Sex-hormone producing adrenal tumors causing behavioral changes as the sole clinical sign in 3 cats.

Three neutered cats with adrenocortical tumors that were presented with behavioral changes but no evidence of hyperaldosteronism or hypercortisolism are described. All 3 cats had resolution of their clinical signs following adrenalectomy. For neutered cats presenting with behavior changes, a sex-hormone secreting adrenal tumor should be considered as a differential diagnosis.

[Hypokalaemic paralysis as a presentation of adrenal tumor].

A 24-year-old patient presented with flaccid quadriplegia due to severe hypokaliemia, initially presumed to have been induced by glycyrrhizin. Persistence of low potassium levels and hypertension led to the diagnosis of primary hyperaldosteronism related to an adrenal cortical tumor. After surgery, the patient recovered from hypertension and hypokaliemia.

[Indications for surgery in adrenocortical disease].

The surgical treatment of adrenocortical diseases indicated for hormone-secreting neoplasms and for definite cases of Cushing's disease. The surgical indication will be discussed. Since primary hyperaldosteronism caused by nodular hyperplasia of the adrenals needs a medical rather than a surgical treatment, a preoperative diagnosis and location of aldosterone producing adenomas has to be performed.

Glucocorticoid-remediable aldosteronism in a young adult with a family history of Conn's syndrome.

Glucocorticoid-remediable aldosteronism is a hereditary form of primary hyperaldosteronism and the most common monogenic cause of hypertension. We present the case of a 24-year-old man with a family history of Conn's syndrome. Yet, in the index patient, classical characteristics of mineralocorticoid excess could be reversed by exogenous glucocorticoids.

[Edema caused by diuretic abuse].

We present the case of an "idiopathic" edema induced by the chronic use of diuretics in a 44-years-old patient. The patient showed a significant secondary hyperaldosteronism which have been erroneousley labelled as primary in previous studies. The pathogenicity of the idiopathic edema and its relation to diuretic abuse are discussed.

[Spironolactone in the treatment of hypertension: a neglected molecule].

Spironolactone, a mineralocorticoid receptor antagonist, is used in the treatment of hypertension for over 50 years. Due to the absence of morbidity and mortality studies, it is not considered to be a first-choice drug in the treatment of patients with primary hyperaldosteronism. However, it has a secure and stable position in the treatment of resistant hypertension. The effect of spironolactone on blood pressure lowering in antihypertensive combination therapy was demonstrated in several uncontrolled trials. The analysis of ASCOTBPLA study also contributed significantly to this area. In the l…

Clock genes and salt-sensitive hypertension: a new type of aldosterone-synthesizing enzyme controlled by the circadian clock and angiotensin II.

With the current societal norm of shiftwork and long working hours, maintaining a stable daily life is becoming very difficult. An irregular lifestyle disrupts circadian rhythms, resulting in the malfunction of body physiology and ultimately leading to lifestyle-related diseases, including hypertension. By analyzing completely arrhythmic Cry1/Cry2 double-knockout (Cry-null) mice, we found salt-sensitive hypertension accompanied by hyperaldosteronism. On the basis of a DNA microarray analysis of the adrenal gland and subsequent biochemical analyses, we discovered that Hsd3b6/HSD3B1, a subtype o…

Gene mutations that promote adrenal aldosterone production, sodium retention, and hypertension.

Primary aldosteronism (PA) is the most common form of secondary hypertension, found in about 5% of all hypertension cases, and up to 20% of resistant hypertension cases. The most common forms of PA are an aldosterone-producing adenoma and idiopathic (bilateral) hyperaldosteronism. Rare genetic forms of PA exist and, until recently, the only condition with a known genetic mechanism was familial hyperaldosteronism type 1, also known as glucocorticoid-remediable aldosteronism (FHA1/GRA). FHA type 3 has now been shown to derive from germline mutations in the KCNJ5 gene, which encodes a potassium c…

Contemporary approach to preoperative preparation of patients with adrenal cortex hormones dysfunction.

