted to the cardiac intensive care unit (CICU). However, it is unknown how often CICU survivors qualify for cardiac rehabilitation, how often they are referred, and why eligible patients are not referred. This is a retrospective single-center study of 417 consecutive patients admitted to CICU for >48 hours from March

Ketones provide an extra source of fuel for the failing heart without impairing glucose oxidation.

BACKGROUND: Cardiac glucose oxidation is decreased in heart failure with reduced ejection fraction (HFrEF), contributing to a decrease in myocardial ATP production. In contrast, circulating ketones and cardiac ketone oxidation are increased in HFrEF. Since ketones compete with glucose as a fuel source, we aimed to determine whether increasing ketone concentration both chronically with the SGLT2 inhibitor, dapagliflozin, or acutely in the perfusate has detrimental effects on cardiac glucose oxidation in HFrEF, and what effect this has on cardiac ATP production. METHODS: 8-week-old male C57BL6/

Effects of central apneas on sympathovagal balance and hemodynamics at night: impact of underlying systolic heart failure.

BACKGROUND: Increased sympathetic drive is the key determinant of systolic heart failure progression, being associated with worse functional status, arrhythmias, and increased mortality. Central sleep apnea is highly prevalent in systolic heart failure, and its effects on sympathovagal balance (SVB) and hemodynamics might depend on relative phase duration and background pathophysiology. OBJECTIVE: This study compared the effects of central apneas in patients with and without systolic heart failure on SVB and hemodynamics during sleep. METHODS: During polysomnography, measures of SVB (heart r

Severe Aortic Valve Stenosis During Pregnancy: Role of Multidisciplinary Pregnancy Heart Team to Guide Decision-Making.

BACKGROUND: Pregnant women with underlying cardiovascular disease are at an increased risk of maternal-fetal morbidity and mortality especially in the setting of symptomatic stenotic valvular heart disease. CASE SUMMARY: A 35-year-old female at 17 weeks gestation presented to the emergency department with exertional dyspnea for 3 months and was diagnosed with decompensated heart failure. Echocardiogram demonstrated left ventricular ejection fraction of 45% with severe aortic valve stenosis and moderate aortic regurgitation. A multidisciplinary pregnancy heart team recommended urgent surgical

Cheyne-Stokes respiration screening using cyclic variation in heart rate detected by Holter electrocardiogram.

BACKGROUND: Cheyne-Stokes respiration (CSR) is a major form of central sleep apnea in patients with heart failure and is associated with mortality in these patients. Although full polysomnography is required for CSR diagnosis, access may be limited. Cyclic variation of heart rate (CVHR) detected by Holter electrocardiogram reflects changes in cardiac autonomic activity associated with apnea-hypopnea events and is easier to perform than polysomnography. METHODS: We examined whether RR interval shortening time in the CVHR analysis could be useful for screening CSR. In this study, 41 patients we

Neural influences on cardiovascular variability: possibilities and pitfalls.

Altered variability in the cardiovascular system is associated with a range of cardiovascular diseases and increased mortality. Because blood pressure and heart rate show distinct low-frequency oscillations that appear to be affected by either vagal or sympathetic activity, it has been hoped that measurement of the strength of these oscillations could be used as an index of autonomic tone and thus form the basis of a diagnostic test. This review focuses on recent research that has examined the fundamental origin of variability associated with respiration and a slow oscillation at 0.1 Hz in the

Prediction of the oculocardiac reflex from pre-operative linear and nonlinear heart rate dynamics in children.

This study was aimed to determine whether pre-operatively measured linear and nonlinear analysis of heart rate variability might predict the occurrence of the oculocardiac reflex (OCR) or other arrhythmia during strabismus surgery in children (n = 185, mean (SD) age = 8.09 (3.31) years). We compared time- and frequency-domain, and nonlinear dynamic indices of pre-operatively measured RR interval data between the OCR-positive group (maximum heart rate decrement = 20 beat.min-1 during the traction of extraocular muscle, n = 54), OCR-negative group (< 20 beat x min(-1), n = 78) and arrhythmia-pos

[Paradoxical respiratory response, changes in heart rate and skin circulation in progressive isocapnic hypoxia in sleep apnea patients].

In 73 patients with sleep apnea syndrome, lung function tests, night-time polysomnography for determining the apnea index (AI), hyperoxic progressive hypercapnia tests under rebreathing conditions, and, after a short pause, isocapnic progressive hypoxia tests were carried out. The objective of these examinations was to study peripheral and central chemosensitivity, changes in heart rate and capillary cutaneous circulation under isocapnic hypoxia in patients with sleep apnea syndrome (SAS). The following linear relationships were presupposed for the evaluation: for peripheral chemosensitivity b

Atropine and glycopyrrolate: hemodynamic effects and placental transfer in the pregnant ewe.

Atropine administration in pregnant women may cause fetal tachycardia and a decrease in beat-to-beat variability of the fetal heart rate. In the present study, placental transfer and maternal and fetal hemodynamic effects of atropine and glycopyrrolate were compared in chronically and in acutely instrumented, unanesthetized pregnant ewes. Administration of either atropine (0.05 mg/kg) or glycopyrrolate (0.025 mg/kg) increased maternal heart rate by 25% without changing maternal arterial pressure, fetal arterial pressure, fetal heart rate, or beat-to-beat variability. Maternal and fetal blood g

Moderate preterm birth affects right ventricular structure and function and pulmonary artery blood flow in adult sheep.

