SARIMA-modelled greater severity and mortality during the 2010/11 post-pandemic influenza season compared to the 2009 H1N1 pandemic in English hospitals.

OBJECTIVE: The COVID-19 pandemic demonstrates the need for understanding pathways to healthcare demand, morbidity, and mortality of pandemic patients. We estimate H1N1 (1) hospitalization rates, (2) severity rates (length of stay, ventilation, pneumonia, and death) of those hospitalized, (3) mortality rates, and (4) time lags between infections and hospitalizations during the pandemic (June 2009 to March 2010) and post-pandemic influenza season (November 2010 to February 2011) in England. METHODS: Estimates of H1N1 infections from a dynamic transmission model are combined with hospitalization

Desmopressin prescription safety in adult inpatients: a real-world tertiary centre experience.

ABSTRACT: Omissions or delays in desmopressin can result in serious patient harm in patients with arginine vasopressin deficiency (AVP-D), formally known as cranial diabetes insipidus. Desmopressin administration practice in hospitals has not been thoroughly investigated previously. This study evaluated desmopressin prescription and administration practice at a large tertiary centre. A retrospective electronic case notes review of all admissions (≥24 h) with AVP-D between 2018 and 2021 at Sheffield Teaching Hospitals was undertaken. The outcomes were defined as the total number of missed and d

Impact of E-visits on Outpatient Volume and Service Time: Evidence from a Large Public Hospital in China.

The emergence of electronic visits (e-visits) as a convenient, efficient alternative to in-person outpatient care has reshaped service delivery in large public hospitals in China. However, how this dual-channel outpatient model reshapes overall medical demand and clinicians' service patterns remains insufficiently understood. Drawing on real-world data from a large public hospital in China, this study systematically estimates the impacts of e-visits on outpatient volumes and clinicians' service time. The authors adopt a twofold difference-in-differences model employed for the analysis that acc

Management by Walking Around and Nurses' Voice Behaviour: A Structural Equation Modelling Approach to the Mediating Role of Trust in Intensive Care Units.

BACKGROUND: Management by walking around (MBWA) is a leadership practice in which head nurses maintain a visible presence in clinical settings, interact directly with nurses, promote open communication, observe workflow and foster a supportive work environment. These practices may contribute to building nurses' trust and encouraging them to speak up. AIM: To examine the direct and indirect effects of head nurses' MBWA on nurses' voice behaviour with trust as a single mediating variable. STUDY DESIGN: A cross-sectional correlational design was employed using a convenience sample of nurses wor

Economic evaluation of a hybrid type 1 effectiveness-implementation trial comparing a community health worker palliative care intervention to enhanced standard of care in African American patients across four cancer centres in the USA: a study protocol.

INTRODUCTION: Despite increasing palliative care capabilities in the USA, utilisation rates remain low for patients with advanced cancer, particularly among African American patients. To address this gap, a theory-driven, stakeholder-informed community health worker (CHW) palliative care intervention for African American patients with advanced cancer and their informal caregivers is currently being assessed through a hybrid type 1 effectiveness-implementation trial at four cancer centres across the USA. To improve the quality and efficiency of palliative care delivery, inform resource allocati

Position Statement on In-hospital/Clinic Point-of-care Coagulation Testing for Anticoagulation Monitoring in Saudi Arabia.

OBJECTIVES: Hospital overload is a persistent occurrence in daily practice. Interventions such as point-of-care testing (POCT) are needed to alleviate the pressure faced by healthcare providers and administrators. METHODS: An invited panel of experts from Saudi Arabia was formed under the auspices of the Saudi Heart Association in order to discuss local treatment gaps in the management of patients receiving anticoagulation therapy. This was done in a series of meetings, which resulted in the development of official recommendations for the implementation of POCT for anticoagulation monitoring

Mitigating disrespect and abuse during childbirth in Tanzania: an exploratory study of the effects of two facility-based interventions in a large public hospital.

BACKGROUND: There is emerging evidence that disrespect and abuse (D&A) during facility-based childbirth is prevalent in countries throughout the world and a barrier to achieving good maternal health outcomes. However, much work remains in the identification of effective interventions to prevent and eliminate D&A during facility-based childbirth. This paper describes an exploratory study conducted in a large referral hospital in Dar es Salaam, Tanzania that sought to measure D&A, introduce a package of interventions to reduce its incidence, and evaluate their effectiveness. METHODS: After exte

[Efficacy of antioxidant therapy in patients with acute and chronic purulent otitis media].

The examination of 90 patients referred to hospital for otitis media purulenta (OMP) and 25 healthy subjects included assessment of lipid peroxidation by content of malonic dialdehyde and intensity of chemoluminescence. Enzymes of antioxidant system were also measured (superoxide dismutase and catalase in blood plasma and wash-offs from the acoustic meatus and postoperative cavities). The measurements gave different values for the above parameters for healthy and OMP subjects. Conventional treatment reduced levels of LPO products and raised the activity of antioxidant enzymes, but control valu

A time and motion study of peripheral venous catheter flushing practice using manually prepared and prefilled flush syringes.

Peripheral venous catheters (PVCs) are the simplest and most frequently used method for drug, fluid, and blood product administration in the hospital setting. It is estimated that up to 90% of patients in acute care hospitals require a PVC; however, PVCs are associated with inherent complications, which can be mechanical or infectious. There have been a range of strategies to prevent or reduce PVC-related complications that include optimizing patency through the use of flushing. Little is known about the current status of flushing practice. This observational study quantified preparation and a

Assessment of endothelial glycocalyx degradation in sepsis using direct visualization with sublingual microscopy.

BACKGROUND: The endothelial glycocalyx plays a crucial role in the pathophysiology of sepsis, but assessment of glycocalyx injury in clinical settings remains challenging. Perfused boundary region (PBR) is derived from sublingual intravital video microscopy and has been studied as a surrogate of endothelial glycocalyx barrier function. The goal of this study was to determine whether PBR is associated with circulating heparan sulfate (a biochemical marker of glycocalyx degradation), sepsis status, in-hospital mortality, and intravenous (IV) fluid administration. METHODS: This was an ancillary
