Long-term clinical outcomes and complications after Open Reduction and Internal Fixation (ORIF) with locking plate for proximal humerus fractures: a systematic review and meta-analysis.

BACKGROUND: Open reduction and internal fixation (ORIF) using locking plates remains a widely used head-preserving treatment for displaced proximal humerus fractures in adults. While short-term outcomes are well documented, long-term functional performance, complication burden, and implant survivorship beyond 5-years remain poorly defined. This systematic review and meta-analysis aimed to synthesise long-term (≥ 5-years) functional outcomes, complication rates, and reoperation rates following locking plate ORIF for proximal humerus fractures. METHODS: A protocol-registered (PROSPERO CRD420251

Using autograft in the surgical treatment of isolated distal ulna fractures with open reduction internal fixation improves short-term clinical outcomes: 11 years of experience.

BACKGROUND: Most of the studies on ulna fractures assess the treatment of fractures accompanied by other tissue injuries; thus, the number of studies focused directly on isolated distal ulna fractures are very few. In this study, it was aimed to evaluate the short-term results of open reduction internal fixation (ORIF) and autograft+ORIF approaches in the treatment of isolated distal ulna fractures. HYPOTHESIS: Using autograft in the surgical treatment of isolated distal ulna fractures with open reduction internal fixation improves short-term clinical outcomes. PATIENTS AND METHODS: The reco

Comparison of Regional Anesthesia Timing on Pain, Opioid Use, and Postanesthesia Care Unit Length of Stay in Patients Undergoing Open Reduction and Internal Fixation of Ankle Fractures.

Regional nerve blocks are an effective method of managing acute pain associated with surgery. The relative benefit of preoperative versus postoperative peripheral nerve blocks is not entirely clear. The primary aim of this study was to determine differences in pain scores in patients undergoing preoperative block versus postoperative block versus no block. We hypothesized that patients receiving preoperative blocks would have reduced pain scores and decreased opioid use in the immediate postoperative period. We conducted a retrospective cohort analysis of 302 consecutive patients undergoing un

Fragility fractures of the ankle in the elderly: Open reduction and internal fixation versus tibio-talo-calcaneal nailing: Short-term results of a prospective randomized-controlled study.

INTRODUCTION: The management of ankle fractures in the elderly remains unpredictable, secondary to their various co-morbidities. Although tibiotalocalcaneal (TTC) nailing has been an effective option for ankle arthrodesis due to ankle arthritis or Charcot arthropathy there are few reports regarding the use of TTC nail for the treatment of ankle fractures. PURPOSE: Aim of this study was to compare the results of ORIF versus TTC nailing for the treatment of unstable ankle fractures in the elderly. We hypothesized that the elderly may benefit from TTC nailing, as it allows the patient to be mobi

Changes in the radiological measurements of the tibiofibular syndesmal area in patients with Weber C ankle fractures who were treated with open reduction, internal fixation, and transyndesmal screw.

The tibiofibular syndesmosis provides stability to the ankle mortise. The ankle syndesmosis is compromised in all Weber C type injuries. The radiographic method described by Merle DAubigné considers the bony relationships as a measure of syndesmotic widening. We sought to investigate whether the patients with a C type ankle fracture treated with ORIF and placement of a transyndesmal screw have an increment of the tibiofibular space and decrease of the tibiofibular overlap after the transyndesmal screw is removed. Our sample included 52 patients with Weber C ankle fractures treated by ORIF and

Minimally invasive (sinus tarsi) approach for open reduction and internal fixation of intra-articular calcaneus fractures in children: surgical technique and case report of two patients.

Calcaneus fractures in children differ from those in adults. Most calcaneus fractures in children can be managed nonoperatively, with good long-term results expected. The width and height of the calcaneus can remodel with time in children. Recently, there has been a trend toward operative treatment of displaced intra-articular fractures of the calcaneus in children to correct the articular deformity. Studies of calcaneal fracture fixation in children used an extended lateral approach, with its possible complications. In the present report, we describe the operative treatment of 2 children (12

Supracondylar fractures with circulatory failure after reduction, pinning, and entrapment of the brachial artery: excellent results more than 1 year after open exploration and revascularization.

OBJECTIVES: Neurovascular injuries in children with dislocated supracondylar humeral fractures are not uncommon. Closed reduction and pin fixation usually will restore the circulation. In some patients, there is still compromised circulation and a neurologic deficit, and they are treated with open exploration and microvascular reconstruction. We have investigated the clinical and functional outcome more than 1 year after the injury in this most serious group of patients. DESIGN: Retrospective follow-up study. PATIENTS: During 2001 to 2007, five patients were referred to our department with a

Conservative management versus open reduction and internal fixation for mid-shaft clavicle fractures in adults--the Clavicle Trial: study protocol for a multicentre randomized controlled trial.

BACKGROUND: Clavicle fractures account for around 4% of all fractures and up to 44% of fractures of the shoulder girdle. Fractures of the middle third (or mid-shaft) account for approximately 80% of all clavicle fractures. Management of this group of fractures is often challenging and the outcome can be unsatisfactory. In particular it is not clear whether surgery produces better outcomes than non-surgical management. Currently there is much variation in the use of surgery and a lack of good quality evidence to inform our decision. METHODS/DESIGN: We aim to undertake a multicentre randomised

Early functional and operative outcomes of three open surgical approaches compared with closed reduction and percutaneous pinning in Gartland type III pediatric supracondylar humerus fractures.

Supracondylar humerus fractures (SHFs) are commonly treated with closed reduction and percutaneous pinning (CRPP); however, open reduction is required in selected cases. To compare early functional and operative outcomes of anterior, lateral, and posterior open reduction techniques, using CRPP as a reference standard. This retrospective study included 60 pediatric patients with Gartland type III SHFs (CRPP, n = 15; anterior, n = 15; lateral, n = 15; posterior, n = 15). Outcomes included operative time, fluoroscopy use, union time, joint range of motion, Flynn functional and cosmetic scores, an

Clinical outcomes and complication rates of minimally invasive plate osteosynthesis (MIPO) versus open reduction and internal fixation (ORIF) in midshaft clavicular fractures: a comparative study.

BACKGROUND: Conventional open reduction and internal fixation (ORIF) remains the standard approach for midshaft clavicle fractures, yet its traumatic nature has driven the need for minimally invasive techniques. Locking minimally invasive plate osteosynthesis (MIPO) demonstrates clinical potential through biomechanical superiority and tissue-preserving characteristics. OBJECTIVE: To compare the clinical efficacy and safety of MIPO versus ORIF for midshaft clavicular fractures. METHODS: A retrospective cohort study included 203 patients (MIPO group:101 cases; ORIF group:102 cases) treated bet

Reduced 90-Day Cardiovascular and Infectious Complications After Open Reduction Internal Fixation of Ankle Fractures in Type II Diabetic Patients Using Semaglutide Preoperatively.

BACKGROUND: Patients with type II diabetes mellitus (T2DM) are known to have inferior postoperative outcomes after open reduction and internal fixation (ORIF) of ankle fractures. Semaglutide (the active agent in Ozempic) has encouragingly been associated with improved postoperative outcomes across multiple elective orthopaedic surgeries; however, this has not been assessed for patients with T2DM undergoing ankle fracture ORIF. METHODS: Adult patients with T2DM who underwent ankle fracture ORIF were identified from the 2010-Q1 2023 PearlDiver M170Ortho database. Exclusion criteria included the
