Subclavian perivascular block for open reduction and internal fixation of left midshaft humeral fracture--a case report.

Traumatic injuries affecting bones of the hand and forearm often require peripheral nerve blocks for analgesia and surgical intervention. The successful use of subclavian perivascular block as a sole anaesthetic for orthopaedic surgery has not been reported in our environment. We report the use of this technique for open reduction and internal fixation of a left midshaft humeral fracture. The trunk of the brachial plexus was localized by a Polystim II nerve stimulator. Complete sensorimotor block was achieved within 15 minutes and surgery lasted 55 minutes without complications. This technique

Injury to the radial nerve caused by fracture of the humeral shaft: timing and neurobiological aspects related to treatment and diagnosis.

The radial nerve may not function in association with fractures of the humeral shaft. There are various opinions about the causes and treatment. We report a case of complete rupture of the radial nerve after a fracture of the proximal shaft of the humerus. The nerve injury was treated with grafting and TENDON transfer. Here we discuss diagnoses and treatments including neurobiological aspects of nervous regeneration. We suggest that electrodiagnostic examination after a radial nerve palsy caused by a humeral fracture is done 5-6 weeks after injury and that nerve repair and reconstruction shoul

Fixation of humeral surgical neck fracture using contoured pins versus straight pins: a mechanical study.

The conventional procedure for displaced fracture of the surgical neck of the humerus uses straight pins inserted from the lateral aspect towards the head of the humerus. The objective of this study was to compare the mechanical properties of fixation by contoured (curved) pins to those fixed with regular straight pins. A transverse osteotomy was made in 30 fresh-frozen sheep humeri in the proximal metaphyseal bone region, and pins were inserted using either three parallel straight pins or three contoured pins in different planes. The assemblies were subjected to bending or rotational stresses

Primary replacement of the humeral head in iatrogenically displaced fracture-dislocations of the shoulder: a report about six cases.

Dislocation of the shoulder is a common injury and may be associated with a variety of complications. We report six cases of primary replacement of the humeral head where closed reduction of a shoulder dislocation associated with an undisplaced fracture of the humeral neck led to displacement of the neck fracture. All dislocations examined were anterior with a displaced greater tubercle fracture. The patients had undergone closed reduction at other medical centres and were referred to us because of iatrogenically displaced fracture-dislocations of the shoulder. Three were women and three were

Effect of ketorolac administration on the rate of nonunion of operatively treated humeral shaft fractures: A matched cohort analysis.

BACKGROUND: Humeral shaft fractures treated surgically have a 5-10 % risk of nonunion. NSAIDs, including ketorolac, are frequently prescribed postoperatively for pain management, but concerns persist regarding their effects on bone healing. Although prior studies suggest a potential association between ketorolac and nonunion, findings remain inconclusive. This study aims to assess the impact of ketorolac on nonunion risk in adults undergoing surgical treatment for humeral shaft fractures. METHODS: The TriNetX Research Database was queried using ICD and CPT codes to identify patients who under

Medium-term clinical results in the treatment of supracondylar humeral fractures in children: does the surgical approach impact outcomes?

BACKGROUND: Recent literature has found a consensus in favor of conservative treatment for type II supracondylar humeral fractures (SCHF). This retrospective observational study compares the short- to medium-term functional outcomes of conservative versus surgical treatment in 31 patients with SCHF (Gartland II and III) to assess the potential superiority of one approach over the other. MATERIALS AND METHODS: Thirty-one pediatric patients treated for SCHF-19 classified as Gartland II and 12 as Gartland III-were assessed in our department. Eight patients underwent closed reduction and cast imm…
