A narrative approach for the management of low back pain in general practice: An in-depth patient interviews study.

BACKGROUND: The broad range of definitions of low back pain (LBP) and the many associated risk factors on which management strategies are based do not seem to be relevant for general practitioners. OBJECTIVES: Given the challenges of treating LBP and its impact on individuals' life, we aimed to explore the lived experiences of LBP patients to better understand their feelings, needs, and the internal and external resources they use for coping and treatment. METHODS: We used a comprehensive, qualitative, biographical approach to understand the patient's life events and the origins of these eve

Semantic details in autobiographical memory narratives increase with age among younger adults.

It is well documented that older adults, compared to younger adults, produce fewer episodic details and more semantic details when recalling autobiographical memories. However, group comparisons have provided limited insight into the trajectories of detail generation across the lifespan. Utilising an open source dataset [Clark, I. A., & Maguire, E. A. (2023). Release of cognitive and multimodal MRI data including real-world tasks and hippocampal subfield segmentations. Scientific Data, 10(1), 1-29. https://doi.org/10.1038/s41597-022-01899-x], we examined how episodic and semantic detail genera

Mothering a Child With Complexity and Rarity: A Narrative Inquiry Exploring Prader-Willi Syndrome.

Daily experiences of mothers caring for children with Prader-Willi syndrome (PWS) are largely unknown and unvoiced. Knowledge of PWS has generally focused on pathology of the disorder. This emphasis overlooks the challenging moments of everyday life caring for children with PWS. Storied accounts of mothers caring for children with PWS offer expanded narratives to medicalized descriptions of experience. An understanding of everyday challenges in managing physical and mental health issues of PWS including hyperphagia and anxiety may create shifts in social and clinical perspectives. This underst

(Re)constructing identity following acquired brain injury: The complex journey of recovery after stroke.

INTRODUCTION: People with poststroke acquired brain injury (ABI) face a complex and often troubled identity reconstruction journey. The literature is rich with studies related to the psychological and neuropsychological components involved in rehabilitation, but it is lacking with respect to the investigation of the existential dimensions and the challenges associated with finding new senses and meanings for one's identity and future perspectives, body and interpersonal relationships. METHODS: The aim of this study is to investigate the narrative processes of identity reconstruction after bra

Self-reported Cannabidiol (CBD) Use for Conditions With Proven Therapies.

IMPORTANCE: Use of cannabidiol (CBD) has markedly increased in the past 5 years, concurrent with marketing claims that over-the-counter CBD can be used to treat almost any health condition. However, the reasons why individuals use CBD remain unclear. OBJECTIVE: To assess whether individuals are using CBD for diagnosable conditions that have evidence-based therapies. DESIGN, SETTING, AND PARTICIPANTS: This case series assessed claimed treatment applications reported by CBD users in public testimonials shared on the Reddit forum r/CBD. The r/CBD forum was selected because it includes a large,

Putting the "where" in HIV care: Unpacking narratives of antiretroviral therapy adherence among HIV-Positive men who have sex with men.

This study explores the spatial constitution of adherence to antiretroviral therapy (ART) by recasting therapeutic landscapes (Gesler, 1992) and how it structures the exercise of expressive agency (Bowden, 2014). Engagement in antiretroviral therapy among HIV-positive men who have sex with men (MSM) is contextualized within the discursive-materiality of emplaced assemblages for HIV Care in the Philippines. Combining qualitative data from field visits and semi-structured interviews, three key spatial narratives were derived illustrating how adherence to ART unfolds in place: (a) an unwelcoming

Using Self-Reported Patient Experiences to Understand Patient Burden: Learnings from Digital Patient Communities in Ankylosing Spondylitis.

INTRODUCTION: Online communities contain a wealth of information containing unsolicited patient experiences that may go beyond what is captured by guided surveys or patient-reported outcome (PRO) instruments used in clinical settings. This study described patient experiences reported online to better understand the day-to-day disease burden of ankylosing spondylitis (AS). METHODS: Unguided, English-language patient narratives reported between January 2010 and May 2016 were collected from 52 online sources (e.g., general/health social networking sites, patient-physician Q&A sites, AS forums).

Care, Control, and the Electroconvulsive Therapy Ritual: Making Sense of Polarized Patient Narratives.

