Delay in seeking health facility and associated factors among tuberculosis patients in South Gondar Zone, Ethiopia, mixed methods study.

Tuberculosis is a major public health concern worldwide, particularly in low-income countries such as Ethiopia. Delays are among the greatest challenges in tuberculosis control. However, evidence is scarce regarding patient delay and context-specific contributors. Therefore, this study aimed to assess the delay in seeking health facilities and its contributors in the South Gondar Zone. A sequential explanatory mixed cross-sectional study with a phenomenological design was conducted among 332 tuberculosis patients from 26 public health facilities in the South Gondar Zone. Data were collected vi

Conversational Artificial Intelligence and Neuropsychiatric Risk: A Narrative Review and Case-Based Synthesis Proposing a Delusional Feedback Loop.

Conversational AI, powered by artificial intelligence, is becoming a common tool for accessing health information, educating patients, and obtaining general medical advice. These advanced systems, known as large language models, can produce responses that sound remarkably human. Nevertheless, these systems are prone to "AI confabulations," whereby they confidently generate incorrect information that could harm patients. This highlights the need to inform healthcare workers and individuals who may be prone to trusting these devices. New evidence suggests that AI may also exacerbate mental healt

It takes two to talk about prostate cancer: a qualitative assessment of African American men's and women's cancer communication practices and recommendations.

Prostate cancer (PrCA) is the most commonly diagnosed nonskin cancer among men. African American (AA) men in South Carolina have a PrCA death rate 150% higher than that of European American (EA) men. This in-depth qualitative research explored AA men's and women's current practices, barriers, and recommended strategies for PrCA communication. A purposive sample of 43 AA men and 38 AA spouses/female relatives participated in focus groups (11 male groups; 11 female groups). A 19-item discussion guide was developed. Coding and analyses were driven by the data; recurrent themes within and across g

Closing the Gap in Pre-Exposure Prophylaxis Access for Sexual and Gender Minorities in South Korea: Implications for HIV Prevention Policy.

Despite the availability of pre-exposure prophylaxis (PrEP) in the Republic of Korea, structural and social barriers continue to hinder its widespread adoption. This study aimed to examine longitudinal changes in PrEP awareness, utilization, and perceived barriers among sexual and gender minorities to inform national HIV prevention strategies. Two independent online cross-sectional surveys were conducted in January 2022 (DB1) and December 2024 (DB2), primarily targeting men who have sex with men; transgender individuals comprised 0.6% of DB1 and 10.6% of DB2. The surveys assessed sociodemograp

The Patient-AI Relationship in Obsessive-Compulsive and Related Disorders: A Cognitive-Behavioral Framework.

Generative artificial intelligence (AI) has evolved from a passive information tool into a responsive conversational system. For individuals with obsessive-compulsive and related disorders (OCRDs) that are, in part, maintained by safety behaviors and other negative reinforcement loops, large language models (LLMs) can function as highly available, potentially sycophantic, and readily repeatable sources of reassurance that may undermine the principles of exposure and response prevention (ERP). Measurement paradigms inherited from the early digital era (e.g., screen time and problematic internet

The CB1 receptor: linking impulsivity and substance use disorder.

The cannabinoid receptor type 1 (CB1R) is the most widely expressed G protein-coupled receptor in the brain, with high concentrations in the basal ganglia, hippocampus, and cerebellum. Predominantly localized in presynaptic terminals, CB1R modulates synaptic transmission through retrograde endocannabinoid signaling. Its expression follows an ontogenetic trajectory, with region- and age-specific patterns that are particularly dynamic during adolescence. This developmental window is characterized by marked neuroplasticity and heightened impulsivity, a trait closely associated with increased vuln

Involvement of the Dorsal Vagal Complex in Alcohol-Related Behaviors.

