The correlation between the level of therapy adherence and inhalation technique in children with uncontrolled asthma using a smart inhaler: the IMAGINE study.

BACKGROUND: Asthma is a chronic condition with a high health and social burden in children. Although many well-studied effective therapies are available, because of suboptimal adherence and inhalation technique, asthma in children remains frequently uncontrolled. It is often assumed that adherence and technique are highly correlated items, but this assumption has not been thoroughly validated. OBJECTIVE: This trial evaluates the correlation between adherence and inhalation technique and the association of patient-related factors with these two important parameters. DESIGN: Observational phas…

Is Inhaled Colostrum as Effective as Inhaled Lavender Essential Oil for Pain Control in Neonatal Frenotomies? A Prospective, Randomized Clinical Trial.

BACKGROUND/OBJECTIVES: Neonatal pain must be treated due to its potential short- and long-term adverse effects. A frenotomy is a painful procedure where common strategies to relieve pain (oral sucrose solutions and sucking) cannot be used because the technique is performed on the tongue. Lavender essential oil (LEO) is useful in treating pain during blood sampling, heel punctures, vaccination, and frenotomies. We aimed to determine whether smelling colostrum had similar effects as inhaled LEO during frenotomies. METHODS: A prospective, randomized clinical trial was carried out with neonates w

Therapeutic Segmental Pulmonary Lavage for Methanol Inhalation Poisoning in a Shrimp Processing Plant Worker: A Novel Approach to Toxic Inhalants.

Inhalation of methanol vapor is a rare but potentially life-threatening occupational hazard, particularly in environments with inadequate ventilation and safety controls. We report the case of an 18-year-old male shrimp processing worker who developed acute respiratory failure following accidental inhalation of methanol vapors mislabeled as kerosene. The patient presented with severe dyspnea, hypoxemia, metabolic acidosis, and radiographic findings of diffuse pulmonary infiltrates. He required mechanical ventilation, corticosteroids, antibiotics, and intensive supportive care. Bronchoscopy rev…

Switch to non-CFC inhaled corticosteroids: a comparative efficacy study of HFA-BDP and CFC-BDP metered-dose inhalers.

Inhaled corticosteroids, such as beclomethasone dipropionate (BDP), recommended for the treatment of persistent, mild, moderate, or severe asthma, have traditionally been administered via chlorofluorocarbon (CFC) propellant. The imminent phasing out of CFCs from pharmaceutical preparations due to the Montreal Protocol means patients will have to switch to a CFC-free alternative. One such preparation is hydrofluoroalkane-BDP (HFA-BDP), a press-and-breathe metered-dose inhaler. This 8-week, open-label, multicentre study assessed asthma control in patients switching from CFC-BDP to HFA-BDP (QVAR)

[Development of an Innovative Training Tool for Inhalers - Inhalation training for patients in practices, hospitals and pharmacies during times of climate change].

BACKGROUND: Cornerstones of inhalation therapy are training in the correct use of inhalers and selecting the most suitable method of administration. At the same time, guidelines call for participatory therapy decisions and, increasingly, consideration of ecological impacts. METHODS: As part of a 12-month pilot study a practical training tool was developed at the Hospital Havelhöhe in Germany. The tool uses a visual traffic-light system to integrate technique, equipment suitability, sustainability, and climate impacts. The material was iteratively revised in 49 group training sessions (n = 145…

Proximal policy optimisation based black-box optimizer for tailoring inhalation instructions of an inhaler for minimising mouth-throat deposition.

Inhalation therapy using dry-powder inhalers (DPIs) offers targeted drug delivery for asthma and chronic obstructive pulmonary disease (COPD). Variation in deposition rate in the mouth-throat region depends on patient inhalation manoeuvres and techniques. The present study aimed to identify optimal combinations of head position, oral opening size, and mouthpiece insertion depth for three representative inhalation manoeuvres, corresponding to low, medium and high peak inspiratory flow rates (PIFRs). An idealised adult airway model was reconstructed and coupled with transient computational fluid…

Impact of taste changes caused by inhalers on adherence to inhalation therapy among patients with asthma and chronic obstructive pulmonary disease.

