Prophylactic bilateral intramedullary femoral nails for bisphosphonate-associated signs of impending subtrochanteric hip fracture.

In the short and midterm, bisphosphonates have proven highly efficacious in the prevention of low-energy fractures, but long-term results and adverse effects have yet to be definitively identified. Of particular concern are emerging reports of long-term bisphosphonate users presenting with unusual low-energy subtrochanteric femur fractures. Perhaps associated with hyperactive bone remodeling leading to an eventually weakened bony architecture, the efficacy of longer-term bisphosphonate use has come into question, especially in those with >5 years of therapy. This article describes a case of a 6

A simple and practical method in treatment of ingrown nails: splinting by flexible tube.

BACKGROUND: The ingrown nail (or onychocryptosis) is a common problem that occurs mostly in the big toe and causes high amounts of morbidity in affected patients. Many therapeutic methods have been described, most of them leading to severe damage to the nail or to frequent relapses. The nail splinting technique is a successful, simple and non-invasive therapeutic method for treating ingrown nails. OBJECTIVE: To determine efficacy of the plastic tube insertion technique in patients with ingrown toenail and its use as a standard treatment. MATERIALS AND METHODS: We encountered 32 cases (age ra…

[Ender nails in the stabilization of trochanteric fractures of the femur in the elderly].

Results of treatment for trochanteric fracture of the femur in 50 patients aged 25-91 (mean 71 years) are presented. In all cases reviewed the fracture has been stabilized with 3 or 4 Ender nails. In majority of cases bony union has been achieved. Short operating time, small incision remote from fracture site, early weight bearing constitute unquestionable advantages of the method. Most frequent complications as shortening of the extremity involved, excessive external rotation, pain related to migration of the nails, varus deformity at the fracture site were usually found in patients with mark…

Neurologic and vascular structures at risk during anterior-posterior locking of retrograde femoral nails.

OBJECTIVE: To evaluate the neurovascular structures at risk with the placement of anterior-posterior locking screws in the proximal femur. DESIGN: Cadaveric study. MAIN OUTCOME MEASURE: Anatomic relationships. RESULTS: The femoral artery lies medial to the femur, and its branches cross the anterior femur more than four centimeters distal to the lesser trochanter. Branches of the femoral nerve cross the anterior femur proximal to the lesser trochanter and start four centimeters distal to the piriformis fossa. CONCLUSIONS: Risks to the neurovascular structures during anterior-posterior locki

Oral terbinafine (Lamisil) in the treatment of fungal infections of the skin and nails.

The efficacy and safety of antifungal drugs depend upon their mode of action, the minimal inhibitory concentration (MIC) and its relationship to the minimal fungicidal concentration (MFC), the spectrum of activity and drug kinetics at the involved site. Terbinafine acts at the fungal cell wall. Its MIC against dermatophytes is the lowest of all currently available systemic antifungal agents. It is the only one with an MIC:MFC ratio of 1:1 so that terbinafine should be effective over very short treatment durations in dermatophyte infections of the scalp, palms and soles, and nail, providing tha

The use of Ender nails in femoral shaft fractures: what are the remaining indications?

The use of Ender nails for the treatment of femoral shaft fractures has been described as technically easier and less time consuming than current intramedullary nailing techniques. We reviewed our results with unlocked Ender nails in 26 stable and 17 unstable fracture patterns an average of 3-4 years after injury. Because of continued instability, 42% of the stable and 76% of the unstable groups required adjunctive stabilization in the form of skeletal traction, a cast, or an external fixator. Additionally, nail migration and shortening and loss of motion at the knee were seen in 14 fractures …

Yellow nails, lymphedema and pleural effusion. Treatment of chronic pleural effusion with pleuroperitoneal shunting.

Pleural effusion secondary to lymphedema may be chronic, symptomatic and refractory to treatment, occasionally requiring invasive and painful procedures such as chemical pleurodesis, open pleural abrasion or pleurectomy to achieve control of the effusion and gain symptomatic relief. We report a patient with yellow nail syndrome and chronic pleural effusion successfully treated with pleuroperitoneal shunting.

Pleural effusion and recurrent broncho-pneumonia with lymphedema, yellow nails and protein-losing enteropathy.

A case is reported in whom the triad generalized lymphedema, nail dystrophy, and pleural effusion was associated to protein-losing enteropathy. This combination, not previously described, was also characterized by exacerbations of pleural effusion with recurrent episodes of broncho-pneumonia. Albumin turnover study showed depletion of the total body pool, decreased catabolic rate, and elevated albumin removal through the gastrointestinal tract. During bronchopneumonia, increased capillary permeability due to pleural involvement may worsen the basic deficit of pleural lymphatic drainage.

Cosmetics and ancillary preparations for the care of nails. Composition, chemistry, and adverse reactions.

