Progressive Myopathy and Respiratory Failure in a 7-Year-Old Boy With m.3251A>G MT-TL1 Mutation.

We report a pediatric case of severe isolated mitochondrial myopathy because of the rare m.3251A>G variant of the MT-TL1 gene. A 7-year-old boy presented to the hospital with acute-on-chronic weakness and respiratory insufficiency. Initial laboratory tests were notable for elevated lactate, aldolase, and lactate dehydrogenase. Despite a negative autoimmune panel, he was presumed to have myositis and treated with steroids and intravenous immunoglobulin. He continued to deteriorate, eventually requiring intubation and ventilation. Muscle biopsy revealed numerous ragged red fibers, abnormal intra

Targeting Lactylation Offers Therapy to Reverse Cell Death Resistance.

Lactylation, a lactate-derived post-translational modification, emerges as a master metabolic regulator of resistance to regulated cell death (RCD) across pathologies including cancer, inflammatory disorders, and degenerative diseases. By dynamically modifying histones and non-histone proteins via lactyltransferases and delactylases, lactylation orchestrates convergent molecular pathways that suppress ferroptosis, cuproptosis, and apoptosis. This review synthesizes current understanding of lactylation as a central regulator of RCD resistance in diseases, especially in cancers. We dissect the m

The role of lactylation modifications in musculoskeletal diseases: From mechanism to clinical application.

Lactylation, a recently discovered histone and non-histone modification driven by lactate, has redefined the understanding of how metabolic by-products regulate gene expression. Beyond its conventional role as an energy substrate, lactate emerges as a pivotal signaling mediator that couples cellular metabolism with epigenetic regulation. In musculoskeletal disorders, aberrant lactylation has been implicated in the disruption of osteogenic-osteoclastic balance, extracellular matrix homeostasis, and regenerative capacity, thereby linking cellular metabolic stress to pathological tissue remodelin

Topics in Lung Disease: Noninfectious Pleural Effusions.

Noninfectious pleural effusions arise from an imbalance between fluid entering and leaving the pleural space. Effusions are classified as transudative or exudative, based on protein and lactate dehydrogenase levels. Common symptoms are dyspnea, cough, and chest pain. Physical examination findings may include dullness to percussion and decreased breath sounds. Transudates are associated with conditions such as congestive heart failure and cirrhosis, whereas exudates are linked to pneumonia, malignancy, pulmonary embolism, and gastrointestinal disorders. Diagnosis relies on chest imaging and ima

Mitochondrial DNA rearrangements with onset as chronic diarrhea with villous atrophy.

We report two unrelated children with onset of chronic diarrhea and villous atrophy in the first years of life. Elevated plasma lactate concentrations and lactate/pyruvate and ketone body molar ratios suggested a genetic defect of oxidative phosphorylation. Analysis of the mitochondrial respiratory chain showed a complex III deficiency in muscle of both patients. Southern blot analysis provided evidence of heteroplasmic mitochondrial DNA rearrangements that involve deletion and deletion-duplication. Directly repeated sequences (10 and 11 base pairs, respectively) were present in the wild type

Trazodone Levels in Maternal Serum, Cord Blood, Breast Milk, and Neonatal Serum.

Background: Trazodone is used to treat anxiety disorder, insomnia, and sleep disorders, which occur in ∼15% of pregnant and lactating women. However, pharmacokinetic information on the transfer of trazodone and its active metabolite, 1-m-chlorophenylpiperazine (mCPP), across the placenta or into breast milk is limited. In this study, we describe the pharmacokinetic profile of trazodone and mCPP concentrations in maternal and neonatal blood and breast milk. Case Presentation: A 44-year-old female received oral trazodone 50 mg once daily during pregnancy (28-38 gestational weeks) and lactation,

Targeting Glial Glycolysis with EGCG Alleviates Parkinson's Disease in a PINK1 mutant Drosophila Model.

ETHNOPHARMACOLOGICAL RELEVANCE: Camellia sinensis (L.) Kuntze (Theaceae), commonly known as green tea, has been traditionally utilized in Traditional Chinese Medicine (TCM) as a neurotonic to clear the mind, alleviate fatigue, and counteract aging-related cognitive decline. Epigallocatechin-3-gallate (EGCG), its principal bioactive polyphenol, exhibits well-documented neuroprotective properties, providing a modern pharmacological rationale for its traditional botanical application in managing neurological disorders such as Parkinson's disease (PD). AIM OF THE STUDY: While the neuroprotective

Both elevated lipopolysaccharide and D-lactic acid concentrations in blood contribute to systemic inflammation in acute ruminal acidosis.

Acute ruminal acidosis is a major metabolic disorder in agriculturally important ruminants, such as cattle and sheep. A common symptom of acute ruminal acidosis is systemic inflammation. This study aimed to identify the underlying causes of systemic inflammation during acute ruminal acidosis using both in vivo and in vitro experiments. In the in vivo experiments, twelve 9-month-old sheep were randomly assigned to two groups, six sheep per group. Sheep in the acidotic group were fasted for 24 h and then provided ad libitum access to a 1:1 mix of whole corn and wheat flour to induce acute rumina

Burkitt lymphoma and lactic acidosis: A case report and review of the literature.

Type A lactic acidosis is a potentially life-threatening complication in critically ill patients and is the hallmark of a shock state as a result of tissue hypoperfusion and dysoxia. Type B lactic acidosis results from mechanisms other than dysoxia and is a rare condition in patients with solid tumors or hematological malignancies. We present a case of a 60-year-old man with lactic acidosis who was found to have a Burkitt lymphoma related to a posttransplant lymphoproliferative disorder. Lactagenic cancers are characterized by increased aerobic glycolysis and excessive lactate formation, a phe

"Pregnancy and Lactation Associated Osteoporosis".

