Tumor Recurrence or Radionecrosis: A Clinical Dilemma in Long-Term Survivors of Low-Grade Diffuse Glioma With Drug-Resistant Epilepsy.

Low-grade diffuse gliomas with isocitrate dehydrogenase (IDH) mutations predominantly affect young adults and are frequently associated with prolonged survival, requiring extended neuro-oncological follow-up. In these patients, differentiating tumor recurrence from radionecrosis after radiotherapy remains a major diagnostic challenge because both entities may present with overlapping clinical manifestations and conventional imaging findings. We present the case of a 35-year-old man with an IDH-mutant grade 2 astrocytoma located in the left temporo-parieto-insular region who underwent subtotal

Integrated Metabolomic and Proteomic Analyses Reveal Differential Regulation of the Citrate Cycle and Pentose Phosphate Pathway by Cannabis sativa Extract and the Synthetic Cannabinoid HU210 in GT1-7 Neuronal Cells: Sequential Extraction of Metabolites and Proteins from a Single Cell Pellet.

BACKGROUND: Cannabis contains Δ9-tetrahydrocannabinol (THC) together with numerous other constituents, whereas HU210 (HU) is a synthetic full agonist at cannabinoid receptors. We applied integrated metabolomics and proteomics to compare Cannabis extract (CS) and HU in GT1-7 neuronal cells. METHODS: Cytotoxicity was evaluated using a lactate dehydrogenase assay. Metabolites and proteins were extracted sequentially from single-cell pellets and analyzed using gas chromatography-tandem mass spectrometry and nano-liquid chromatography-quadrupole-Orbitrap mass spectrometry, respectively. Cannabinoi

Proteome-wide profiling of lysine β-hydroxybutyrylation in Reynoutria japonica (Huzhang) and its dynamics under hydrogen sulfide stress.

Reynoutria japonica is both an important industrial source of resveratrol and a medicinal plant notable for its strong environmental adaptability. However, its responses to hydrogen sulfide (H2S), a key environmental signaling molecule, remain largely unexplored. Lysine β-hydroxybutyrylation (Kbhb) is a newly identified short-chain fatty acylation, but its role in regulating plant stress responses is unclear. Here, we performed the first proteome-wide profiling of Kbhb in R. japonica, identifying 17,102 high-confidence sites across 6185 proteins. Motif analysis revealed six conserved motifs. F

Identification and Management of Differentiation Syndrome in Emergency Settings: A Narrative Review.

Background: Differentiation therapy is a cornerstone in treating hematologic malignancies, particularly acute myeloid leukemia (AML). Differentiation agents target molecular defects blocking myeloid differentiation. However, rapid differentiation can precipitate a life-threatening complication, differentiation syndrome (DS). DS manifests with fever, pulmonary infiltrates, pleural or pericardial effusions, hypotension, and organ dysfunction, often mimicking sepsis or infection. Early recognition in the emergency department (ED) is critical to mitigate morbidity and mortality. This review aims t

Antileukemic therapies for older adults with AML ineligible for conventional therapy: systematic review and meta-analysis.

Older adults with newly diagnosed acute myeloid leukemia (AML) are often ineligible for conventional "7+3" induction chemotherapy. Despite recent drug approvals, treatment outcomes remain poor in this population. We conducted an updated systematic review to inform the 2025 American Society of Hematology (ASH) AML update guidelines in older adults. This review compared the efficacy and safety of low-dose cytarabine (LDAC), azacitidine (AZA), 5- and 10-day decitabine (DEC), and gemtuzumab ozogamicin, alone or combined with drugs such as venetoclax (VEN), in older adults with AML ineligible for c

A Deep Learning-Driven Framework Integrating Organoid-Based Functional Validation Identifies Universal Neoantigens from Recurrent Glioma Mutations.

UNLABELLED: Glioblastoma (GBM) is the most common malignant intracranial tumor in adults, with a median survival of only 16 to 20 months. Neoantigen therapy has shown advantages in the treatment of GBM, as it improves the immunosuppressive microenvironment within the tumor. However, the identification of truly immunogenic neoantigens remains a major challenge. Current computational prediction tools primarily focus on antigen presentation, whereas algorithms that incorporate T-cell immunogenicity features remain limited. Furthermore, standard validation methods, such as enzyme-linked immunospot

Prognostic impact of co-mutations in adults with IDH1/2-mutated acute myeloid leukemia.

Acute myeloid leukemia (AML) is characterized by the accumulation of cytogenetic and molecular abnormalities. Isocitrate dehydrogenase 1 and 2 (IDH1/2) mutations occur in 11% to 20% of adults with AML. The outcome of IDH1/2-mutated AML is heterogeneous and affected by co-mutational patterns. We retrospectively analyzed 118 patients with IDH1/2-mutated AML who were retrieved from 1597 patients newly diagnosed with AML and treated with intensive chemotherapy. Univariate analysis revealed the NPM1 mutation was a favorable factor (p = 0.019) for overall survival (OS), whereas the DNMT3A mutation w

IDH2 mutation is associated with favorable outcome among older adults with newly diagnosed acute myeloid leukemia treated with hypomethylating agent-based therapy.