Preoperative preparation of the patients with adrenal cortex dysfunction is based on the careful preoperative evaluation of the type and the severity of the disturbance. The dysfunction involving adrenal glands may be: insufficiency (severe, mild, expressed) and hyperfunction (hypercorticism and/or hyperaldosteronism). If we speak about the patients with limited adrenal reserve (Addison's disease, therapeutic glucocorticoid application etc.) they need necessary corticosteroid supplementation, during preoperative preparation, as well as, during complete perioperative period. Doses needed for th…

TASK1 and TASK3 potassium channels: determinants of aldosterone secretion and adrenocortical zonation.

Potassium channels control the membrane voltage of aldosterone-producing zona glomerulosa cells. They are responsible for the unique K(+) se … thase occurred. In double mutant TASK1(-/-)/TASK3(-/-) mice, also adult male mice displayed primary hyperaldosteronism. Therefore, these knockout mice are interesting models to study mechanisms of autonomous aldosterone production and adrenocortical zonation. These data suggest that modifications of the adrenocortical K(+) conductances could also contribute to auton …

[New data on mineralocorticoid hormones. Physiopathological implications].

Our better understanding of the terminal steps of aldosterone production based on enzyme cloning has led to new clinical approaches to three aldosterone-related types of hypertension: mineralocorticoid excess, Liddle's disease and glucocorticoid-sensitive hyperaldosteronism. In all three cases, hypertension results from volume expansion due to increased reabsorption of sodium in the distal renal tube with decreased plasma renin activity and hypokaliaemia. Genetic testing now allows early diagnosis of these severe familial diseases and specific treatment. Although both glucocorticoid-sensitive …

[Debatable questions of strophanthin and spironolactone treatment in certain forms of acute myocardial infarct].

The results of clinical and instrumental examination of 67 patients and experimental findings provide evidence of myocardial resistance to cardiac glycosides in acute myocardial infarction complicated by the initial stage of cardiac insufficiency. It is suggested that symptoms of secondary hyperaldosteronism may develop in acute myocardial infarction and that medication with cardiac glycosides in combination with spironolactones is expedient.

An Endocrine Cause of Acute Post-partum Hypertension.

This is a case of acute peri-partum hypertension secondary to Conn's syndrome. The timing of presentation offers a rare insight into the hormonal physiology of pregnancy and its impact on blood pressure regulation. This case highlights the challenges of diagnosing primary hyperaldosteronism in the peripartum period and the high index of suspicion required by the obstetric physician.

Adrenal disorders in cats.

Although adrenal disorders are uncommon in cats, they are being recognized with increasing frequency. The low incidence and unique characteristics of adrenal disease in cats often make the diagnosis and treatment of these disorders challenging. This article summarizes information on the diagnosis and treatment of feline adrenal disorders, including hyperadrenocorticism, hypoadrenocorticism, pheochromocytoma, and hyperaldosteronism.

[Effect of isolated ultrafiltration on plasma hormone levels in patients with heart failure resistant to drug therapy].

The positive action exerted by isolated ultrafiltration on 69 patients with refractory heart failure was shown to be due to the complex interaction of effects produced by ultrafiltrate removal. Among the effects, a leading role is played by therapy for hyperaldosteronism and reduction of antidiuretic hormone levels along with improvement of the functional status of the liver and heart.

[Diagnosis of Conn's adenoma. Comparative study of x-ray computed tomography and scintigraphy using 19-noriodocholesterol].

OBJECTIVE: In order to differentiate an aldosterone producing adenoma (APA) and a bilateral adrenal hyperplasia (BAH) in case of primary hyperaldosteronism, an adrenal CT-scan is usually used as first line exploration. The contribution of adrenal 19-noriodocholesterol (NP59)-scintigraphy is controversial. PATIENTS AND METHODS: We describe 17 cases of primary hyperaldosteronism referred to surgery for suspected APA. The value of abdominal CT-scan and of adrenal scintigraphy was studied. RESULTS: After unilateral adrenalectomy, 15 cases with confirmed APA were cured and 2 cases with an unilate…
