[Preliminary results of the relevance the sentinel node in head and neck tumors].

OBJECTIVES: The objective of this study is to assess the value of the sentinel node (SN) in head and neck tumours and to create a lymphatic … tus of the neck. In 2 patients, the SN was negative for metastases but there were metastases in the neck dissection. CONCLUSIONS: This technique represents an advance over selective dissection as an accurate method of neck staging, but its value in this field remains unclear.

Squamous cell carcinoma of the upper aerodigestive tract: the prevalence of microscopic extracapsular spread and soft tissue deposits in the clinically N0 neck.

BACKGROUND: With squamous cell carcinoma of the upper aerodigestive tract the presence or absence of neck metastases is the most important prognostic factor. This makes the histopathologic assessment of neck dissections of paramount importance. With the clinically N0 neck the prevalence of microscopic extracapsular spread and soft tissue deposits has not previously been described. METHODS: We have prospectively analyzed 96 elective neck dissections in 63 patients with upper aerodigestive tract squamous cell carcinoma and clinically N0 necks to assess the prevalence of microscopic extracapsula…

Carcinoma ex pleomorphic adenoma of the submandibular gland: report of a case with an unusual malignant component of clear cell squamous cell carcinoma.

A large carcinoma ex pleomorphic adenoma (CXPA) with regional lymph node metastasis occurred in the left submandibular gland of a 64-year-old man. Initially, the patient underwent tumor extirpation and ipsilateral radical neck dissection. Histologic examination revealed the malignant component of CXPA was clear cell squamous cell carcinoma (SCC), which is quite uncommon in head and neck region. In the metastatic regional lymph nodes, aggregates of clear cells were observed. To the best of our knowledge, this is the first case of CXPA of which the malignant component is clear cell SCC. Four mon…

Management of head and neck melanoma: results of a national survey.

We recently reported on the safety of minimally invasive parotid region sentinel node biopsy and level I-sparing radical neck dissection for head and neck melanoma. We therefore wished to assess the state of practice in the United States through a survey of specialists in head and neck surgery. We hypothesized that there would be significant variation in the management of these facets of head and neck melanoma. To test this hypothesis, a 10-question online survey on management of head and neck melanoma was distributed to the members of the American Head and Neck Society. Responses were matched…

Risk factors and clinical presentation of craniocervical arterial dissection: a prospective study.

BACKGROUND: Craniocervical arterial dissection is a major cause of ischaemic stroke in young adults. The pathogenesis is not fully understood but is thought to be related to a combination of an intrinsic weakness in the arterial wall and an external trigger. Intrinsic susceptibility is thought to be a generalised arteriopathy, vascular anomaly or genetic predisposition. Proposed extrinsic factors include recent viral infection and minor mechanical trauma to the neck, including neck manipulation, which has raised concerns amongst manual practitioners in particular as to the appropriate screenin

Stent-assisted coil embolization of ruptured vertebral artery dissected aneurysm with severe stenosis of bilateral vertebral artery V4 segment by the transmountain technique: a case report and review of the literatures.

A 46-year-old woman presented with acute head and neck pain for 10 h. Head CT showed subarachnoid hemorrhage (SAH) and digital subtraction angiography (DSA) identified a ruptured dissected aneurysm of the right vertebral artery with severe artery stenosis. Moreover, an unruptured dissecting aneurysm and severe vascular stenosis were also found in the left vertebral artery. How to deal with ruptured bleeding aneurysm and prophylactically deal with contralateral unruptured dissecting aneurysm and the stenosis of the vertebral artery has become a thorny problem. By adopting the Transmountain tech

The value of PET/CT to assess clinically negative necks.

The aim was to evaluate the efficacy of [(18)F]-2-fluorodeoxyglucose positron emission tomography/computed tomography (PET/CT) in evaluating metastasis of head and neck squamous cell carcinoma (HNSCC) to the cervical lymph nodes, with specific attention to the efficacy in regard to clinically negative necks. This was a retrospective review of 243 patients with HNSCC between years 2005 and 2007 treated at a comprehensive cancer care institution who underwent pre-operative PET/CT and neck dissection with either an N0 (112 patients) or N+ (131 patients) clinical nodal status. PET/CT findings were…

Partial Horner's Syndrome as a Manifestation of Carotid Artery Dissection: A Case Report and Diagnostic Approach.

Horner's syndrome is a neurological condition resulting from a disruption in the sympathetic pathways supplying the head and neck. This syndrome often occurs secondary to various aetiologies, including trauma, neoplastic compression, or vascular pathology such as carotid artery dissection. Although the clinical presentation is well-defined, Horner's syndrome remains relatively rare, and there are limited established management guidelines. Of particular concern is the acute onset of occipital pain in association with partial Horner's syndrome, as it raises the suspicion of cervical artery disse

Distribution and determinants of submandibular gland involvement in oral cavity squamous cell carcinoma.

INTRODUCTION: Though the submandibular gland (SMG) is routinely sacrificed for several reasons during neck dissection in patients undergoing curative surgery for oral cavity cancers, it might be an innocent bystander and should be considered for preservation. This study aimed to identify the incidence, different patterns of invasion, and risk factors of SMG involvement in oral cavity squamous cell carcinoma (SCC). METHODS: This was a retrospective study of the patients who underwent upfront curative surgery for a biopsy-proven oral cavity SCC. A consistent protocol-based treatment strategy wa…

Traumatic Carotid Artery Dissection Following a Brazilian Jiu-Jitsu Chokehold: A Case Report.

Carotid artery dissection is a rare but important cause of stroke in young adults, often resulting from trauma rather than atherosclerotic disease. This case describes a previously healthy 37-year-old male who experienced a sudden onset of left facial and arm numbness and weakness during exercise, shortly after participating in a Brazilian Jiu-Jitsu (BJJ) session. A chokehold maneuver one week earlier was identified as a likely precipitating factor. Initial symptoms resolved within an hour, but imaging a week later revealed a right internal carotid artery dissection with a small cortical infar
