[The lumbar infusion test and transcranial Doppler ultrasonography in the diagnosis and treatment of normotensive hydrocephalus. Prospective evaluation of a group of patients from 2000 to 2002].

BACKGROUND: The normal pressure hydrocephalus syndrome (NPH) is characterized by the presence of a clinical triad comprising dementia, motor apraxia, and sphincter complaint, along with the presence of distinct dilatation of the ventricular system in the absence of major cortical atrophy. The diagnosis of NPH must be confirmed by including dynamic liquor tests into the algorithm of examination. The lumbar infusion test (LIT) represents the "gold standard" of liquor dynamic tests. METHODS AND RESULTS: The authors present 24 patients in a prospective managed study. Indication of shunt surgery w

[Warm saline enema for reduction of intestinal invagination under the ultrasonographic guidance: preliminary results apropos of 2 cases].

The authors describe the use of isotonic normal saline enema under US guidance in the treatment of acute uncomplicated intestinal intussusception in infants. The technique was successfully used in two patients aged respectively 18 and 16 months, with 11 and 9-month follow-up. The various steps of this technique and the results are analyzed and discussed. It is a valuable alternative to surgical treatment or hydrostatic reduction with opaque x-ray contrast under fluoroscopic guidance.

Rapid termination of intraoperative stimulation-evoked seizures with application of cold Ringer's lactate to the cortex. Technical note.

One major risk of intraoperative stimulation mapping is the production of stimulation-evoked seizures. Cold Ringer's lactate solution was applied directly to the irritated cortex in 22 patients with stimulation-induced seizures that occurred during intraoperative brain mapping procedures. The irrigation rapidly and reliably terminated these simple partial seizures and eliminated the need for intravenously administered short-acting barbiturates with antiepileptic properties. The authors describe a practical and simple method for controlling stimulation-induced seizure activity during brain mapp

Pseudomyxoma peritonei: does a true mucolytic agent exist? In vitro and in vivo studies.

Reproducibility of the so-called "mucolytic effect" of the 2%-10% dextrose-water solution is questioned. To test the mucolytic effect of 5% and 10% dextrose-water solutions and to determine what could be a true mucolytic agent, in vitro and in vivo studies were undertaken in two proven pseudomyxoma peritonei cases. In vitro study: Immediately after the mucin jelly was taken out of the peritoneal cavity, the jelly was immersed in various 40 cc solutions including: 1) 5% dextrose-water; 2) 10% dextrose-water; 3) normal saline; 4) lactated Ringer; 5) distilled water. The mucolytic effects of thes

Amnioinfusion: does the choice of solution adversely affect neonatal electrolyte balance?

OBJECTIVE: To determine whether various solutions commonly used in amnioinfusion during labor affect neonatal electrolyte and blood gas values. METHODS: Amnioinfusion for thick meconium or severe variable fetal heart rate decelerations is used at our institution according to a standardized protocol. During alternating 3-week periods, the only solution made available for amnioinfusion was either normal saline or Ringer's lactate. Bolus volume, rate, and duration of infusion were determined by the individual physicians. At delivery, cord blood was collected for electrolyte and blood gas determi

Modulation of cat antral slow waves by ion substitution, Ca2+ and K+ channel blockade, and ACh stimulation.

Two distinct slow waves can be recorded from smooth muscle cells on the submucosal border (submucosal cells) and the myenteric border (myenteric cells) in the circular layer of the cat antrum. The present studies examined the effects of decreasing extracellular Na+ ([Na+]o) or Ca2+ concentration ([Ca2+]o), Ca2+ and K+ channel blockade, and acetylcholine (ACh) stimulation on "submucosal" and "myenteric" slow waves using intracellular recording techniques. Decreasing [Na+]o (30 mM) reduced the upstroke depolarization of slow waves from submucosal cells but had no effect on the upstroke depolariz

Protective effects of preheparinization on the microvasculature during and after hemorrhagic shock.

Despite the fact that trauma patients are not heparinized at the time of injury, most models of hemorrhagic shock involve heparinization before the initiation of hemorrhage. To determine whether preheparinization has any effects on the microvasculature of organs during or after hemorrhage, rats (with and without preheparinization) were bled to a mean arterial blood pressure (BP) of 40 mm Hg. In two groups, this pressure was maintained by withdrawing additional blood until BP could no longer be maintained at 40 mm Hg (maximal bleedout, MB). In another two groups, the BP was maintained at 40 mm

Mechanisms regulating neurohypophysial blood flow and function during isotonic volume expansion.

Regional cerebral and neurohypophysial blood flow responses to isotonic extracellular fluid volume expansion were studied in pentobarbital-anesthetized dogs using radiolabeled microspheres. Measurements were made at baseline and after increasing pulmonary capillary wedge pressure by 8 +/- 2 mmHg. Plasma arginine vasopressin (AVP) decreased by 50%, and neural lobe blood flow (NLBF) decreased by 47%. Blood flow through median eminence and other brain regions was unchanged. We investigated potential mechanisms responsible for the NLBF changes. Following control vagotomy, AVP concentration increas

Adverse effects of topical prostacyclin application in microvascular surgery: an experimental study.

The efficacy of topical prostacyclin as an antithrombotic agent was tested in a model of microvascular trauma. Preliminary measurements were made to determine a suitable dosage. Twenty-seven central arteries of rabbit ears were then prepared and 32P-labelled platelets infused intraaortically. Arteriotomy (7 mm) was followed by intimectomy (5 mm). Fifteen vessels in a control group were irrigated with Ringer's lactate and 12 vessels in an experimental group were treated with prostacyclin (10 ng/ml) in normal saline. Bleeding times at the sites of arteriotomy intimectomy, in vivo accumulations o

Hydroxyethyl starch 120, dextran 70 and acetated Ringer's solution: hemodilution, albumin, colloid osmotic pressure and fluid balance following replacement of blood loss in pigs.

Twenty healthy pigs weighing 12-17 kg were anesthetized and the small intestines were exteriorized into saline-moistened gauze. During a 2-h period 4% of the animals' body weight was bled through an arterial cannula in six increments and replaced immediately by the fluid tested: hydroxyethyl starch 120 (HES, Plasmafusin, Orion Corp., Mw 120,000), dextran 70 (DEX) and Ringer's acetate (RA). The amount of fluid infused for replacement of blood loss was equal to the amount of blood withdrawn in the colloid groups but fourfold in the RA group. Five non-bled pigs served as controls. After the hemod

Traditional vs. modified Ringer lactate-based del Nido cardioplegia impacts on clinical outcomes in patients undergoing coronary artery bypass grafting: Results from a prospective, randomized, blinded trial.

INTRODUCTION: Myocardial protection with cardioplegia is a crucial approach to mitigate myocardial damage during coronary bypass grafting surgery (CABG) with cardiopulmonary bypass (CPB). The major component of the del Nido cardioplegia solution, Plasma-Lyte A, is difficult to obtain in Iran due to high cost. The objective of the current study was to study if the lactated Ringer's solution as the base for del Nido solution (LR DN) usage is a viable option as a substitute for Plasma-Lyte A in adult patients presenting for CABG surgery. STUDY DESIGN AND METHODS: The present prospective, randomi
