The Effectiveness of Vestibular Rehabilitation Interventions in Treating Unilateral Peripheral Vestibular Disorders: A Systematic Review.

BACKGROUND AND PURPOSE: Various types of vestibular rehabilitation therapy are routinely used in clinical practice to treat unilateral peripheral vestibular hypofunction. The purpose of this systematic review was to compare the effectiveness of vestibular rehabilitation interventions (adaptation, substitution and habituation) in people with unilateral peripheral vestibular hypofunction, exclusionary of benign paroxysmal positional vertigo and Meniere's disease. METHODS: A search of the literature was conducted using PubMed, CINAHL and Scopus. Studies were eligible for inclusion if they were 1

Progress toward development of a multichannel vestibular prosthesis for treatment of bilateral vestibular deficiency.

This article reviews vestibular pathology and the requirements and progress made in the design and construction of a vestibular prosthesis. Bilateral loss of vestibular sensation is disabling. When vestibular hair cells are injured by ototoxic medications or other insults to the labyrinth, the resulting loss of sensory input disrupts vestibulo-ocular reflexes (VORs) and vestibulo-spinal reflexes that normally stabilize the eyes and body. Affected individuals suffer poor vision during head movement, postural instability, chronic disequilibrium, and cognitive distraction. Although most individua

Intratympanic steroids for inner ear disorders: a review.

BACKGROUND/AIM: The use of intratympanic steroids (ITS) has proliferated over the past 10-15 years to include treatments for inner ear disorders, like Ménière's Disease (MD) and sudden sensorineural hearing loss (SSNHL). The aim of this study was to review the clinical trials of ITS for inner ear disorders. METHODS: PubMed and Ovid Medline databases were searched from 1966 to present for clinical trials on ITS in the treatment of MD and SSNHL. Studies were evaluated based on comparability and internal and external validity. RESULTS: Thirty-eight studies were identified in total, 13 studies o

Pneumolabyrinth: an unusual finding in a temporal bone fracture.

Pneumolabyrinth or pneumocochlea are rarely found in temporal bone fractures. The presence of air in the inner ear is evidence of a pathological connection between the inner ear and the air-filled mastoid or middle ear cavities. A case of a pneumolabyrinth in a 2-years-old child is presented here. Diagnosis was made by means of a high resolution, thin sections computed tomography (CT) scan of the temporal bones, which is the imaging modality of choice in cases of otologic complaints after head trauma.

Endolymphatic hydrops precipitated by haemodialysis.

We report a case of endolymphatic hydrops preceded by haemodialysis in the contralateral ear of a patient with known Menière's disease, and discuss the possible mechanism. It is suggested that the sudden decrease of plasma osmolality during haemodialysis acts as a reverse glycerol test. The audiometric and electrophysiological recordings pre- and post-dialysis and the induction of the Menière's triad of symptoms during dialysis, were strongly indicative of the presence of endolymphatic hydrops.

Menière's symptoms resulting from bilateral otosclerotic occlusion of the endolymphatic duct: an analysis of a causal relationship between otosclerosis and Menière's disease.

An association between otosclerosis and Menière's disease has been proposed on both a clinical and temporal bone histopathologic basis for well over three-quarters of a century. Controversy persists over a causal relationship between these two entities, however, and the underlying pathophysiologic mechanisms relating capsular otosclerosis with Menière's disease remain speculative. The first case of total, bilateral endolymphatic duct occlusion resulting from extensive capsular otosclerosis is presented in a woman manifesting Menière's symptoms. The severe endolymphatic hydrops resulting from o

[Possibilities for quantifying vestibular vertigo].

The problem of the quantification of vertigo is still unsolved. In this article, two methods in the field of visual orientation in which the vestibular system is involved, are proposed. The visual subjective vertical allows to determine the degree of deviations, which is augmented in acute vestibular lesions. - The degree of deviation correlates well with the subjective intensity of vertigo. Both methods seem to be useful in the quantification of subjective vestibular complaints.

Radioanatomy of cochlear and stapedial otosclerosis.

Otospongiosis of the otic capsule is a primary focal disease, which can be reproduced by both conventional multidirectional tomography and computed tomography. The final stage of otospongiosis, otosclerosis has a complete different appearance in radiography. When the otospongiosis focus is located at the oval window a progressive conductive hearing loss will result, which in its final stage will have the character of otosclerosis with fixation of the stapes in the oval window. The radiographic diagnosis with either conventional multidirectional tomography or CT is more or less pathognomonic an

Transmastoid labyrinthectomy.

Labyrinthectomy is an effective technique in the surgical treatment of vertigo. We believe that the transmastoid approach is the most direct method of vestibular ablation. This technique provides direct visualization of vestibular neuroepithelium and allows for positive identification of the facial nerve. Vertigo was successfully controlled by this method in thirty-seven (97%) of thirty-eight patients.

[Complications of decompression of the internal ear during diving].

Inner ear decompression sickness belongs inner ear barotrauma. It occurs after a dive with factors of risk. The diagnostic needs an accurate history and physical examination which allows emergency treatment: therapeutic recompression, then hyperbaric oxygen and perfusions of vasodilators + corticoids. The symptomatology is mainly vestibular and the discussion between peripheric and/or central lesions is still open.

Histopathological study of the cochlea with altered perilymph metabolism.

In 41 guinea pigs an experiment was conducted to determine the effect of kanamycin on ears with or without blockage of the cochlear aqueduct. In 24 animals the cochlear aqueduct was blocked on one side, and after 1 to 21 days, kanamycin (300 mg/kg) was given daily and subcutaneously until the pinna reflex began to decrease. The remaining animals were used for controls: exposure of and trauma to the aperture of the cochlear aqueduct and kanamycin administration (8), blockage of the cochlear aqueduct only (3 plus 17 previous cases), and kanamycin administration to normal animals (6). The surviva

Vestibular signs and symptoms in Meniere's disorder: mechanical considerations.

Considerations of available evidence suggests that the vestibular signs and symptoms of typical Meniere attacks with return to normal function in the interval have mechanical causes as has long been advocated for their auditory counterparts. Membrane ruptures, the alternative explanation, are suggested to be the cause of the severe, and usually singular, attacks that are immediately followed by permanent auditory and vestibular impairments.
