Evaluation of Shoulder Strength and Kinematics as Risk Factors for Shoulder Injury in United States Special Forces Personnel.

BACKGROUND: Musculoskeletal injuries at the shoulder are highly prevalent and place a large burden on United States Special Forces personnel. Literature is lacking regarding the risk factors for these types of injuries. PURPOSE/HYPOTHESIS: The purpose of this study was to evaluate the association of shoulder strength and kinematic characteristics, which have shown retrospective associations with shoulder conditions/injuries, with prospectively collected shoulder injuries. We hypothesized that lower strength and abnormal kinematics would be predictive of future shoulder injury. STUDY DESIGN:

Molecular imaging: integration of molecular imaging into the musculoskeletal imaging practice.

Chronic musculoskeletal diseases such as arthritis, malignancy, and chronic injury and/or inflammation, all of which may produce chronic musculoskeletal pain, often pose challenges for current clinical imaging methods. The ability to distinguish an acute flare from chronic changes in rheumatoid arthritis, to survey early articular cartilage breakdown, to distinguish sarcomatous recurrence from posttherapeutic inflammation, and to directly identify generators of chronic pain are a few examples of current diagnostic limitations. There is hope that a growing field known as molecular imaging will

Pediatric acute leukemia mimicking osteomyelitis: clinical and MRI features.

BACKGROUND: Pediatric acute leukemia (AL) typically presents with symptoms of cytopenias, organomegaly, and fever. In some instances, however, musculoskeletal pain may be the predominant-occasionally the sole-initial symptom, leading to misdiagnoses such as osteomyelitis. METHODS: We conducted a retrospective, multicenter, observational, cohort study including children and adolescents (<18 years) diagnosed with acute lymphoblastic leukemia (ALL) or acute myeloid leukemia (AML) between 2017 and 2024 who initially presented with osteomyelitis-like clinical and/or radiological features. Clinical

Standardization of body weight distribution using a Force-Sensitive Resistor (FSR) matrix and foot analytics.

Balanced body weight distribution across both feet is essential for maintaining musculoskeletal health and minimizing postural and ambulatory issues. Conventionally foot weight distribution is measured using specialized clinical equipment which are expensive and limits routine screening. Therefore, in this work, we present a standardized foot weight distribution model having a custom 32×32 force-sensing resistor (FSR) matrix based embedded system. A total of 4,000 volunteers participated in the Physical Fitness Assessment (PFA). Of these, 2,200 who passed the musculoskeletal fitness criteria w

Pain associated with the musculoskeletal system in children from Warsaw schools.

OBJECTIVE: To assess the prevalence of pain in the musculoskeletal system and possible reasons for these complaints among early age children from Warsaw schools. MATERIAL AND METHODS: The study was conducted in 34 randomly selected primary schools in Warsaw in 2011. 2748 survey-questionnaires were given to parents or legal guardians by children. Of these, 1509 surveys were subject to a final analysis. The survey included 66 questions regarding, among other things, pain in the musculoskeletal system in children. Additionally, there were questions about possibly occurring diseases, any postural

Ectopic pseudojoints in fibrodysplasia ossificans progressiva.

Pseudoarthroses (false joints) occur as a complication of fracture healing in the normotopic skeleton but have not been reported at ectopic sites. Fibrodysplasia ossificans progressiva (FOP) is an ultra-rare genetic disorder characterized by progressive heterotopic ossification (HO) and congenital skeletal abnormalities including developmental arthropathy in the normotopic skeleton. Here, we report ectopic pseudojoint formation in 6 patients with FOP, all occurring after painful flare-ups and HO in soft connective tissues of the knee, hip, and forearm. To our knowledge, FOP is the only human c

Ganglioneuromatous polyposis associated with type 2 B multiple endocrine neoplasia (MEN 2B) - case report.

Multiple endocrine neoplasia type 2B (MEN 2B) is a rare autosomal dominant hereditary cancer syndrome which is characterized by the appearance of medullary thyroid carcinoma (MTC), pheochromocytoma, parathyroid adenomas, ganglioneuromas of the digestive tract, and musculoskeletal abnormalities. The case is presented of a 31-year-old male patient with numerous polyps in the colon described as ganglioneuromas which are ectodermal neoplasms emerging from a proliferation of ganglionic cells of the sympathetic nervous system. The results show elevated levels of normetanephrine, which is an endogeno…

Malignant hyperthermia associated with exercise-induced rhabdomyolysis or congenital abnormalities and a novel RYR1 mutation in New Zealand and Australian pedigrees.

Malignant hyperthermia (MH) is rarely associated with specific myopathies or musculoskeletal abnormalities. Three clinical investigations of MH associated with either non-specific myopathies or congenital disorders in three separate families are presented. Two of these cases also show evidence of exercise-induced rhabdomyolysis. In each case MH susceptibility was confirmed by in vitro contracture testing of quadriceps muscle. DNA sequence analysis of each kindred revealed the presence of a common novel mutation that results in an arginine401-cysteine substitution in the skeletal muscle ryanodi…

Changes of elastic fibers in musculoskeletal tissues of Marfan syndrome: a possible mechanism of joint laxity and skeletal overgrowth.

