Intersegmental coordination of the leech swimming rhythm. II. Comparison of long and short chains of ganglia.

Preparations of the nearly isolated leech nerve cord containing as few as two ganglia are sufficient to generate intersegmentally coordinated swim oscillations, provided that they receive tonic excitation from other segments via the median connective (Faivre's nerve). Due to their greatly reduced complexity, these preparations should provide useful experimental models of neuronal coordination. As a step in the development of such models, we have characterized the intersegmental coordination of nerve-cord chains ranging from 2 to 18 ganglia in length. We found that increases in swim-cycle perio

Monomelic amyotrophy.

From 1977 through 1981, we examined 23 patients with single-limb atrophy. Thirteen had upper-limb and ten had lower-limb involvement. The characteristic clinical features were insidious onset in the second and third decades, male preponderance, sporadic occurrence, wasting and weakness confined to one limb, and absence of involvement of the cranial nerves, cerebrum, brain stem, and sensory system. The electromyographic features, along with histologic features of neurogenic atrophy, were suggestive of an anterior horn cell lesion. The slow progression of illness for two to four years followed b

[Adie's syndrome and polyneuropathy: apropos of a new case and a case of pupillotonia, polyneuropathy and generalized amyloidosis sparing the nerve].

The association of Adie's syndrome and of neuropathy of chronic evolution is not a frequent occurrence; first made known by Togi in 1967, four cases have been described ever since. The authors report two new observations: one concerns a man showing a unilateral tonic pupil associated to sensory neuropathy, raised C.S.F. protein, rarefaction of myelinated fibres with schwannian proliferation, without amyloid deposit. The other is that of a woman affected with bilateral pupillotonia, motor neuropathy of the four limbs and generalized amylosis. If the first observation is comparable to the five c

Predominant sensory Guillain-Barré syndrome in hereditary sensory neuropathy--case report of a three-year-old boy.

We observed a three-year-old boy with increasing ataxia over a period of two weeks, which cleared up completely within four months. The electroneurographic features and their course in the child and the father allow us to suspect the additional existence of a hereditary sensory neuropathy. The authors put forward the hypothesis that the genetic disturbance of the sensory nerves is responsible for this clinical variant of Guillain-Barré syndrome.

[Tarsal tunnel syndrome caused by granulomatous arteritis of an artery supplying the posterior tibial nerve. Apropos of a case].

A patient presented with a tarsal tunnel syndrome due to granulomatous arteritis of the artery supplying the tibial nerve. The pathogenicity of these types of lesion is discussed based on findings during operation, and a logical therapeutic attitude established: early opening of the tarsal tunnel with interfascicular neurolysis of the tibial nerve.

Occupationally acquired vibratory angioedema with secondary carpal tunnel syndrome.

After several years of occupational exposure to vibration, a man developed episodic vibratory hand swelling, erythema, and pruritus. He subsequently developed symptoms compatible with carpal tunnel syndrome, but his median nerve conduction velocity remained normal. After experimental vibration of the forearm, plasma histamine levels in the ipsilateral antecubital vein rose in association with localized angioedema and slowed median nerve conduction velocity. There was no evidence of other causes of angioedema. Occupational exposure to vibration may predispose to the development of vibratory ang

Tarsal tunnel syndrome.

In 14 patients tarsal tunnel syndrome was associated with varus heels and pronated, splayed forefeet. Review of the literature and the author's experience suggest that these conditions may be a common cause of the tarsal tunnel syndrome. Treatment of fixed varus deformities of the heel by outer heel wedges has been shown to be ineffective. Although outer heel wedges provide symptomatic relief in patients with relatively flexible hindfeet, surgical release of the flexor retinaculum behind the medial malleolus is the treatment of choice in patients who have the tarsal tunnel syndrome with planov

Myokymia in Guillain-Barré syndrome.

Facial or limb myokymia was present in 8 patients (17%) of 48 consecutive cases of Guillain-Barré syndrome. It occurred early in the course of the illness and persisted 5 to 40 days as the patients recovered. Facial myokymia usually occurred in clinically weak muscles, was most often bilateral, and was more common in women. Bilaterality and transient nature differed from the facial myokymia seen in pontine tumors and demyelinating disease.

The origin of fasciculations.

