Evaluation and impact of fetal physiology training on fetal heart rate analysis.

INTRODUCTION: Evaluation of fetal well-being during labor is based on fetal heart rate (FHR) analysis, which requires physiology expertise. The aim of the present study was to assess medical residents' fetal physiology training in terms of theoretical knowledge, FHR interpretation, and use of second-line examinations. METHODS: This single-center, prospective study of obstetrics and gynecology residents (N = 34) at CHU de Lille Hospital (Lille, France) was conducted from November 2017 to November 2018. Evaluation and training were conducted in three stages. First, residents' pre-training knowl

An observational study of umbilical cord clamping practices of maternity care providers in a tertiary care center.

BACKGROUND: Severing the umbilical cord at birth is likely the oldest intervention, the timing of which remains fraught with controversy. Emerging evidence suggests benefit in delaying cord clamping for both term and preterm infants. The objective of this study was to investigate actual cord clamping time and circumstances at a large tertiary care center in Canada. METHODS: We used a stopwatch to time the interval from the time the infant was born as far as the umbilicus until the time that the umbilical cord was clamped before cutting. We reported on timing of the umbilical cord clamping ove

New challenges in operative vaginal delivery: the Odon device. A systematic review.

Operative vaginal delivery is the best option for accelerating labor in the second stage labor; however, this technique requires precise clinical assessment, customized care, skilled and appropriately educated workers, in order to avoid both maternal and fetal complications. The aim of this systematic review is to analyze the innovation, design, and effectiveness of a new instrument for Operative Vaginal Delivery, the Odon device, in order to improve intrapartum obstetric care in clinical practice, to facilitate the instrumental vaginal birth, and increase maternal and neonatal outcomes. A lit

The impact of perineal warm compress implementation on obstetric anal sphincter injury: initial evaluation of a quality improvement project.

BACKGROUND: Obstetric anal sphincter injury (OASI) is a severe complication of vaginal birth. Perineal warm compress during the second stage of labor has been shown to decrease the rate of OASI in randomized controlled trials. OBJECTIVES: This retrospective study evaluated the outcomes of a Quality Improvement (QI) project whose aim was to increase the use of warm compress on a labor and delivery unit at a tertiary-care center. STUDY DESIGN: We included all vaginal births ≥24+0 weeks' gestation occurring on the labor and delivery unit in the thirteen months before (June 2023 to June 2024) an

Using Magnetic Resonance Imaging During Childbirth to Demonstrate Fetal Head Moldability and Brain Compression: Prospective Cohort Study.

BACKGROUND: Childbirth is a physiological process with significant medical risk, given that neurological impairment due to the birthing process can occur at any time. Improvements in risk assessment and anticipatory interventions are constantly needed; however, the birthing process is difficult to assess using simple imaging technology because the maternal bony pelvis and fetal skeleton interfere with visualizing the soft tissues. Magnetic resonance imaging (MRI) is a noninvasive technique with no ionizing radiation that can monitor the biomechanics of the birthing process. However, the effect

Parity-stratified risk factors for obstetric anal sphincter injuries: A retrospective cohort study.

OBJECTIVE: Obstetric anal sphincter injuries (OASIS) are among the most serious complications of vaginal delivery. While various prediction models have been proposed, most fail to stratify risk by parity and commonly rely on birth weight, a parameter unavailable during labor, rather than estimated fetal weight (EFW). The aim of the present study was to identify maternal and intrapartum risk factors for OASIS in term vaginal births and to explore parity-specific associations. METHODS: This retrospective cohort study included all women who achieved or intended to achieve vaginal delivery of a l

Long-term outcomes of twins based on the intended mode of delivery.

OBJECTIVE: Recent studies have shown that for twin pregnancies with a cephalic presenting first twin, planned vaginal delivery is not associated with adverse short-term neonatal outcomes, as compared to planned cesarean delivery. Our objective was to compare long-term outcomes in twins, based on planned mode of delivery. STUDY DESIGN: This was a prospective, observational cohort of twin pregnancies delivered by a single MFM practice. All the patients with a twin pregnancy >34 weeks delivered from 2005-2014 were surveyed regarding pediatric outcomes at or after 2 years of life. The survey was

Risks and Associations of Third- and Fourth-Degree Lacerations: An Urban Single Center Experience.

