Late-onset neonatal sepsis (LOS) in very low birth weight (VLBW) preterm infants varies in severity from mild to fatal and often presents with cardiorespiratory instability. We hypothesized that heart rate (HR) and systemic oxygenation (SpO2) patterns would differ based on pathogen type and mortality. We analyzed HR and SpO2 changes before LOS diagnosis for Gram-negative, Coagulase-negative Staphylococcus, and other Gram-positive bacteria (GN, CONS, OGP) and compared survivors and nonsurvivors. Using continuous every-2-second vital sign data from 365 VLBW infants in four neonatal intensive care

Neonatal jaundice is a significant cause of neonatal morbidity worldwide and accounts for 75% of hospital readmissions in the first week of life. New-born babies can develop severe neonatal jaundice that may cause irreversible brain damage or even death. To assess the knowledge, attitude and practices of registered nurses on neonatal jaundice among neonates at Neonatal Intensive Care Unit at Windhoek Central hospital. A quantitative, descriptive, cross sectional research design was used. The population was all 34 registered nurses working at Windhoek Central Hospital neonatal intensive care un

Neonatal toxic shock syndrome-like exanthematous disease (NTED) is a new entity of methicillin-resistant Staphylococcus aureus (MRSA) infection. Most of NTED cases reported previously in the literature were sporadic ones. In the present report, we describe an outbreak of NTED that occurred in a neonatal intensive care unit (NICU) between April, 1999 and April, 2000 in Japan. All MRSA strains isolated from 14 patients (6 NTED, 2 infections and 6 colonizations) in this outbreak belonged to the group of coagulase II and produced toxic shock syndrome toxin 1 (TSST-1). Of these, 14 strains produced

Maintenance fluids for neonates, which are commonly composed of glucose 10%, sodium chloride (NaCl) 0.225% w/v with potassium chloride (KCl) 20 mmol/L (GSP), are administered as a continuous infusion, and prescribed medications are required to be co-administered frequently through a Y-site. There are limited studies on the compatibility of commonly used medicines with this maintenance fluid. Therefore, the objective of this study is to study the compatibility of commonly used medicines in the neonatal intensive care unit with maintenance fluid (GSP). An audit at the RCH Neonatal Intensive Care

INTRODUCTION: NICU sepsis evaluation balances rapid antibiotic administration for mortality reduction against unnecessary treatment. Objective risk stratification optimizes resource allocation.
METHODS: We retrospectively studied 191 sepsis evaluations from 136 NICU patients. Sepsis was defined as positive blood culture or clinical sepsis with ≥5 days of antibiotics. A machine learning score (POWS) used cardiorespiratory data 1 h prior to evaluation to quantify sepsis risk. We performed ROC analysis, identified optimal thresholds by Youden Index, and assessed postmenstrual age (PMA) and gesta

INTRODUCTION: Infection control measures against methicillin-resistant Staphylococcus aureus (MRSA) are challenging in advanced neonatal intensive care units (NICUs) providing care for high-risk newborns. To design and implement better infection control strategies, it is necessary to identify endemic MRSA clones that are recurrently detected and understand how they survive and transmit in the NICU.
METHODS: We performed whole-genome sequencing (WGS) for all MRSA strains isolated in the level II-IV NICU in Kyushu University Hospital, Fukuoka, Japan, from March 2015 to December 2021. Strains wi

PURPOSE OF REVIEW: Exposures to endocrine disrupting chemicals (EDCs) in early life have demonstrable adverse implications on child health and development. Yet, there is a dearth of studies evaluating the potential exposures to EDCs, such as bisphenols, parabens, phthalates, and volatile organic compounds (VOCs), in hospital-based settings among children who are critically ill and/or particularly vulnerable. This narrative review seeks to provide up-to-date evidence on the sources and magnitude of exposure to EDCs in neonatal-, pediatric-, and cardiac intensive care units (NICUs/PICUs/CICUs) a