Cosmetics and ancillary preparations for care of the nails have long been used in abundance by women and to a much lesser extent by men. In this article, types of such cosmetics and ancillary preparations, their composition, and something of the chemistry of their ingredients are given. Several examples of adverse reactions to them are described and illustrated. The incidence of adverse reactions to them is small as a percentage, but significant as an absolute number because the number of them used is huge. When adverse reactions occur they may be severe.

Prevalence of pathogenic fungi in the toe-webs and toe-nails of diabetic patients.

100 diabetic and 100 diabetes-free patients were mycologically examined for the presence of pathogenic fungi in their toe-webs and toe-nails. While there were clinical signs of presumed mycotic infection in 73 of the diabetic and in 66 of the non-diabetic subjects, the examination of the KOH-treated specimens revealed fungal elements in only 70 of the former and in 53 of the latter group. Isolation of the causative agent was possible in 57 of the diabetic patients (T. rubrum in 46%, C. albicans in 31%, T. mentagrophytes in 21% and E. floccosum in 3%) and in 40 of the control group (T. rubrum 5…

Pleural-fluid kinetics in a patient with primary lymphedema, pleural effusions, and yellow nails.

We present the clinical findings in the thirteenth recorded patient with lymphedema, pleural effusions, and ungual dystrophy--the so-called "yellow nail" syndrome. The rate of pleural-fluid turnover was measured by using radioiodinated albumin to trace the efflux of fluid by lymphatics. The rate of pleural lymphatic flow was low in comparison to previous estimates obtained by the same method in a variety of other conditions. Such slow lymphatic flow is consistent with the postulate that accumulation of pleural fluid in this syndrome is due to defective lymphatic drainage.

Comparing outcomes of plate augmentation, nail exchange, and nail exchange with plate augmentation in the treatment of atrophic femoral shaft nonunion after intramedullary nailing: a multicenter retrospective study.

INTRODUCTION: Intramedullary (IM) nailing is the treatment of choice for femoral shaft fractures, but nonunion rates have been reported to be as high as 12%. Surgical interventions for nonunion involve exchange nailing or plate augmentation. Recently, a combined treatment of exchange nailing and plate augmentation has demonstrated good results, but its comparative effectiveness remains unclear. This study aimed to compare the clinical and radiographic outcomes of three different surgical interventions for atrophic femoral shaft nonunion, and investigate the factors that affect bone healing aft

[Histological investigation of nail growth in human embryos and regrowth of nail after its removal in adult monkeys--with reference to clinical observation in injuries to the nail and its surrounding tissues].

As the basis to develop adequate treatments which can preserve the nail plate or assure its regrowth in frequently encountered injuries to the nail-surrounding tissues, a histological investigation was made of nail growth in human embryos and regrowth of nail after its removal in adult monkeys. The author's investigation in these two series indicated the following. Firstly the nail plate composed of parakeratotic layer is produced solely of "nail matrix", thus, supporting Zaias' and opposing Lewis' theory as to the nail production. Secondly though the normal, completely formed nail matrix and

Reaming-induced joint surface destruction in tibiotalocalcaneal nailing: a cadaveric comparison of straight and valgus-curved nails.

BACKGROUND: Tibiotalocalcaneal (TTC) nailing has become an increasingly preferred option for the treatment of complex ankle pathologies and fragility fractures, particularly in geriatric patients. Despite its growing clinical use, limited data are available regarding how different nail designs impact joint surface integrity during intraoperative preparation. This cadaveric study aimed to compare the articular surface destruction caused by straight and valgus-curved TTC nails. METHODS: 20 fresh-frozen cadaveric lower limbs (mean age 70.1 ± 4.1 years) were randomized to receive either a straigh…

Percutaneous Straightening and Exchange Nailing of a Bent Intramedullary Nail Following Tibial Shaft Fracture: A Case Report.

INTRODUCTION: Bending of tibial intramedullary (IM) nails is an extremely rare complication, usually resulting from high-energy trauma. Management options are not standardized, and removal or exchange nailing can be technically challenging. CASE REPORT: We report the case of a 23-year-old male who sustained a displaced fracture of the left tibial shaft following a motorcycle accident. He was treated with closed reduction and IM nailing (9 mm), with uneventful recovery and fracture union at 6 months. One year later, the patient was involved in another motorcycle accident, leading to acute pain…

Cosmetic-Associated Acute Paronychia With Green Nail-Bed Pigmentation in a Child Following Nail Polish Application.

Acute paronychia in children is typically associated with trauma or habitual behaviors, whereas cosmetic-related cases are rarely reported. We describe a 4-year-old girl who developed acute paronychia with persistent green nail-bed pigmentation following the application of nail polish to a nail with pre-existing cuticular disruption. Staphylococcus epidermidis was isolated from purulent discharge, while Pseudomonas aeruginosa was not detected. The clinical course supports a dual pathogenic mechanism involving bacterial invasion through a compromised cuticle and chemical irritation from cosmeti
