Evaluation of Mean Platelet Volume and Platelet Distribution Width as an Early Marker of Neonatal Sepsis: A Case-Control Study.

BACKGROUND: Neonatal sepsis (NS) is one of the greatest concerns in developing countries and the leading cause of mortality in the neonatal intensive care unit. Due to non-specific and overlapping signs, it poses a difficulty for early diagnosis of sepsis. Blood culture, though the gold standard for diagnosis of sepsis, takes at least 72 h to show positive growth. Early indicators are the need of the hour to predict NS, so that early empirical antibiotics could be initiated. AIM OF THE STUDY: To evaluate Mean platelet volume (MPV) and Platelet distribution width (PDW) as early diagnostic mark…

Umbilical cord myiasis in a preterm neonate: a rare sepsis aetiology uncovered by root cause analysis - case report.

Myiasis is the infestation of living human or animal tissue by fly larvae and is most common in wounds, body orifices or necrotic tissue in tropical and subtropical regions where hygiene is poor. Of the various forms of human myiasis, umbilical cord myiasis is exceedingly rare, with most reported cases occurring after the 5th day of life in infants cared for at home. A preterm neonate born at 30 weeks gestation developed umbilical cord myiasis with early-onset sepsis by 36 hours of life in a neonatal intensive care unit. This is one of the earliest and rarest presentations to be reported. Live

Optimizing Vancomycin Exposure for Early Treatment Response in Very-Low-Birth-Weight Neonates With Late-Onset Sepsis.

BACKGROUND: Vancomycin is widely used to treat neonatal late-onset sepsis (LOS). However, its optimal pharmacokinetic/pharmacodynamic targets in neonates, particularly those with very low birth weight (VLBW), remain inadequately defined. Therefore, the objective of this study is to identify vancomycin exposure thresholds associated with early clinical and microbiologic effectiveness in VLBW neonates with LOS. METHODS: A multicenter observational study included VLBW neonates who received vancomycin for LOS and had at least 1 steady-state trough concentration measured. Based on clinical and mic

Impact of Early-Onset Sepsis on the Development and Severity of Intraventricular Hemorrhage in Premature Neonates: A Retrospective Single-Center Study.

OBJECTIVE: Intraventricular hemorrhage (IVH) is a major cause of morbidity and mortality in premature infants. Early onset neonatal sepsis (EOS) may increase the risk and severity of IVH through inflammation and hemodynamic instability. This study evaluated the association between sepsis and IVH severity. METHODS: This retrospective, single-center study included neonates diagnosed with IVH and followed in the Neonatal Intensive Care Unit of a tertiary hospital between January 2021 and December 2022. IVH grading was based on Papile's classification, and patients were categorized as low-grade (…

Protocol: Pentoxifylline optimal dose finding trial in preterm neonates with suspected late onset sepsis (PTX-trial).

BACKGROUND: Late onset sepsis is a leading cause of death and morbidity in preterm infants. Despite optimal antibiotic treatment, sepsis related mortality and morbidity is still high. Pentoxifylline (PTX) is a methylxanthine with promising immunomodulatory properties, which can be used as an additional therapy next to antibiotics in preterm infants. PTX is increasingly used off-label in neonatal intensive care units, however up till now no dose finding study has been done for PTX in this specific population. The aim of this study (PTX-trial) is to determine the optimal dose of PTX in preterm i

Prolonged Post-Discontinuation Antibiotic Exposure in Very Low Birth Weight Neonates at Risk for Early-Onset Sepsis.

BACKGROUND: Premature, very low birth weight (VLBW) neonates are at risk for early-onset sepsis and receive ampicillin and gentamicin post-birth. Antimicrobial stewardship supports short-course antibiotics, but how long antibiotic concentrations remain therapeutic post-last dose is unknown. METHODS: Using Monte Carlo simulations (NONMEM 7.3), we analyzed antibiotic exposures in a retrospective cohort of 34 689 neonates (<1500 g, 22-27 weeks of gestation). Therapeutic exposure for ampicillin and gentamicin was evaluated relative to the minimum inhibitory concentration (MIC) for common pathogen