Pregnancy-associated osteoporosis (PAO) is a rare condition of skeletal fragility affecting women in pregnancy or the postpartum period. During normal pregnancy and lactation, substantial changes in calcium metabolism and skeletal physiology occur in order to meet the demands of the developing foetus. Whilst these adaptations are reversible and generally of no clinical consequence for the mother, a small number of women will develop osteoporosis and suffer fragility fractures. Vertebral fractures occur most commonly in PAO and are often multiple. Due to the rarity of PAO, systematic study to d

Vocal cord paralysis and hypoventilation in a patient with suspected Leigh disease.

The authors report the case of a 16-month-old male with suspected Leigh disease, which was diagnosed on the basis of the clinical manifestations, abnormal lactate stimulation test, proton magnetic resonance spectroscopy, and neuroradiologic findings. Progressive stridor resulting from bilateral vocal cord paralysis and hypoventilation was evident. The authors suggest that for infants or children who exhibit vocal cord paralysis, mitochondrial disorders, such as Leigh disease, should be considered.

Schizophrenia and Leigh syndrome, a simple comorbidity or the same etiopathogeny: about a case.

Leigh syndrome is a mitochondrial encephalomyopathy that occurs due to "cytochrome c oxidase deficiency". Few psychiatric disorders have been defined that are associated with Leigh syndrome. The objective of this work is to study relations between mitochondrial dysfunction and psychiatric disorders. It was a 20 year old male patient, who received Modopar, for severe extra pyramidal symptoms caused by Leigh syndrome. He developed, four months ago, acute psychotic symptoms such as audio-visual hallucinations, persecution and mystic delirium. The cerebral MRI has shown signal abnormalities in cen

Evaluating the impact of sprinkler cycle and flow rate on dairy buffalo performance during heat stress.

The aim of this study was to evaluate the effect of the sprinkler cycle and flow rate on physiological, behavioural, and productive responses in dairy buffaloes. Nine Nili Ravi lactating buffaloes were subjected to three sprinkler cycles and two flow rates using a double replicated 3 × 3 Latin square design. The flow rates were 1.25 and 2 l/min, and the sprinkler cycles (minutes water on/off, number of cycles/h) were: 3/3, 10 cycles; 3/6, 7 cycles and 3/9, 5 cycles. The showering was applied from 0800 till 1630 h daily. In the first square of 21 d, each of the three sprinkler cycles was applie

Behavioral patterns as indicators of resilience after parturition in dairy cows.

During the transition phase, dairy cows are susceptible to develop postpartum diseases. Cows that stay healthy or recover rapidly can be considered to be more resilient in comparison to those that develop postpartum diseases. An indication of loss of resilience will allow for early intervention with preventive and supportive measures before the onset of disease. We investigated which quantitative behavioral characteristics during the dry period could be used as indicators of reduced resilience after calving, using noninvasive Smart Tag neck and Smart Tag leg sensors in dairy cows (Nedap N.V.).

Nutritional strategies for alleviating the detrimental effects of heat stress in dairy cows: a review.

Heat stress responses negatively impact production performance, milk quality, body temperature, and other parameters in dairy cows. As global warming continues unabated, heat stress in dairy cows is likely to become more widespread in the future. To address this challenge, researchers have evaluated a number of potentially available nutritional strategies, including dietary fat, dietary fiber, dietary microbial additives, minerals, vitamins, metal ion buffer, plant extracts, and other anti-stress additives. In this paper, we discuss the evidence for the efficacy of these nutritional strategies

Lactational phlegmon: A distinct clinical entity affecting breastfeeding women within the mastitis-abscess spectrum.

We aimed to describe the presentation and treatment of lactational phlegmon, a unique complication of mastitis in breastfeeding women that may require surgical management. We retrospectively analyzed medical charts of breastfeeding women treated by a single breast surgeon for lactational phlegmon or the related conditions of abscess or uncomplicated mastitis (UM) from July 2016 to October 2018. Demographic variables and treatment details were analyzed using ANOVA and Pearson's Chi-square test. Ten women with lactational phlegmon (19.2%), 15 women with abscess (28.8%), and 27 women with UM (51.

Modeling heat stress under different environmental conditions.

Renewed interest in heat stress effects on livestock productivity derives from climate change, which is expected to increase temperatures and the frequency of extreme weather events. This study aimed at evaluating the effect of temperature and humidity on milk production in highly selected dairy cattle populations across 3 European regions differing in climate and production systems to detect differences and similarities that can be used to optimize heat stress (HS) effect modeling. Milk, fat, and protein test day data from official milk recording for 1999 to 2010 in 4 Holstein populations loc

Vulvar lactating adenoma associated to a fibroadenoma: common neoplasms in an uncommon site.

Ectopic breast tissue is defined as glands located outside of the breast. Ectopic breast tissue should be excised because it may develop benign (fibroadenoma) or malignant pathologic processes. Less than forty cases of fibroadenomas have been reported in the literature. Although lactation changes can occur, lactating adenoma in the vulva are extremely rare. Only four cases have been reported. We report a case of a young woman who presented with vulvar mass during her lactation. The mass was excised, and histology confirmed vulvar lactating adenoma associated with fibroadenoma. This is the firs

Lactogenesis failure following successful delivery of advanced abdominal pregnancy.

Abdominal pregnancy is a rare condition with significant risk of maternal and fetal morbidity and mortality. Because of the vascular involvement, the placenta is often left in situ. Prior reports have neglected to discuss the implications of leaving the placenta in situ on breastfeeding and lactation. We present a case of failure of lactogenesis II following the successful delivery of an advanced abdominal pregnancy in which the placenta was left in situ. This is the first report to discuss the effects on lactation of leaving the placenta in situ as a conservative management option in advanced
