Preservation of conductive hearing in approaches to tumors of the jugular foramen.

OBJECTIVE: Surgical approaches to the jugular foramen, most often used for the resection of glomus jugulare tumors, may include removal of the external auditory canal wall and overclosure of the meatus, resulting in maximal conductive hearing loss. Modifications have been described that maintain hearing by preserving the canal wall at the price of decreased exposure and are, therefore, suitable only for small and favorably located tumors. Our technique for removal and then reconstruction of the canal wall with hydroxyapatite cement allows for complete anterior translocation of the facial nerve

The Role of Total Transcanal Endoscopic Modified Radical Mastoidectomy in Extensive Mastoid Cholesteatoma: A Preliminary Report.

INTRODUCTION: Mastoid cholesteatoma is classically managed through a postauricular mastoidectomy approach. With the increasing use of endoscopic techniques in otologic surgery, various middle ear diseases can now be managed endoscopically. Modified radical mastoidectomy (MRM) still has a role in managing extensive mastoid cholesteatoma. OBJECTIVES: To evaluate the feasibility of total transcanal endoscopic modified radical mastoidectomy (TTEMRM) for mastoid cholesteatoma at a single tertiary center. METHODS: This 5-year retrospective review included 16 patients involving 17 operated ears wit

Acoustically functional 3D‑printed middle ear model for prosthesis development and otosurgical training.

BACKGROUND: Cadaver temporal bones are traditionally used in otosurgical training and research, but they have many limitations. The development of novel individualized middle ear prostheses requires easy and durable artificial middle ear models that are as realistic as possible anatomically, functionally, and acoustically. The aim of this study was to create an acoustically and mechanically functional, three-dimensionally (3D) printed middle ear model that overcomes the challenges of cadaver temporal bones while promoting prosthesis development. METHODS: Structures from microcomputed tomograp

Adverse skin reactions following percutaneous bone conduction implant surgery using the linear incision technique with and without subcutaneous tissue reduction.

CONCLUSIONS: Adverse skin reactions were equally distributed among age groups, surgical technique, or presence of a retroauricular incision. Implant loss was observed more frequently in children when compared to adults and elderly. OBJECTIVE: A bone conduction (osseointegrated) implant can be used for rehabilitation of patients with conductive hearing loss or single-sided deafness. The surgical technique has been modified to minimize adverse skin reactions and other complications. Two commonly used techniques are the linear incision technique with subcutaneous tissue reduction and the techniq

Correlations of External Landmarks With Internal Structures of the Temporal Bone.

HYPOTHESIS: The internal anatomy of a temporal bone could be inferred from external landmarks. BACKGROUND: Mastoid surgery is an important skill that ENT surgeons need to acquire. Surgeons commonly use CT scans as a guide to understanding anatomical variations before surgery. Conversely, in cases where CT scans are not available, or in the temporal bone laboratory where residents are usually not provided with CT scans, it would be beneficial if the internal anatomy of a temporal bone could be inferred from external landmarks. METHODS: We explored correlations between internal anatomical vari

Acute VOR gain differences for outward vs. inward head impulses.

BACKGROUND: Vestibular rehabilitation is a sub-specialization within the practice of physical therapy that includes treatments designed to reduce gaze instability. Gaze stability exercises are commonly given for head rotations to the left and right, even in subjects with one healthy vestibular system (as in unilateral loss). Few studies have investigated the difference in the angular vestibular ocular reflex gain (aVOR) measured in the acute phase after deafferentation for ipsilesional head rotations that move the head away from center or towards center. OBJECTIVE: The purpose of this study w