BACKGROUND: Serratia marcescens complex (SMC) is a group of opportunistic pathogens frequent in neonatal intensive care unit (NICU) outbreaks. We report a seven-month NICU outbreak of multidrug-resistant (MDR) SMC strain and assess the acquisition risk factors.
METHODS: Bacterial isolates were sequenced using Illumina technology and profiled for antimicrobial resistance (AMR). Cases (culture positive for SMC) were compared with birth date and ward-matched controls (SMC negative) to identify clinical and environmental factors associated with SMC acquisition.
RESULTS: A single strain of Serrat

Bloodstream infections caused by Staphylococcus epidermidis are among the most frequent and serious infections in neonatal intensive care units (NICU), being responsible for high rates of morbidity and mortality. However, how S. epidermidis prevails, disseminates and evolves to cause outbreaks in NICU is poorly understood. We aimed to understand what is the genomic basis of S. epidermidis outbreaks in NICU to provide guidelines for a better infection control. We collected all S. epidermidis isolated from newborn blood cultures admitted to a hospital NICU in Brazil over one-year and compared th

BACKGROUND: The global preterm birth rate is currently around 10%, making it a major contributor to both neonatal mortality and long-term health complications. Length of stay (LOS) in hospital is a key metric when assessing the quality of medical care for preterm infants. Therefore, a comprehensive analysis of LOS in preterm infants, along with the identification of associated risk factors, is essential in order to improve clinical outcomes and optimize healthcare strategies.
OBJECTIVE: This study examined the LOS and total hospitalization costs of preterm infants admitted to neonatal intensi

OBJECTIVE: Even with all the worldwide effort in the care of pregnant women and newborns in the last 10 years, perinatal asphyxia remains a crucial public health problem worldwide, being responsible for the high rates of morbidity and mortality in term and preterm newborns. In this perspective, research was carried out which aimed to investigate the prevalence and analyze the environmental and maternal-fetal risk factors associated with perinatal asphyxia.
METHODS: This cohort study (2013-2018), corresponding to six years, was conducted in Fortaleza, Ceará, Brazil. The study involved 480 post

BACKGROUND: The AAP recommends that infants born <37 weeks gestation or with risk factors for positional cardiorespiratory instability undergo a car seat tolerance screen (CSTS) before discharge. Understanding characteristics of CSTS failure may inform screening practices.
OBJECTIVE: Compare clinical, oxygen saturation, and heart rate features in infants who failed vs. passed CSTS.
METHODS: We retrospectively reviewed CSTS data from a single level IV NICU (2018-2024). Clinical characteristics of infants who failed vs. passed their first CSTS were compared. For a subset with SpO₂ and HR data,

Recently, the usefulness of near-infrared spectroscopy-based cerebral oximetry in extremely preterm infants to prevent death or severe brain injury was evaluated in a study called SafeBoosC-III (N = 1601). In this study, the incidence of both outcomes was similar between the cerebral oximetry and the control group. In a previous smaller study called SafeBoosC-II (N = 166), there was a borderline significant advantage for cerebral oximetry. There is therefore a highly significant (p = 0.010) difference between the two outcomes. What could be the reason for the difference? One major difference w

The fear of missing sepsis episodes in neonates frequently leads to indiscriminate use of antibiotics, and prescription program optimization is suggested for reducing this inappropriate usage. While different authors have studied how to reduce antibiotic overprescription in the case of early onset sepsis episodes, with different approaches being available, less is known about late-onset sepsis episodes. Biomarkers (such as C-reactive protein, procalcitonin, interleukin-6 and 8, and presepsin) can play a crucial role in the prompt diagnosis of late-onset sepsis, but their role in antimicrobial

BACKGROUND: In the context of healthcare, satisfaction is the attainment of adequate or acceptable treatment and serves as both a benchmark for quality and the ultimate objective of providing care. In neonatal care facilities, parent satisfaction is a key measure of the quality of the services offered to the newborns and aids in improving healthcare delivery. This is the first systematic review aiming to address critical knowledge gaps regarding factors influencing parental satisfaction with neonatal intensive care unit services, and determine pooled prevalence in Ethiopia.
METHODS: After com
