Ophthalmology update for primary practitioners. Part I. Update on optic neuritis.

Optic neuritis is a common cause of acute visual loss. It is typified by sudden onset of visual impairment and pain with eye movements, followed by spontaneous recovery of vision over several months. Pathologically, optic neuritis is an acute demyelinating event affecting the optic nerve. Objective physical findings are typically few, including an afferent pupillary defect or Marcus-Gunn pupil, whereas subjective psychophysical findings abound (ie, diminished central visual acuity, color vision, decreased contrast sensitivity, and visual field abnormalities). These characteristics have made th…

Seasonal patterns in optic neuritis and multiple sclerosis: a meta-analysis.

To quantify and characterize seasonal variation in monosymptomatic optic neuritis (MON) onsets, multiple sclerosis (MS) onsets and MS exacerbations (MSE), a meta-analysis was performed, using established methods and pooling weighted information obtained from nine reports on MON, six reports on MS onsets and nine reports on MSE, which fulfilled specific criteria for report quality and data homogeneity. The results suggested that MON, MS onsets and MSE in the Northern hemisphere present a similar pattern with highest frequencies in spring and lowest in winter. These differences were highest for …

Screening for mt-DNA mutations in optic neuritis of unknown cause.

OBJECTIVE: To investigate mitochondrial DNA (mt-DNA) mutations in optic neuritis of unknown cause (ONUC) and to assess the practical value of mt-DNA mutation detection in etiologically and differentially diagnosing ONUC. METHODS: Thirty patients with ONUC were screened for mt-DNA mutations of nt11778, nt3460 and nt15257 by using SSCP, mutation-specific primer PCR and sequencing. RESULTS: mt-DNA mutations were found in twelve of thirty ONUC patients. All of the mutations were at nt11778 position, but no one at nt3460 and nt15257. CONCLUSIONS: Forty percent (12/30) of ONUC patients were cause…

Recovery of visual field function in the optic neuritis treatment trial.

PURPOSE: To assess the pattern of recovery of the visual field of patients with optic neuritis and to determine whether all affected portions of the visual field recover similarly or certain portions of the visual field have greater recovery. METHODS: We reviewed the Humphrey Visual Field (Allergan-Humphrey Inc, San Leandro, California) data from the initial and 6-month examination for the involved and fellow eyes of patients enrolled in the Optic Neuritis Treatment Trial (ONTT). The average threshold for each patient was calculated for the entire tested field and for locations within concent…

Conjugated deferoxamine reduces blood-brain barrier disruption in experimental optic neuritis.

The purpose of this paper was to investigate the role of deferoxamine (DFO) scavenging of hydroxyl radical (.OH) on disruption of the blood-brain barrier (BBB) and demyelination in experimental optic neuritis. Eighteen strain-13 guinea pigs were sensitized for experimental allergic encephalomyelitis. Nine animals received 100 mg/kg of hydroxyethyl starch-conjugated (HES) DFO by daily intraperitoneal injection commencing the day of antigenic sensitization. Nine paired litter mates received daily IP injections of HES. Serial fat-suppressed magnetic resonance imaging of the optic nerves was obtai…

Bilateral optic neuritis with branch retinal artery occlusion associated with vaccination.

A case of a 40-year old marine with bilateral optic neuritis and a branch retinal artery occlusion after vaccination is presented. Blood investigations showed no abnormalities. Cerebrospinal fluid studies revealed a lymphocytic pleocytosis and IgG antibodies against hepatitis A and rabies. Computerized tomography and magnetic resonance imaging of the brain were negative. A diagnosis of vaccine-induced auto-immune demyelinative optic neuritis was made. The clinical picture improved after systemic corticosteroid treatment.

[Visual functions of optic neuritis in children caused by demyelinating disease].

Optic neuritis in childhood caused by demyelinating disease is very rare, and its effect on visual function has not been well documented. The visual function of 11 patients treated in the last 11 years was retrospectively studied and the results were compared with 15 cases of optic neuritis of unknown etiology. Optic neuritis in children with demyelinating disease was characterized as follows. 1. abnormal visual evoked cortical potentials (VECP) and color vision were found not only in the affected eye but also in the non-affected fellow eye, 2. VECPs became normal long after the recovery of vi…

Loss of P retinal ganglion cell function in resolved optic neuritis.