A collision method in a single axon was used to determine the origin of 100 fasciculations in various lower motor neuron lesions. For 80% of the fasciculations, it was situated on the distal extremity of the axon regardless of the type of lesion, its duration, or the severity of the denervation. For several reasons, this proportion is underestimated. The fasciculations of distal origin were sometimes, associated with double discharges, the mechanism of production and clinical significance of which probably differ.

Peripheral motor neuropathy associated with autonomic dysfunction in two sisters: new hereditary syndrome?

We describe two sisters with distal, slowly progressive muscular weakness and hypotrophy since childhood, autonomic dysfunction characterized by profuse sweating, distal cyanosis related to cold weather, orthostatic hypotension, and esophageal achalasia. Nerve conduction velocity of several motor nerves was slow, and although no sensory abnormalities were present, sural nerve biopsy revealed severe nonspecific demyelination. No similar patients could be found in the literature and we therefore suggest the possibility that these individuals have a newly recognized hereditary syndrome.

[Compression syndromes of peripheral nerves of the upper extremity (author's transl)].

Among the non-traumatic lesions of peripheral nerves the compression syndromes occupy a numerically leading position. After mentioning the common characteristics the individual compression syndromes of the brachial plexus and peripheral nerves of the upper extremity are described: the compression of the brachial plexus in the posterior triangle of the neck, in the costoclavicular passage and on hyperabduction. The suprascapular notch syndrome ist mentioned. The distal compression syndrome of the median nerve, the carpal tunnel syndrome is described in detail as the most common of the compressi

Palmar conduction time of median and ulnar nerves of normal subjects and patients with carpal tunnel syndrome.

The wrist-palm conduction time for the median and ulnar nerves was determined using antidromic technique in thirty normal subjects. For the median nerve, the conduction time was 1.6 msec. to initial deflection from baseline and 1.7 msec. to peak of the initial negative deflection for the mean plus 2 SD. The median wrist-palm conduction time was then compared to the difference between the median and ulnar wrist-digit conduction times (mean plus 2 SD of 0.5 msec. measured either to initial deflection or peak negative deflection) in order to determine which technique aids more in the electrodiagn

Effects of temperature on some membrane characteristics of carotid body cells.

Excised cat carotid bodies and nerves were placed in a Lucite chamber through which saline flowed at different temperatures. Action potentials were recorded with a suction electrode. Glomus cells were impaled with microelectrodes and identified by ejecting Procion navy blue from the micropipette. Membrane potentials (MP) averaged 20 mV and input resistances (R0), 40 Momega. Cooling induced depolarization and decreased R0. This effect had a reversal potential of about -6 mV. [K+]0 did not affect MP, R0, or cooling effects. Lack of sodium increased MP, R0, and depressed cooling effects. Low chlo

On-line identification of sensory systems using pseudorandom binary noise perturbations.

A technique of on-line identification of linear system characteristics from sensory systems with spike train or analog voltage outputs was developed and applied to the semicircular canal. A pseudorandom binary white noise input was cross-correlated with the system's output to produce estimates of linear system unit impulse responses (UIRs), which were then corrected for response errors of the input transducers. The effects of variability in the system response characteristics and sensitivity were studied by employing the technique with known linear analog circuits. First-order unit afferent re

Heredopathia atactica polyneuritiformis (phytanic acid storage disease). A new case with special reference to dietary treatment.

A 33-year-old man with Refsum disease exhibited clinical features of night blindness, dysequilibrium, hearing loss, itchy dry skin, symmetrical polyneuropathy, distal muscle weakness, pes cavus, and hammer toe. His total serum protein was increased, nerve conduction velocities were slow, and serum phytanic acid levels were high. Dietary restriction of phytol resulted in a decrease in serum phytanic acid without any visual and autitory changes; however, coordination, skin lesions, and nerve conduction velocities definitely improved.

Dendritic spikes and their inhibition in alligator Purkinje cells.

Alligator Purkinje cells generate action potentials in the peripheral dendritic tree, after synaptic depolarization via superficial parallel fibers. These action potentials are inhibited at the dendrite level by preceding parallel-fiber volleys at close intervals. We conclude that this inhibition is produced by the activation of the inhibitory interneurons of the molecular layer, the stellate cells, which establish synaptic contacts with the dendrites of the Purkinje cells.