The aim of the study was to analyze by histochemical, ultrastructural, and morphometric methods the musculoskeletal tissues in three humans affected with Marfan syndrome. Histochemical and morphometric data demonstrated that the content of elastic fibers in the perichondrium, periosteum, and knee capsule of the individuals with Marfan syndrome was dramatically reduced in comparison with control tissues. Ultrastructurally the elastic fibers appeared fragmented and indented, because of the presence of discontinuous aggregates of elastin among randomly dispersed filaments. These abnormalities of

Management of malignant hyperthermia.

A case of malignant hyperthermia in a young girl with vesicoureteral reflux and multiple congenital abnormalities is discussed. Malignant hyperthermia is a relatively newly defined problem that is associated with anesthesia. It is being recognized with increasing frequency, particularly in children with multiple musculoskeletal anomalies. The term itself is a misnomer. It is a drug-induced syndrome of hypermetabolism that may represent an inborn error of metabolism. Recognition, prophylaxis and crisis management are reviewed, and the use of dantrolene sodium in the care of patients with malign

Subacute cutaneous lupus erythematosus: a cutaneous marker for a distinct lupus erythematosus subset.

We have characterized the clinical and laboratory features of 27 patients who had in common a recurring, superficial, nonscarring type of cutaneous lupus erythematosus (LE) that occurred in a characteristic distribution (subacute cutaneous lupus erythematosus [SCLE]). This clinically distinct form of cutaneous LE has not previously been analyzed as a separate entity and thus, its clinical importance has not been fully appreciated. We found that these patients frequently had a mild systemic illness marked by musculoskeletal complaints and serologic abnormalities. Forty-eight percent had systemi

Ultrasound Screening of Achilles Tendon, Patellar Tendon, and Plantar Fascia for Pain Development in Collegiate Athletes: A Multi-institutional Prospective Evaluation.

BACKGROUND: Diagnostic ultrasound can evaluate musculoskeletal structures for abnormalities that correlate with tendinopathy and fasciopathy. Previous literature suggests that sonographic screening of tendons and fascia can identify structures that are at risk for developing pain, although this has not been evaluated in collegiate athletes competing in a variety of sports. PURPOSE: To evaluate the use of ultrasound for screening the patellar tendon, Achilles tendon, and plantar fascia for time-loss injury in collegiate athletes during a full year. STUDY DESIGN: Cohort study; Level of evidenc

Spasticity Interventions: Decision-Making and Management.

Spasticity results from an abnormality of the central nervous system and is characterized by a velocity-dependent increase in muscle tone or stiffness. In children, it can cause functional impairments, delays in achieving developmental or motor milestones, participation restrictions, discomfort, and musculoskeletal differences. Unique to children is the ongoing process of a maturing central nervous system and body, which can create the appearance of worsening or changing spasticity. Treatment options include physical interventions such as stretching, serial casting, and bracing; oral and injec

Magnetic Resonance Imaging of the Shoulder: Interpretation of Common Orthopaedic Injuries.

Magnetic resonance imaging (MRI) is frequently obtained to assess for pathology in the setting of shoulder pain and dysfunction. MRI of the shoulder provides diagnostic information that helps optimize patient management and surgical planning. Both general and subspecialized orthopaedic surgeons routinely order and review shoulder MRIs in practice. Therefore, familiarity with the MRI appearance of common shoulder pathologies is important. This document reviews the most common shoulder pathologies using a standardized MRI interpretative approach. Instructional videos demonstrating a musculoskele

Poland's syndrome: a case report and review of literature.

Poland's Syndrome is a rare congenital condition. It is classically characterised by absence of unilateral chest wall muscles and sometimes ipsilateralsymbrachydactyly (abnormally short and webbed fingers). The condition typically presents with unilateral absence of the sternal or breast bone portion of the pectoralis major muscle which may or may not be associated with the absence of nearby musculoskeletal structures. Most of the Poland Syndrome is sporadic. We report a 19-year-old patient with variant of Poland Syndrome. To the best of our knowledge, this is one of the few documented cases o

Unbalanced t(4;11)(q32;q23) in a 34-year-old man with manifestations of distal monosomy 11q and trisomy 4q syndromes.

We present a 34-year-old man with an unbalanced translocation between the long arms of chromosome 4 and chromosome 11. He had manifestations of monosomy 11(q23)--minor facial anomalies, abnormal head shape, cryptorchidism; trisomy 4(q32)--hirsutism, renal disease; and manifestations attributable to both imbalances--heart disease, musculoskeletal anomalies, and mental retardation. FISH studies showed that the chromosome 11q23.3 translocation breakpoint was distal to the rare folate sensitive fragile site (FRA11B). The patient is the oldest reported with both imbalance of 4q+ and 11q-.
