The Fabrication of Requisite, Durable, Printed Upper Extremity Orthoses Through Innovative Means in an Austere Environment.

INTRODUCTION: In deployed military settings, ensuring consistent access to durable medical equipment (DME) remains a significant challenge due to logistical and supply chain limitations. This study retrospectively reviews the clinical use of 3D-printed upper extremity orthoses fabricated out of necessity at Al Udeid Air Base (AUAB), Qatar, in response to such shortages. Devices were implemented during routine care and assessed through provider documentation and photographic evidence. Although representative of logistics-supported Role 2s, the results may not fully extend to forward combat Role

Total and partial knee arthroplasty versus non-surgical interventions of the knee for moderate to severe osteoarthritis.

RATIONALE: Total and partial knee arthroplasty (TKA, PKA) are common treatments for moderate to severe knee osteoarthritis. While effective in symptom management, these surgeries carry potential risks, necessitating careful evaluation of benefits and harms. High-quality evidence comparing TKA/PKA to placebo, sham, or non-surgical interventions is essential to support informed decision-making. OBJECTIVES: To assess the benefits and harms of TKA and PKA for people with moderate to severe knee osteoarthritis compared to placebo, sham (efficacy), or non-surgical interventions for the knee (effect

Orthosis in the Treatment of Osteoporosis-associated Fractures and Chronic Symptoms in the Spine - a Systematic Review.

Osteoporosis is a systemic skeletal disease of multi-aetiological origin and is a major factor in health economics. The reduction in bone mass and disruption of the microarchitecture lead to an increased risk of fracture. The therapy is versatile, with orthoses being used in the treatment of acute vertebral fractures as well as chronic pain. The aim of this work is to formulate evidence-based recommendations for the use of orthoses in osteoporotic vertebral fractures and chronic symptoms. The literature search was conducted according to the PRISMA protocol at PubMed, ScienceDirect, Cochrane and

Gender differences in trunk appearance perception and health-related quality of life (HRQoL) in the patients with moderate adolescent idiopathic scoliosis (AIS) undergoing orthotic treatment: An observational study.

BACKGROUND: Orthotic treatment is commonly used as a non-surgical intervention for managing moderate adolescent idiopathic scoliosis (AIS). Although prior studies have evaluated various factors influencing health-related quality of life (HRQoL) in the patients with AIS, the association between trunk appearance perception and HRQoL, including potential gender differences, remains insufficiently defined. This study investigated gender differences in trunk appearance perception and its relationship with QoL among the patients with moderate AIS undergoing orthotic treatment. METHODS: Patients wit

Immediate effects of standing unstable board intervention on the non-paralyzed leg on sitting balance in severe hemiplegia: a randomized controlled trial.

BACKGROUND: Unstable board intervention for patients with stroke improves sitting balance and trunk function. However, because patients with severe stroke are at high risk of falling, it is mostly adapted in mild cases. OBJECTIVE: We aimed to examine the effect of standing unstable board intervention for the non-paralyzed lower limbs on sitting balance in patients with hemiplegia. METHODS: The participants were 42 patients with stroke who were randomly assigned to a control or intervention group. In the intervention group, the non-paralyzed leg was placed on an unstable board, and the patien

The effect of a modified elastic band orthosis on gait and balance in stroke survivors.

BACKGROUND: Gait is crucial for independent living for stroke survivors and assistive devices have been developed to support gait performance. Ankle foot orthosis (AFOs) are commonly provided to stroke survivors to prevent foot drop during walking. However, previous studies have reported limitations of AFOs including them being too heavy, creating skin irritation, and being a stigma of disability. OBJECTIVE: The purpose was to compare the gait and balance improvement between elastic band orthosis (EBOs) and AFOs. STUDY DESIGN: Experimental study design. METHODS: The AFOs and EBOs were provi

Long-term Results of Developmental Hip Dysplasia Under Therapy With Pavlik Harness.

INTRODUCTION: The Pavlik harness is a commonly used treatment for developmental dysplasia of the hip (DDH) of types IIc or worse, based on the Graf classification. This study aimed to assess the long-term follow-up outcomes after treating DDH with the Pavlik harness. METHODS: Between 1995 and 2006, ultrasound screening was conducted on 7372 newborns within the first days of life. Among them, 203 dysplastic hips in 152 patients, whose sonograms were Graf type IIc or worse, were treated with Pavlik harness. Treatment was discontinued if the measurement indicated Graf type Ia/b. More than 20 yea

Prognostic factors for failed Pavlik harness treatment in infants with developmental dysplasia of the hip: a retrospective cohort study.