To determine the relative loss of P and M (X and Y) ganglion cell function in unilateral resolved optic neuritis, 10 patients with 20/20 or better Snellen acuity in both eyes had contrast sensitivity testing, color vision testing, and automated perimetry. We used contrast sensitivity gratings of 0.5 cycles per degree (cpd) with a rate of counterphase temporal modulation of 30 Hz and gratings of 11.4 cpd at 1 Hz. On 1 trial, patients responded when they detected the pattern of the grating and on the next trial when they 1st perceived movement. There was a significant difference in the 1 Hz high…

[MR imaging of optic neuritis using short T1 IR (inversion recovery)].

We evaluated the ability of MRI using short T1 inversion recovery (STIR) to diagnose optic neuritis. Eleven patients with previous or recent attack of optic neuritis were studied with MRI at 0.5 tesla. STIR images revealed high signal regions in 7 of 12 symptomatic and 5 of 10 asymptomatic nerves. Three of five asymptomatic nerves with high intensity were pertinent to the cases with past attack and seemed to reflect the demyelinating change. The other two nerves were pertinent to the cases without past attack and seemed to show occult lesions. We consider that STIR is useful in detection of op…

Treatment of optic neuritis with intravenous megadose corticosteroids. A consecutive series.

Twelve consecutive patients with optic neuritis of unknown etiology treated with 250 to 500 mg methylprednisolone every 6 hours are described. All patients improved, some dramatically and rapidly. Patients with swollen discs and visual loss for less than 6 weeks improved uniformly, rapidly, and completely. In some patients, progressive visual deterioration was reversed rapidly. Serum serologies and cerebrospinal fluids findings were of limited use in determining etiology or prognosis with treatment. Side effects included exacerbation of a preexisting psychosis, elevated glucose levels, and ora…

The visual evoked potential contrast response function in recovering optic neuritis.

VEP contrast response functions were obtained in 7 patients with recovered optic neuritis and in 6 age-matched controls. A 1.5 c/d sinusoidal grating was counterphase modulated at 4 Hz (8 reversals/sec) about a mean screen luminance of 141 cd/m2. Grating contrasts ranged from 2% to 85% and were randomly presented. In all control observers, VEP amplitude was a nonmonotonic function of contrast; an amplitude inflection occurred between contrasts of 33% to 47% followed by a sharp increase in amplitude at higher contrasts. The slope of the function relating VEP phase to contrast was variable betwe…

Retrobulbar optic neuritis in a patient with acute type B hepatitis.

A case is described of retrobulbar optic neuritis in a 33-year-old female with acute type B hepatitis. The ocular distress, principally affecting the left eye, followed the normalisation of liver function tests and subsided after steroid therapy. At the onset of ocular symptomatology, complement activation, involving both classic and alternative pathways, and high levels of circulating immune complexes were present. HBsAg was positive. No relapse was observed during follow-up for 3 years. An association between HBV-infection, optic neuritis and an immune complexes-mediated neurotoxic activity …

Two common neuro-ophthalmic problems. Optic neuritis and transient visual disturbances.

Optic neuritis and transient visual disturbances are common and challenging neuro-ophthalmic problems. Optic neuritis may occur during the course of several neurologic and systemic disorders and is characterized by reversible central visual loss. In many patients, signs and symptoms of multiple sclerosis occur after an episode of optic neuritis. Although several risk factors for development of multiple sclerosis have been identified, the relationship between optic neuritis and multiple sclerosis is still controversial. Transient visual disturbances may take the form of visual loss or visual ha…

[Results of static perimetry in subclinical optic neuritis in multiple sclerosis].

Static perimetry of the central visual field was performed using Harms apparatus and Friedmann's analyser in 20 M.S. patients with subclinical optic neuritis at least for one eye. Sub-clinical optic neuritis was defined as delayed or absent pattern reversal visual evoked potentials (VEPs) associated with normal routine ophthalmological examination (visual acuity, optic fundi, colour vision, kinetic perimetry). Results in patients were compared to data obtained in 22 control subjects, matched for age. A highly significant loss of mean retinal sensitivity was observed in the 20 central degrees o…

Optic neuritis, diabetes mellitus and multiple sclerosis: a three-way association.

Patients with multiple sclerosis (MS) and a history of optic neuritis were compared with MS patients who had no such history for other illnesses among themselves and their family members. Significantly more of the patients with a history of optic neuritis than of those without (70% v. 41%) were diabetic or reported that at least one first- or second-degree relative had diabetes mellitus. No differences were found with regard to any other illnesses.
