Management of childhood lead poisoning: clinical impact and cost-effectiveness.

OBJECTIVES: No consensus exists regarding the preferred treatment of childhood lead poisoning. The authors used decision analysis to compare the clinical impacts and cost-effectiveness of four management strategies for childhood lead poisoning, and to investigate how effective chelation therapy must be in reducing neuropsychologic sequelae to warrant its use. METHODS: The model was based on a 2-year-old child with moderate lead poisoning [blood lead level 1.21 to 1.88 mumol/L (25 to 39 micrograms/dL)]. The following strategies were compared: 1) no treatment; 2) EDTA provocation testing, follo…

PBK modeling for metals. Examples with lead, uranium, and chromium.

Physiologically-based models for metals differ in several key respects from models for organic compounds. Although sequestration by binding … les are given of applications of physiologically-based kinetic models for the bone-seeking elements lead, chromium, and uranium to assist in answering practical questions relating to bioavailability, distribution, and data interpretation.

Transvenous implantable cardiovascular defibrillator lead system: experience in 28 patients.

Morbidity and mortality following Automatic Implantable Cardioverter Defibrillation (AICD) is mostly related to thoracotomy performed during placement of epicardial leads. From July 1991 to January 1994, a transvenous lead system (TLS) (Endotak C; CPI) was implanted in 28 patients (24 males, 4 females, age 15-76 years) who suffered from life threatening ventricular tachyarrhythmias. Twenty four patients had a previous MI, mean LVEF was 26% (14-65%), two had mitral valve prolapse (one with prolonged QT), one had hypertrophic and another had dilated cardiomyopathy. The implantation procedure was…

Modified fabrication techniques lead to improved centrifugal blood pump performance.

The authors are developing an implantable centrifugal blood pump for short- and medium-term (1-6 months) left ventricular assist. They hypothesized that the application of result dependent modifications to this pump would lead to overall improved performance in long-term implantation studies. Essential requirements for pump operation, such as durability and resistance to clot formation, have been achieved through specialized fabrication techniques. The antithrombogenic character of the pump has been improved through coating at the cannula-housing interfaces and the baffle seal, and through cha…

X-linked bulbospinomuscular atrophy (Kennedy's disease) masquerading as lead neuropathy.

A 43-year-old male was referred by a veterinarian who evaluated his dog for a seizure and suspected a toxic lead exposure for both. He refurbished houses, removing old paint, and complained of decreased cognition, fatigue, and muscle cramps. He had a depressed affect, postural tremor, right arm weakness with partial denervation on EMG, and borderline-low sensory nerve action potential (SNAP) amplitudes. A mild anemia and elevated serum and urine lead levels supported a diagnosis of lead neuropathy. Chelation therapy increased urine lead excretion without symptomatic improvement. His brother wo…

A unique pulse generator safety feature for bipolar lead fracture.

In this report, we describe two pacemaker dependent patients with outer-coil fractures in their bipolar ventricular leads who averted potentially catastrophic consequences because of a new safety feature incorporated into their pulse generators (Intermedics Cosmos II, model 284-05). Both patients were asymptomatic; the malfunctions were discovered at routine pacemaker evaluation. Magnet application resulted in symptomatic loss of ventricular capture. In each instance, the pulse generator detected an outer-coil fracture and reprogrammed itself from bipolar DDD to unipolar DOO pacing to prevent …

The effect of blood lead on blood pressure in children.

The effect of elevated blood lead levels on the blood pressure of children has not been clearly described. In order to define this association better, we conducted a cross-sectional study, evaluating the association between lead and high blood pressure. Using a Dinamap monitor to measure blood pressures, blood pressures and blood lead levels were measured in 149 children (ages 1-10 years) receiving medical care at the General Medical and Lead Poisoning Clinics at Children's Hospital of Philadelphia. Blood lead levels ranged from 7 to 70 mg/dL with a mean of 27 mg/dL. The mean systolic blood pr…

A survey of lead poisoning in wild waterfowl in Japan.

Ingested shotgun pellets were found in 13 of 56 hunter-killed wild waterfowl between October and December 1991 from two hunting grounds in Japan. Four of 33 other waterfowl found dead in lightly hunted areas between January 1991 and March 1992 were diagnosed as having lead poisoning. We propose that lead poisoning maybe a threat to waterfowl in Japan.

Evaluation of an in-line bipolar polyurethane ventricular pacing lead.

A study was undertaken to evaluate the performance of the Medtronic 4012 polyurethane in-line bipolar pacing electrode. One hundred twenty six patients implanted with the 4012 lead were followed by our clinic with 116 of these leads implanted at our institution. Patients were followed a minimum of every 4 months. Testing included the performance of stimulation thresholds, sensing thresholds, and provocative maneuvers to evaluate for myopotential inhibition testing a minimum of every eight months. There were 12 documented lead failures of the total 126 patients. Further analysis was limited to …

[Acute lead poisoning in calves: clinical, pathological and toxicological findings].