KEY POINTS: Preterm birth occurs when the heart muscle is immature and ill-prepared for the changes in heart and lung function at birth. MRI imaging studies show differences in the growth and function of the heart of young adults born preterm, with the effects more pronounced in the right ventricle. The findings of this study, conducted in sheep, showed that following moderate preterm birth the right ventricular wall was thinner in adulthood, with a reduction in the number and size of the heart muscle cells; in addition, there was impaired blood flow in the main artery leading from the right v

[DETERMINATION OF THE STATE OF MICROCIRCULATION BY MEANS OF COMPUTER CAPILLAROSCOPY IN CHILDREN SEDATED WITH RETENTION OF CONSCIOUSNESS IN OUTPATIENT PEDIATRIC DENTAL TREATMENT].

UNLABELLED: Dental treatment of infants and pre-school children is challenging because of high rate of dental fear General anesthesia is the only option for behavioral control in this age group. In case of minor medical procedure physical restrain is also used often enough aggravating the fear An alternate method for comfortable treatment is monitored conscious sedation (MCS). Standard techniques of monitoring do not always provide accurate determination of stress level in children since the external stress manifestations may appear non-significant and unobtrusive. [Pharmacokinetics and tissue distribution characteristics of the novel photosensitizer 32-(4-methoxyphenyl)-152-aspartyl-chlorin e6].

Photodynamic therapy (PDT) has garnered immense research interest. PDT can directly kill the cells via a combination of photosensitizer, light, and molecular oxygen. It has emerged as a promising therapeutic option for cancer treatment owing to its advantages such as minimized systemic toxicity, minimal invasiveness, high therapeutic efficacy, and potential for developing antitumor immunity. The novel photosensitizer 32-(4-methoxyphenyl)-152-aspartyl-chlorin e6 (DYSP-C34) was synthesized by introducing a 32-aryl substitution and amino acid substituent of the Chenghai chlorin (CHC). Briefly, 32

A Systematic Review of the Relationship Between Traumatic Brain Injury and Disruptions in Heart Rate Variability.

Autonomic nervous system dysfunction is increasingly recognized as a common sequela of traumatic brain injury (TBI). Heart rate variability (HRV) is a specific measure of autonomic nervous system functioning that can be used to measure beat-to-beat changes in heart rate following TBI. The objective of this systematic review was to determine the state of the literature on HRV dysfunction following TBI, assess the level of support for HRV dysfunction following TBI, and determine if HRV dysfunction predicts mortality and the severity and subsequent recovery of TBI symptoms. We followed the Prefer

[The dynamics of pulse rate and biochemical parameters in blood of healthy individuals in relation to Pc5-6 geomagnetic pulsations].

Four experiments on long-term monitoring of pulse rate and blood biochemical parameters in four healthy volunteers (women) were conducted. The duration of each experiment was 90 minutes, electrocardiography was performed continuously, taking blood sampling every two minutes. In venous blood the current concentrations of triiodothyronine, cortisol, glucose, stable metabolites of nitrogen oxides (NO(x)) were determined. Synchronicity in oscillations of cortisol and free triiodothyronine levels in the blood of all four volunteers was detected, as well as the presence of the periods of 7-8 min and

Sodium bicarbonate improves sprint performance in endurance cycling.

OBJECTIVES: Oral sodium bicarbonate intake (NaHCO3) may improve performance in short maximal exercise by inducing metabolic alkalosis. However, it remains unknown whether NaHCO3 also enhances all-out performance at the end of an endurance competition. Therefore, the present study investigated the effect of stacked NaHCO3 loading on sprint performance following a 3-h simulated cycling race. DESIGN: Double-blind randomized placebo-controlled cross-over study. METHODS: Eleven trained male cyclists (22.3 (18.3-25.3) year; 73.0 (61.5-88) kg; VO2max: 63.7 (57-72) mlkg-1min-1) ingested either 300mg

Usefulness of modified Pulmonary Index Score (mPIS) as a quantitative tool for the evaluation of severe acute exacerbation in asthmatic children.

BACKGROUND: Acute exacerbation of asthma is divided qualitatively into mild, moderate, and severe attacks and respiratory failure. This system is, however, not suitable for estimating small changes in respiratory condition with time and for determining the efficacy of treatments, because it has a qualitative, but not quantitative nature. METHODS: To evaluate the usefulness of quantitative estimation of asthma exacerbation, modified Pulmonary Index Score (mPIS) values were measured in 87 asthmatic children (mean age, 5.0 ± 0.4 years) during hospitalization. mPIS was calculated by adding the su

Respiratory function dynamics in individuals with increased motor activity during standard exercise testing.

Functional diagnostics is one of the most important areas of sports medicine, which plays an increasingly role in selection of athletes, optimisation of training, early diagnostics and prophylaxis of sports pathology. Functional condition of the body undergoes constant changes under the influence of a number of factors. The differences may be considerable and pose problems related to the recruitment of athletes to teams in game sports and in individual sports. Physical ability is determined by the quality of external breathing and cardiovascular functions and by peculiarities of metabolism rel

Fundamental hemodynamic mechanisms mediating the response to myocardial ischemia in conscious paraplegic mice: cardiac output versus peripheral resistance.

Autonomic dysfunction, a relative sedentary lifestyle, a reduced muscle mass and increased adiposity leads to metabolic abnormalities that accelerate the development of coronary artery disease (CAD) in individuals living with spinal cord injury (SCI). An untoward cardiac incident is related to the degree of CAD, suggesting that the occurrence of a significant cardiac event is significantly higher for individuals with SCI Thus, understanding the fundamental hemodynamic mechanisms mediating the response to myocardial ischemia has the potential to positively impact individuals and families living