Despite evidence of short-term effectiveness of ECT (electroconvulsive therapy), both positive and negative patient reports are common. However, research examining these polarized accounts has not adequately elucidated why such divergences occur. We thus sought to examine opposing patient narratives to better understand underlying meanings. Eighteen interviews were conducted with U.K.-based people who had experienced the treatment. Our analysis revealed that the quality of relations with staff, ECT artifacts (e.g., the ECT suite), and perceived outcomes all play a role in divergent accounts. P

Conscious thought is for facilitating social and cultural interactions: how mental simulations serve the animal-culture interface.

Five empirically based critiques have undermined the standard assumption that conscious thought is primarily for input (obtaining information from the natural environment) or output (the direct control of action). Instead, we propose that conscious thought is for internal processing, to facilitate downstream interaction with the social and cultural environment. Human consciousness enables the construction of meaningful, sequential thought, as in sentences and narratives, logical reasoning, counting and quantification, causal understanding, narratives, and the simulation of events (including no

[Delusional disorder in a chronic renal dialysis patient--deprivation and reclaiming of body and narrative].

We report a female case treated with hemodialysis over 20 years who demonstrated good social functioning, until she suffered from delusional disorder with persecutory delusions involving medical staff performing the dialysis and also auditory hallucinations involving them. We examined the bio-psycho-social invasiveness of the renal dialysis treatment, especially concerning the effects on the patient's self-image of her body resulting in the loss of a feeling of unity between her mind and body, or the sense of her body belonging to her, referring to J. Lacan's concept of "corps imaginaire= imag

Motives and methods of under-age casino gamblers.

Numerous studies have documented that under-age gambling is quite common. The present study employs interviews of 48 university students who gambled under-age to determine the motives and methods associated with casino gambling by minors. The information gathered in these interviews indicates that access to casinos is easily attained, that the risk of exposure once gambling is minimal, and the motivations of under-age gamblers differ in important ways from that of adults, as access to alcohol, accompanying parents, and the desire to experience the "forbidden fruit" of casino gambling are commo

FAMISHED for support: recovering elders after cardiac events.

BACKGROUND: Heart disease causes disproportionately heavy burdens on unpartnered elders (widowed, divorced, never married) who have limited social resources to contend with recovery demands and complications resulting from acute myocardial infarction (AMI) or coronary artery bypass graft (CABG) surgery. Research suggests that the availability of social support improves recovery after AMI and CABG, yet functional support expressly designed to modify health and stimulate recovery remains unspecified. Advanced practice nurses (APNs) in the clinical nurse specialist (CNS) role who provide social s

Making sense of traumatic experiences: telling your life with fragile X syndrome.

The discourse of individuals with fragile X syndrome has been described as fragmented, tangential, and associatively connected, resulting in autobiographical narratives that are incoherent and difficult to understand. In this article, the authors discuss the case of an adolescent female with moderate mental retardation due to fragile X syndrome. The analysis of her autobiographical stories, rather than being primarily reflective of cognitive impoverishment, reveals a different--narrative--mode of thought. The authors present and examine some of her narratives in terms of their specific functio

The Effects of the Processing of Positive Memories Technique on Posttrauma Affect and Cognitions Among Survivors of Trauma: Protocol for a Daily Diary Study.

BACKGROUND: The Processing of Positive Memories Technique (PPMT) is a promising new treatment approach for posttraumatic stress disorder (PTSD), which involves detailed narration and processing of specific positive autobiographical memories. Indeed, preliminary case-series studies have found reductions in PTSD symptoms, negative affect, and negative cognitions among survivors of trauma who have received PPMT. However, PPMT's effects have not been investigated at the daily level. In this study, we describe the protocol for a study that will examine the daily-level impacts of PPMT in a trauma-ex…

Experimental validation of a portable tidal volume indicator for bag valve mask ventilation.

INTRODUCTION: Short-term emergency ventilation is most typically accomplished through bag valve mask (BVM) techniques. BVMs like the AMBU® bag are cost-effective and highly portable but are also highly prone to user error, especially in high-stress emergent situations. Inaccurate and inappropriate ventilation has the potential to inflict great injury to patients through hyper- and hypoventilation. Here, we present the BVM Emergency Narration-Guided Instrument (BENGI) - a tidal volume feedback monitoring device that provides instantaneous visual and audio feedback on delivered tidal volumes, re…
