Management considerations of ameloblastic carcinoma in soft-tissue: case report and literature review.

BACKGROUND: Ameloblastoma is a benign yet locally aggressive odontogenic neoplasm characterized by a significant propensity for local recurrence. Soft-tissue recurrence following mandibular resection and reconstruction represents a distinct clinical entity, often posing diagnostic and therapeutic challenges. In contrast, ameloblastic carcinoma may arise de novo or through malignant transformation of a pre-existing ameloblastoma, reflecting a spectrum of biological behaviour. We report a case of post-resection soft-tissue recurrence with subsequent malignant transformation, underscoring the unp

Effective oral function improvement by restoration-driven implant treatment after mandibular resection with a scapular flap: a case report.

BACKGROUND: The extensive loss of teeth and surrounding tissues due to mandibulectomy for an oral tumor not only impacts negatively on appearance, but also often causes various functional disorders, decreasing quality of life (QOL). In the present case, reconstruction with a scapular flap was carried out along with segmental mandibulectomy, aiming for functional restoration through restoration-driven implant treatment. A good outcome was obtained, with improvement of masticatory function and QOL following the prosthetic treatment. CASE PRESENTATION: The patient was a 37-year-old woman diagnos

Is the mandible intrinsically different in Apert and Crouzon syndromes?

Relative mandibular prognathism is an observed finding in Apert and Crouzon syndromes. This imbalance in the facial profile is generally attributed to the diminished growth of the maxilla, thereby increasing the disparity between maxilla and mandible with increasing age. What is not known is whether the mandible is, indeed, normal. Previous work by Kreiborg with Crouzon syndrome led to the conclusion that the mandible, although somewhat smaller, grew in a "normal" pattern. Our own observations, which used a greater variety of mandibular measurements on patients with both Apert and Crouzon synd

Treacher Collins syndrome (mandibulofacial dysostosis).

Treacher Collins syndrome is a complex congenital malformation. The most important manifestations of this syndrome are hypoplastic or absent malar bone, centrofacial over-projection, eyelid coloboma, and micrognathia. This article presents an approach to re-establish the right centrofacial proportions through "flattening osteotomies" of the maxilla, malar bone reconstruction, and other maxillo-mandibular osteotomies.

Correction of Traumatic Transverse Mandibular Widening With Interdental Wiring.

Mandibular symphysis fractures pose several technical challenges for the craniomaxillofacial surgeon. One classic challenge is restoration of the transverse dimension when the mandible is widened secondary to splaying of the lingual cortex. Failure to diagnose or correct this problem can result in widening of the lower facial third, inadequate fracture reduction, and/or a malocclusion. Surgeons have traditionally utilized techniques such as manual pressure to the rami or lingual splint application to address transverse defects intraoperatively. However, these methods may be inadequate in situa

Association between condylar morphology and changes in bony microstructure and sub-synovial inflammation in experimental temporomandibular joint arthritis.

BACKGROUND: In juvenile idiopathic arthritis involvement of the temporomandibular joints (TMJs) is often associated with mandibular growth deviations. The relation between the growth deviations and severity of the inflammation, condylar shape, the micro-architecture, and the quality of the bone has not previously been investigated. This paper studies the effect on the bony structures in mandibular condylar development in rabbits with antigen-induced arthritis. METHODS: Included were 42 juvenile rabbits with ovalbumin-induced arthritis of the TMJs treated with intraarticular saline, intraartic

Digital technique for anatomic design and bone-implant interface analysis of a custom bar for a mandibular implant-supported fixed dental prosthesis.

With the evolution of computer-aided design and computer-aided manufacturing (CAD-CAM), the digitally guided fabrication of implant-supported fixed dental prostheses has become increasingly precise and customized. Conventional substructure designs may fail to ensure a passive fit or adequate biomechanical performance in edentulous patients with severe mandibular bone resorption. Anatomic limitations and load concentration at the bone-implant interface present significant challenges for long-term prosthetic success. This article describes a step-by-step digital technique for designing a custom

Office-Based Digital Workflow for Managing Old Mandibular Fracture Malunion: Use of Short-Segment Drilling and Osteotomy Guides With Predictive Holes.

Treating old mandibular fractures with malunion is challenging due to the need for osteotomy and interference between bone segments, complicating occlusal restoration. This technical note introduces a novel office-based digital workflow utilizing surgical guides. Preoperative computed tomography (CT) data were imported into MIMICS software for virtual surgical planning to precisely realign bone fragments based on anatomical and occlusal relationships. A pre-bent reconstruction plate was adapted to a 3-dimensional-printed mandible model, and cone beam CT scans captured detailed screw path data.

Mandible preservation in the treatment of cancer of the floor of the mouth.

A segmental anterior mandibulectomy causes severe functional disability and cosmetic problems. Many methods of reconstruction have been used but none of these procedures provides the ideal solution. There has been increasing interest in the possibility of preserving a portion of the anterior mandibular arch and still carrying out an adequate surgical excision to control the primary tumor. Over a 15-year period, 251 patients with cancer of the floor of mouth were seen and treated at Roswell Park Memorial Institute. Of this group, 34 patients underwent a marginal mandibular resection as part of

Biocompatibility Evaluation of an Artificial Metallic Bone with Lattice Structure for Reconstruction of Bone Defect.

Mandibular reconstruction for large bone defects is performed with consideration of patients' specific morphology and sufficient strength. Metal additive manufacturing techniques have been used to develop biomaterials for mandibular reconstruction. Titanium artificial mandibles with a lattice structure have been proposed, and the optimal conditions for their strength to withstand mechanical stress around the mandible have been reported. This study investigated the biocompatibility of a titanium artificial bone with a lattice structure fabricated under optimal conditions. The samples were fabri…

[Giant ameloblastoma of the mandible].

INTRODUCTION: Giant ameloblastomas are more common in developing countries. They raise a serious problem of management. We present the case of one of the largest ameloblastoma ever reported. CASE REPORT: A 48-year-old Congolese female patient was referred for mandibular swelling having begun 23 years before and now inducing severe functional disorders. A cephalic CT scan revealed a multicystic mass, 30×18×10cm in size, with a typical "soap bubble" presentation, and with thinned and inflated cortical bone. The treatment was sub-total segmental mandibulectomy with immediate reconstruction using

Experiences in the use of eight cutaneous and osteocutaneous superficial and deep circumflex iliac free flaps.

Eight patients underwent free tissue transfers involving groin cutaneous or osteocutaneous flaps using the superficial and deep circumflex iliac arteries and veins. Four were used for head and neck reconstruction, two for severe neck scar contractures, and two for composite skin and mandibular reconstruction. Three were used in lower extremity reconstructions, all in situations requiring bone and skin reconstruction. One was used for skin cover alone in an upper extremity reconstruction for wrist cover. One of the head and neck cases sustained vascular thrombosis requiring a second successful …

Total joint replacement with the stock Biomet system in adult hemifacial microsomia without glenoid fossa: a case report and literature review.

BACKGROUND: In patients with hemifacial microsomia accompanied by mandibular fossa deficiency and severe atrophy of the mandibular ramus, customized total joint replacement (TJR) is commonly used to restore masticatory and joint function and improve facial asymmetry. However, in countries where customized TJR is not approved, or for patients for whom the cost is prohibitive, a stock TJR system must be considered. In cases with a severely medially inclined frontal ramal angle and a lack of supporting bone for the fossa component, using a stock TJR poses significant technical challenges. CASE P