OBJECTIVE: Obstetric anal sphincter injuries (OASIs) are severe tears involving the external and internal anal sphincters. We aimed to document the incidence of OASIs within a racially and ethnically diverse institution to elucidate which maternal, fetal, and parturition factors may be associated with OASIs in minority subgroups. METHODS: This was a retrospective unmatched case-control study of reproductive aged women who delivered between January 1, 2014, and December 31, 2017, at our institution. Data for maternal, fetal, and parturition factors were gathered through chart review. We also i

A two-stage OOD-aware approach for mosquito species detection based on RT-DETRv2.

Mosquitoes are important vectors of infectious diseases, and accurate species identification is essential for effective surveillance and control. Traditional image-based identification methods are labor-intensive, while existing deep learning models often exhibit limited generalization in real-world environments due to sensitivity to out-of-distribution (OOD) samples. To address these challenges, this study constructs a real-world mosquito dataset by integrating multiple publicly available sources, comprising 11,514 original images across 33 predefined species. To mitigate class imbalance, dat

Risk of recurrent spontaneous preterm birth following preterm full dilatation cesarean delivery.

INTRODUCTION: Term full dilatation cesarean delivery (FDCD) is associated with an increased risk of subsequent spontaneous preterm birth (sPTB). The impact of preterm FDCD on recurrent sPTB is unknown. We investigated the relationship between recurrent sPTB and the mode of prior sPTB. MATERIAL AND METHODS: This is a retrospective cohort study of singleton pregnant women attending two high-risk preterm birth surveillance clinics (University College London Hospital and St Thomas' Hospital London, UK), with one previous sPTB (24-36 + 6 weeks). Women were categorized according to their mode of bi

Role of Macrophages in Pregnancy and Related Complications.

Macrophages (MФs) are the leukocytes produced from differentiation of monocytes and are located in almost all tissues of human body. They are involved in various processes, such as phagocytosis, innate and adaptive immunity, proinflammatory (M1) and anti-inflammatory (M2) activity, depending on the tissue microenvironment. They play a crucial role in pregnancy, and their dysfunction or alteration of polarity is involved in pregnancy disorders, like preeclampsia, recurrent spontaneous abortion, infertility, intrauterine growth restriction, and preterm labor. About 50-60% of decidual leukocytes

Myometrial contractile strain at uteroplacental separation during parturition.

OBJECTIVE: A simplified geometric model of the uterine wall during the second and third stages of labor was created to estimate the magnitude of myometrial strain associated with the initiation of placental separation. STUDY DESIGN: The uterine wall was modeled as an isovolumetric, incompressible spherical shell whose overall radius decreased and mural thickness increased on uterine muscle contraction after delivery of the fetus. Either a 3.5-MHz or a 5-MHz ultrasonography probe was used to measure the change in uterine mural thickness of 14 healthy patients from just before delivery to the t

[Classifications and therapeutic indications in spontaneous intracerebral hematomas in the absence of any vascular malformation. Preliminary study apropos of 65 cases].

CT Scan and I.C.P. monitoring help us in management of spontaneous intracerebral haemorrhage to decide if surgical evacuation is necessary or not. The various opinions expressed in literature seem to depend on the lack of correlations between clinical state, CT Scan and I.C.P. monitoring. Three groups of patients were studied: Group I: patients with ingravescent coma during the first three days. Their natural history is death. Group 2: patients with disturbances of consciousness which appear, persist or increase during the first week. Group 3: awake patients with spontaneous improvement. In gr

Detection and recognition of the invasive species, Hylurgus ligniperda, in traps, based on a cascaded convolution neural network.

BACKGROUND: Hylurgus ligniperda, an invasive species originating from Eurasia, is now a major forestry quarantine pest worldwide. In recent years, it has caused significant damage in China. While traps have been effective in monitoring and controlling pests, manual inspections are labor-intensive and require expertise in insect classification. To address this, we applied a two-stage cascade convolutional neural network, YOLOX-MobileNetV2 (YOLOX-Mnet), for identifying H. ligniperda and other pests captured in traps. This method streamlines target and non-target insect detection from trap images