The neurobiological mechanisms that regulate the development and maintenance of alcohol use disorder (AUD) are complex and involve a wide variety of within and between systems neuroadaptations. While classic reward, preoccupation, and withdrawal neurocircuits have been heavily studied in terms of AUD, viable treatment targets from this established literature have not proven clinically effective as of yet. Therefore, examination of additional neurocircuitries not classically studied in the context of AUD may provide novel therapeutic targets. Recent studies demonstrate that various neuropeptide

Illegal behaviors in problem gambling: analysis of data from a gambling helpline.

Problem gambling behaviors, particularly the most severe form, which is pathological gambling (PG), represent an emerging public health problem. Compared with the general population, individuals with problem gambling are more likely to have reports of legal issues, including commission of crimes, arrest, and incarceration. The goal of the present study is to examine the characteristics of individuals seeking help for gambling problems with regard to reports of illegal behavior secondary to gambling. Individuals with gambling problems were identified through use of a 24-hour gambling helpline,

Lessons learned: Digital epidemiology of eosinophilic esophagitis and what Google Trends reveals.

Eosinophilic esophagitis (EoE) is a chronic, immune-mediated disease whose incidence and prevalence have continued to rise, yet little is known about how public awareness has evolved alongside clinical progress. We used Google Trends, a freely available tool that indexes the relative popularity of search queries from 0 to 100, to examine interest in the terms 'eosinophilic esophagitis' and 'EoE' in the United States from January 2004 through December 2025, and to compare the disease term with searches for regulatory-approved EoE-relevant therapies (dupilumab/Dupixent, budesonide/Eohilia, and b

Undiagnosed Sacrococcygeal Fistulas: A Case Report.

The clinical presentation of fistulas can include persistent drainage, pain or swelling. In the perianal, gluteal and sacrococcygeal areas, differential diagnosis may include perianal fistula, malignancy, inflammatory bowel disease or pilonidal disease. Lack of treatment can lead to disease progression. Patient appraisal of symptoms plays an important role in clinical outcomes, a role possibly shared with the shame of exposing symptoms. A behavior of avoidance and withholding of information can directly impact clinical outcomes by not seeking medical help and delays in treatment. This case des

Sensilla on Organs of Adults of the Pistachio-Seed Wasp Eurytoma plotnikovi (Hymenoptera: Eurytomidae).

The pistachio-seed wasp, Eurytoma plotnikovi Nikol'skaya, is a major pest of the biomass energy tree Pistacia chinensis. Understanding its sensory system is vital for uncovering host location mechanisms, but information on its olfactory system is limited. This study investigated the morphology of sensilla on the bodies of female and male E. plotnikovi. We identified seven types of sensilla on their antennae: sensilla trichodea I (ST I), sensilla trichodea II (ST II), sensilla trichodea III (ST III), sensilla chaetica (SC), sensilla placodea, multiporous grooved peg sensilla, and finger-like se

Plasma metabolic profiles in alcohol use disorder: diagnostic role of arginine and emotional implications of N6-acetyl-lysine and succinic acid.

BACKGROUND: Alcohol Use Disorder (AUD) poses a significant global health burden, yet its metabolic underpinnings remain poorly understood. The negative affective states that emerge during withdrawal drive relapse to alcohol-seeking behavior, highlighting the need for precise diagnostic criteria. METHODS: This exploratory study utilized targeted plasma metabolomics combined with bioinformatics, machine learning, and correlation analysis to identify biomarkers associated with AUD. Plasma samples from 20 AUD patients and 19 healthy controls were analyzed by liquid chromatography-mass spectrometr

Anhedonia is associated with higher functional connectivity between the nucleus accumbens and paraventricular nucleus of thalamus.

BACKGROUND: Anhedonia stands as a life-threatening transdiagnostic feature of many mental illnesses, most notably major depression and involves neural circuits for processing reward information. The paraventricular nucleus of the thalamus (PVT) is associated with reward-seeking behavior, however, links between the PVT circuit and anhedonia have not been investigated in humans. METHODS: In a sample of adults with and without psychiatric symptoms (n = 75, 18-41 years, 55 female), we generated an anhedonia factor score for each participant using a latent factor analysis, utilizing data from depr