PURPOSE: Pavlik harness treatment is the most common treatment in newborns diagnosed with developmental dysplasia of the hip (DDH). The success rates and predictors for failure have been debated over the last decade. In this study, we explored our treatment failure rate and potential prognostic factors that could predict the failure of Pavlik harness (PH) treatment in patients with DDH. METHODS: Two hundred and sixty-five patients were treated with PH based on the Graf hip types of classification. Age, gender, first born status, family history, foot deformity, plagiocephaly, breech presentati

What Are the Rate and Risk Factors for Developing a Complication With the Pavlik Method for the Treatment of Hip Dysplasia?

BACKGROUND: The Pavlik method for the treatment of developmental dysplasia of the hip (DDH) has been proven successful for over 85 years. The high success rate and reproducibility have made it the mainstay of treatment. METHODS: We performed a retrospective cohort study of patients with DDH treated with the Pavlik method between September 2016 and August 2018 with at least 24 months of follow up in a single academic center. We excluded patients with neuromuscular conditions, teratologic dislocations, and arthrogryposis. We identified and included a total of 307 patients in the analysis. There

The effect of a flexible thoracolumbar brace on neuromuscular scoliosis: A prospective observational study.

Neuromuscular scoliosis is a common deformity seen in patients with neuromuscular diseases. Although rigid thoracolumbosacral orthosis is the most frequently used brace, it has low compliance rates and can lead to complications including skin ulcers. Thus, alternative methods for treating neuromuscular scoliosis are needed. The purpose of this study is to evaluate the clinical effects of a novel flexible brace to prevent the progression of neuromuscular scoliosis. This study is a prospective observational study. Twenty-three patients with neuromuscular scoliosis were enrolled in the study. Amon

Comparison of the quality of life in individuals with spinal cord injury wearing either reciprocating gait orthosis or hip knee ankle foot orthosis: a cross-sectional study.

BACKGROUND: The quality of life (QoL) for patients with spinal cord injuries (SCI) is lower than that for healthy individuals. The main purpose of prescribing orthoses for these individuals is to improve their mobility and QoL. The hip knee ankle foot orthosis (HKAFO) has been the conventional choice for such patients, whilst the reciprocating gait orthosis (RGO) is a more contemporary option. Although the impact of these two types of orthoses on the biomechanics of walking has been previously evaluated in patients with SCI, there has been no specific comparison of their relative effects on Qo

Three-dimensional changes in head shape after extended sagittal strip craniectomy with wedge ostectomies and helmet therapy.

OBJECTIVE Outcome studies for sagittal strip craniectomy have largely relied on the 2D measure of the cephalic index (CI) as the primary indicator of head shape. The goal of this study was to measure the 2D and 3D changes in head shape that occur after sagittal strip craniectomy and postoperative helmet therapy. METHODS The authors performed a retrospective review of patients treated with sagittal strip craniectomy at their institution between January 2012 and October 2015. Inclusion criteria were as follows: 1) isolated sagittal synostosis; 2) age at surgery < 200 days; and 3) helmet manageme

Allergic or Hypersensitivity Reactions to Orthopaedic Implants.

Allergic or hypersensitivity reactions to orthopaedic implants can pose diagnostic and therapeutic challenges. Although 10% to 15% of the population exhibits cutaneous sensitivity to metals, deep-tissue reactions to metal implants are comparatively rare. Nevertheless, the link between cutaneous sensitivity and clinically relevant deep-tissue reactions is unclear. Most reactions to orthopaedic devices are type IV, or delayed-type hypersensitivity reactions. The most commonly implicated allergens are nickel, cobalt, and chromium; however, reactions to nonmetal compounds, such as polymethyl metha

Design, construction, and evaluation of "sensor lock": an electromechanical stance control knee joint.

BACKGROUND AND AIM: Most currently-available stance control knee ankle foot orthoses (SCKAFOs) still need full knee extension to lock the knee joint, and they are still noisy, bulky, and heavy. Therefore, the aim of this study was to design, construct, and evaluate an original electromechanical SCKAFO knee joint that could feasibly solve these problems, and thus address the problems of current stance control knee joints with regards to their structure, function, cosmesis, and cost. METHOD: Ten able-bodied (AB) participants and two (knee ankle foot orthosis) KAFO users were recruited to partic

Comparative effectiveness of ultrasound and paraffin therapy in patients with carpal tunnel syndrome: a randomized trial.

BACKGROUND: Conclusive evidence indicating an effective treatment for carpal tunnel syndrome (CTS), a common entrapment neuropathy, is lacking. Ultrasound therapy (US therapy) has long been used as one of the combination treatments for CTS. In addition, paraffin bath therapy has been applied widely as a physical modality in treating patients with hand conditions. The purpose of this randomized trial was to compare the efficacy of combining a wrist orthosis with either US therapy or paraffin bath therapy in treating CTS patients. METHODS: Patients with CTS were randomized into two groups. All
