Challenges in the future of cancer screening.

The purpose of cancer screening is to reduce mortality, and ideally incidence, from the cancer screened for. Until recently, cancer screening has been offered to all persons in pre-defined sex- and age-groups. The exception is lung screening which is targeted to high-risk individuals. Evidence for effect of screening on cancer-specific mortality is available for cancer of the cervix, breast, colorectal, lung, and prostate, and on cancer-specific incidence for cervix and colorectal cancer. With more effective molecular and computational tools, the current paradigm of cancer screening will chang

Accuracy of Falls Screening Tools in Adult Patients With Cancer: A Systematic Review.

IMPORTANCE: Patients with cancer experience increased falls risk secondary to oncological treatment and cancer-related sequelae. OBJECTIVE: Identifying diagnostically and prognostically accurate screening tool(s) for falls risk in populations with cancer is an important issue. DATA SOURCES: Screening tests were identified in PubMed and CINAHL. STUDY SELECTION: Two independent reviewers screened citations for inclusion. DATA EXTRACTION AND SYNTHESIS: Data extraction was performed by 1 reviewer and verified by a second. Tests were investigated for clinical utility, validity, diagnostic accur

Overdiagnosis in screening for gestational diabetes: A scoping review.

BACKGROUND: Screening for gestational diabetes mellitus (GDM) in healthy pregnancies is disputed, and rising GDM incidence calls for an investigation into the potential for overdiagnosis. AIM: To identify and map empirical evidence on overdiagnosis in GDM screening and explore how different screening strategies impact overdiagnosis. METHODS: This review adheres to the JBI Manual and PRISMA-ScR guidelines. Systematic searches were performed on September 10th, 2024, without date, language, or geography restrictions, in Medline, Embase, CINAHL, Cochrane Library, and Web of Science. One search t

Screening of obstructive sleep apnea in patients with hypertension: review of the literature and results of an Italian survey.

Obstructive sleep apnea (OSA) is a common disorder in the general population and individuals with hypertension. We reviewed the literature on the prevalence of OSA in hypertension and hypertension subgroups. The current literature shows a high prevalence of OSA in patients with nocturnal and resistant hypertension, up to more than 90% in patients with refractory hypertension. The prevalence of OSA in patients with primary aldosteronism is greater than 45%. We also conducted an Italian national survey to assess the diagnostic approach to OSA in centers associated with European and Italian Socie

Evaluation of the performance of screening tools for dysphagia in older adults: A diagnostic meta-analysis.

This systematic review evaluated the diagnostic accuracy of multiple screening tools for dysphagia in older adults. The analysis covered 30 studies involving 13 tools and 2,697 participants. The results showed that the volume viscosity swallow test performed best in sensitivity, specificity, and area under the curve, making it a recommended screening tool. Other tools have low sensitivity, unstable results or insufficient research and need to be further validated. Overall, this study provides an important reference for the screening of dysphagia in older adults.

Strategies for implementing HIV self-testing in diverse populations: an integrative review.

OBJECTIVES: to identify strategies for implementing HIV self-testing in populations. METHODS: an integrative review of MEDLINE/PubMed, Scopus, CINAHL, Web of Science, and LILACS databases. Full articles available online, in Portuguese, English, Spanish, or French, regardless of the year of publication, were included. Letters to the editor, review articles, and duplicates, rapid tests performed by professionals, and home self-tests sent to a laboratory without on-site results, were excluded. The sample of 24 articles had their level of evidence assessed using descriptive data analysis. RESULT

Cross-domain generative translation of cough audio biomarkers for tuberculosis screening across diverse global populations.

Accurate and equitable tuberculosis (TB) screening using cough audio biomarkers is challenged by domain shifts arising from heterogeneous recording environments, devices, and subject demographics. This study addresses these challenges by leveraging a cross-domain generative adversarial framework, specifically CycleGAN, to translate cough audio features across diverse source domains within the TB Audio Kaggle dataset. Key audio features, including MFCCs and spectrograms, were extracted and normalized via generative translation to mitigate non-pathological domain variability. Subsequently, super

For Tuberculosis, Not "To Screen or Not to Screen?" but "Who?" and "How?".

Active case finding leveraging new molecular diagnostics and chest X-rays with automated interpretation algorithms is increasingly being developed for high-risk populations to drive down tuberculosis incidence. We consider why such an approach did not deliver a decline in tuberculosis prevalence in Brazilian prison populations and what to consider next.

Mass Testing and Treatment to Accelerate Malaria Elimination: A Systematic Review and Meta-Analysis.

In regions where malaria transmission persists, the implementation of approaches aimed at eliminating parasites from the population can effectively decrease both burden of disease and transmission of infection. Thus, mass strategies that target symptomatic and asymptomatic infections at the same time may help countries to reduce transmission. This systematic review assessed the potential benefits and harms of mass testing and treatment (MTaT) to reduce malaria transmission. Searches were conducted in March 2021 and updated in April 2022 and included cluster-randomized controlled trials (cRCTs)

Sexually Transmitted Infections in People with Human Immunodeficiency Virus.

Sexually transmitted infections (STIs) are more commonly seen in patients with human immunodeficiency virus (PWH). Routine sexual history taking and appropriate multisite screening practices support prompt identification and treatment of patients, which in turn reduces morbidity and spread of STIs including HIV. Nucleic acid amplification testing has high accuracy for diagnosing many of the major STIs. Diagnosis of syphilis remains complex, requiring 2 stage serologic testing, along with provider awareness of the myriad symptoms that can be attributable to this disease. Prevention through mech

Tuberculosis screening practices and outcomes in an australian dialysis unit.

BACKGROUND: The World Health Organisation (WHO) recommends all dialysis patients undertake routine screening for latent tuberculosis infection (LTBI) in high income countries such as Australia. However, we employ a targeted screening approach in our Australian dialysis unit in line with local and some international guidelines. We analysed our practices to assess the validity of our approach. METHODS: A retrospective review of new dialysis patients during the period 2012-2018 was undertaken. Patient records were reviewed for basic demographic data, comorbidities, LTBI screening using Quantifer

SIED-GISCOR recommendations for colonoscopy in screening programs: Part I - Diagnostic.

The implementation of FIT programs reduces incidence and mortality from CRC in the screened subjects. The ultimate efficacy for CRC morbidity and mortality prevention in a FIT program depends on the colonoscopy in FIT+ subjects that has the task of detecting and removing these advanced lesions. Recently, there has been growing evidence on factors that influence the quality of colonoscopy specifically withing organized FIT programs, prompting to dedicated interventions in order to maximize the benefit/harm ratio of post-FIT colonoscopy. This document focuses on the diagnostic phase of colonosco

Non-invasive mid-term electrocardiogram patch monitoring is effective in detecting atrial fibrillation.

BACKGROUND: Atrial fibrillation (AF) is a cause of serious morbidity such as stroke. Early detection and treatment of AF is important. Current guidelines recommend screening via opportunistic pulse taking or 12‑lead electrocardiogram. Mid-term ECG patch monitors increases the sensitivity of AF detection. METHODS: The Singapore Atrial Fibrillation Study is a prospective multi-centre study aiming to study the incidence of AF in patients with no prior AF and a CHA2DS2-VASc score of at least 1, with the use of a mid-term continuous ECG monitoring device (Spyder ECG). Consecutive patients from bot