BACKGROUND: Adherence to inhalation therapy impacts treatment outcomes of asthma and chronic obstructive pulmonary disease. Although taste changes induced by inhaled medications may influence adherence, this matter remains underexplored. This study aimed to elucidate the characteristics of taste changes caused by inhaled medications and their association with adherence. METHODS: Consecutive patients who regularly visited our hospital from August 2023 to December 2023 were recruited. Sweet, bitter, sour, and salty taste changes were assessed using a visual analog scale. Adherence was measured …

Comparison of Fixed-Dose Inhaled Epoprostenol and Inhaled Nitric Oxide for Acute Respiratory Distress Syndrome in Critically Ill Adults.

PURPOSE: Several reports have demonstrated similar effects on oxygenation between inhaled epoprostenol (iEPO) compared to inhaled nitric oxide (iNO) for acute respiratory distress syndrome (ARDS). Previous studies directly comparing oxygenation and clinical outcomes between iEPO and iNO exclusively in an adult ARDS patient population utilized a weight-based dosing strategy. The purpose of this study was to compare the clinical and economic impact between iNO and fixed-dosed iEPO for ARDS in adult intensive care unit (ICU) patients. METHODS: This retrospective cohort study was conducted at a m

Efficiency of Ipratropium Bromide and Albuterol Deposition in the Lung Delivered via a Soft Mist Inhaler or Chlorofluorocarbon Metered-Dose Inhaler.

The propellant-free Combivent Respimat Soft Mist Inhaler (CVT-R) was developed to replace the chlorofluorocarbon-propelled Combivent metered-dose inhaler (CVT-MDI). This steady-state pharmacokinetic (PK) substudy evaluated drug lung-delivery efficiency, using data from two phase III safety and efficacy trials. PK parameters were obtained from well-controlled population PK analyses. Area under the plasma concentration-time curve (AUC), maximum observed plasma concentration (C(max)), and minimum observed plasma concentration (C(min)) showed systemic exposure to ipratropium bromide and albuterol

In vivo responses to inhaled proteins. II. Induction of interstitial pneumonitis and enhancement of immune complex-mediated alveolitis by inhaled concanavalin A.

An animal model of environmental lung disease is described in which phytomitogen, antigen, or both, are administered in aerosol form to previously immunized or immunologically naive rabbits. Inhalation of concanavalin A alone induced an interstitial pneumonitis in nonimmunized rabbits. Inhalation of concanavalin A alone induced an interstitial pneumonitis in nonimmunized rabbits. Inhalation of bovine serum albumin (BSA) alone typically produced only focal eosinophilic granulomas in BSA-immunized animals, and no injury whatever in nonimmune animals. However, simultaneous administration of BSA-c…

Clinical Pharmacokinetics of Inhaled Drugs for Pulmonary Hypertension.

Pulmonary hypertension (PH) is a serious condition characterized by elevated blood pressure in the pulmonary arteries. Current treatment approaches mainly focus on using vasodilator agents to reduce pulmonary blood pressure and improve blood flow. Inhalation treatments offer targeted delivery to the lungs, improving efficacy and reducing systemic side effects. Understanding the pharmacokinetics (PK) of the molecules used in the inhalation treatment is crucial for dose optimization and drug product development. This review examines the clinical PK characteristics of key inhaled PH drugs (approv…

Inhaled Allergy Diagnostics and Treatment in a Polluted Environment.

Allergic diseases have been increasing in prevalence over the last years. In a polluted environment, this problem can worsen and become more complex. Long-term exposure to air pollution can lead to the aggravation of allergic rhinitis (AR) and even to the development of seasonal asthma. Climate changes can accelerate and extend the pollination season. Research indicates that air pollution may modify the properties of pollen, making it more aggressive. Asymptomatic allergic people disclose their allergies in a polluted environment. A polluted environment complicates the diagnosis of seasonal al