We present a case of acute lead poisoning in 10 calves. All calves died with few or no clinical signs prior to death. The clinical signs included neurologic and gastrointestinal symptoms but were of an unspecific nature. Several painted iron girders, stored on a field close to the farm, were determined as the source of the poisoning. Postmortem findings were minimal. Some animals presented acid-fast, intranuclear inclusion bodies in the renal tubules. A chemical analysis of some frozen parts of the liver and kidney revealed levels of lead as high as 12 ppm in the liver and 63 ppm in the kidney…

Lead toxicity: from overt to subclinical to subtle health effects.

Although the toxicity of lead was recognized centuries ago, concern was restricted to overt symptoms: colic, encephalopathy, anemia, or renal disease. Two major reasons for lack of progress in restricting toxicity were that interest was limited to occupational exposures and there was lack of awareness of specific biochemical or metabolic effects. Identification of subclinical effects has been possible the last 15 or 20 years because of the development of sensitive measures to detect cognitive and behavioral changes that are not apparent clinically and because of methods to measure the reduced …

Chronic lung collapse: a predictable impediment to ipsilateral lead placement.

Implantation of a permanent transvenous pacemaker was undertaken in a patient with longstanding, complete collapse of the right lung. Unanticipated distortion of the lead path prevented entirely introduction of a right subclavian sheath and rendered lead placement from a right cephalic venotomy extremely difficult. An angioplasty guidewire substituted for the standard stylets proved useful in positioning the lead. This report describes the predictable venous deformity, illustrates the consequent impediment to ipsilateral lead placement, and discusses why contralateral entry sites should be sup…

Elevated lead levels in children with nonorganic failure to thrive.

Every child with failure to thrive has at least one organic medical disease: malnutrition. It is well documented that lead and other heavy metals are absorbed more readily in the presence of both malnutrition and iron deficiency anemia. Malnutrition and lead exposure tend to be found in the same population groups. Furthermore, lead poisoning is correlated with many of the identical intellectual and behavioral deficits demonstrated in children suffering from nonorganic failure to thrive. Because of these facts, whole blood lead levels were determined for 45 children with nonorganic failure to t…

Lead in milk and infant blood: a dose-response model.

As part of a longitudinal study of the sources and developmental effects of current urban lead exposure, lead was measured in tap water from the homes of 249 infants, in 100 breast milk samples, and in 73 samples of the infant formula used by non-nursing mothers. Also, the blood lead levels of the infants who received these fluids were determined at birth and at 6 months of age. Among the infants who were breast fed, the lead content of their milks correlated very well with their 6-month blood lead levels (r = .42, P = .0003). The mean lead content of infant formulas and breast milk were not s…

Maturation of resistance to lead encephalopathy: cellular and subcellular mechanisms.

The rat pup fed inorganic lead has been studied extensively as an animal model of human lead encephalopathy. As in man, the sensitivity of the brain to lead toxicity is age-dependent. Pups given daily lead feedings for one week beginning in the first week of life show pathologic changes (i.e., hemorrhage, edema, and neuronal necrosis) throughout the brain including the cerebral cortex and cerebellum. Pups begun on daily lead feedings for two weeks between 10-18 days of age show similar pathologic changes almost entirely confined to the cerebellum. Pups receiving very large quantities of lead f…

The relationship between prenatal exposure to lead and congenital anomalies.

We obtained umbilical cord blood from 5,183 consecutive deliveries of at least 20 weeks' gestation and analyzed them for lead concentration. Those demographic and socioeconomic variables, including lead, which were shown on univariate analysis to be associated with increased risk for congenital anomalies were evaluated and controlled by entering them into a stepwise logistic-regression model with malformation as the outcome. Coffee, alcohol, tobacco, and marijuana use, which were associated with lead level, but not risk of malformation, were also controlled. The model was reduced in steps by e…

Occult lead intoxication in patients with gout and kidney disease.

Ten patients with gout, hypertension, and mild to moderate renal insufficiency were studied for possible lead nephropathy by measuring stimulated urinary lead excretion. Seven had a history of lead exposure, 5 from illegal alcohol and 2 from industrial sources. Occult lead was assessed by 24 h urine collection measurements over a 72 h period after intramuscular administration of calcium disodium EDTA. Two patients with a history of lead exposure excreted 707 and 687 micrograms Pb/72 h, respectively, and a 3rd excreted 506 micrograms Pb/72 h. The remainder had a normal response, with mean urina…

Lead poisoning and hookworm infection as multiple factors in anaemia.

A retrospective study was carried out in a rural area of north-eastern Brazil, to search for interactions between biological and chemical agents present in the environment as causes of anaemia. A lead smelter situated in a hookworm endemic area provided the opportunity to look at the effect of this double exposure in relation to laboratory defined anaemia, by comparing the odds ratios in different subgroups of 109 "cases" and 107 "controls". The subgroup with both hookworm infection and chronic lead poisoning--measured by the concentration of delta-ALA excreted in urine--had an odds ratio of d…