Mutations of isocitrate dehydrogenase (IDH) are recurrent in newly diagnosed acute myeloid leukemia (AML) and their prevalence increases with age. The prognostic impact of IDH mutations in AML remains controversial. IDH inhibitors generally have a favorable side-effect profile, making them an attractive option for older patients. This retrospective analysis aimed to describe the prevalence and prognostic impact of IDH mutations in a large cohort of newly diagnosed AML patients aged ≥60 years enrolled in the Beat AML clinical trial. A total of 1,023 patients were included. IDH mutations were de

Effects of re-challenge with temozolomide in grade 2/3 IDH mutant gliomas at first progression.

BACKGROUND: Patients with WHO grade 2 and 3 isocitrate dehydrogenase mutation (IDHmt) gliomas commonly receive temozolomide (TMZ), with or without radiation therapy, as initial therapy. At progression, TMZ is sometimes reinstated despite a paucity of data on effectiveness. METHODS: We reviewed imaging outcomes of patients with WHO 2016 grade II/III IDHmt gliomas re-treated with TMZ at first progression between 2007 and 2019. Tumor growth rates were calculated over the year preceding re-treatment and throughout the re-treatment period, ranging from 3 to 41 months. RANO criteria were utilized t

Outcome of patients with accelerated and blast-phase myeloproliferative neoplasms not eligible for intensive chemotherapy or allogeneic hematopoietic cell transplantation treated by azacitidine alone or in combination-A FIM study.

Accelerated-phase (AP) or blast-phase (BP) myeloproliferative neoplasms (MPNs) are associated with dismal prognosis, with non-curative therapies such as hypomethylating agents (HMAs) considered in patients not eligible for intensive therapy, while some studies advocate for combination therapy with either ruxolitinib (RUXO) or venetoclax (VEN). To assess the relationship between treatment modalities and outcome, herein, we report a multicentric cohort of 149 patients (median age, 75 years) with AP/BP MPN not eligible for intensive therapy and/or allogeneic hematopoietic cell transplantation who

Identification of drug targets in pan-drug resistant Acinetobacter baumannii via whole genome sequencing and subtractive genomics.

In this study, we report, to the best of our knowledge, the first complete genome sequence of a pan drug resistant (PDR) Acinetobacter baumannii strain (JRCGR-AK-AB-01) from Karachi, Pakistan. Strain JRCGR-AK-AB-01 exhibited a pan drug resistant phenotype, showing susceptibility only to polymyxin B and intermediate susceptibility to colistin. Hybrid genome sequencing using MinION long-reads and DNBSEQ short-reads revealed that the genome size of strain JRCGR-AK-AB-01 is 4.03 Mb. We identified that JRCGR-AK-AB-01 is closely related to other A. baumannii strains based on Average Amino Acid Ident

Tumefactive Demyelinating Lesion Mimicking Low-Grade Astrocytoma with a T2/FLAIR Mismatch Sign: A Case Report and Review of the Literature.

Background and Clinical Significance: Tumefactive demyelinating lesions (TDLs) are large demyelinating lesions that mimic intracranial tumors, posing a diagnostic challenge in both clinical presentation and conventional imaging. Distinguishing TDLs from central nervous system tumors can be challenging due to their similar imaging appearances. Specific magnetic resonance imaging (MRI) features such as open-ring contrast enhancement, mild mass effect, lack of cortical involvement, and rapid responsiveness to corticosteroids favor a demyelinating etiology of the lesion. This report presents a cas

Cost-effectiveness of ivosidenib versus chemotherapy for previously treated IDH1-mutant advanced intrahepatic cholangiocarcinoma in Taiwan.

BACKGROUND: International guidelines recommend ivosidenib followed by modified FOLFOX (mFOLFOX) for advanced intrahepatic cholangiocarcinoma (ICC) with isocitrate dehydrogenase 1 (IDH1) mutations. Taiwan National Health Insurance covers only fluorouracil/leucovorin (5-FU/LV) chemotherapy for this ICC group, and there has been no prior economic evaluation of ivosidenib. Therefore, we aimed to assess ivosidenib's cost-effectiveness in previously treated, advanced ICC-presenting IDH1 mutations compared with mFOLFOX or 5-FU/LV. METHODS: A 3-state partitioned survival model was employed to assess

Enasidenib-induced hepatitis in an individual with Type II D2-hydroxyglutaric aciduria.

Type II D-2-Hydroxyglutaric aciduria (T2D2HGA) is caused by a gain-of-function pathogenic variant in Isocitrate Dehydrogenase 2 (IDH2). Patients with T2D2HGA commonly present with developmental delay, seizures, cardiomyopathy, and arrhythmias. The recently approved IDH2-inhibitor Enasidenib targets the p.Arg140Gln pathogenic IDH2 variant and decreases production of D2HGA. We present a 7-year-old female with T2D2HGA due to the p.Arg140Gln variant. She was diagnosed at 3-years-old after presenting with global developmental delay, leukoencephalopathy, communicating hydrocephalus, seizures, and di
