The role of hospitals in monitoring the emergency: the experience of "Sentinel network" of the Italian Federation of Health Trusts.

The COVID-19 pandemic showed few weak points in health-system resilience and highlighted the need to strengthen surveillance capacities. In Italy, the Federation of Health Trusts (FIASO) established the Sentinel Hospital Network (SHNet) to monitor COVID-19 hospitalizations and support operational decision-making. A multicentre ecological study was conducted across 21 hospitals (November 2021-March 2023), collecting weekly data on admissions, vaccination status, and classification of cases as for or with COVID-19. Trends were compared with national surveillance from the Istituto Superiore di Sa

Identification of a Potent Pan-Coronaviral Main Protease Inhibitor.

The global impact of SARS-CoV-2 and the continued emergence of zoonotic coronaviruses underscore the urgent need for broad-spectrum antivirals for pandemic preparedness. Herein, we report AVI8122, a covalent pan-coronaviral inhibitor that targets 19 Mpros across the α, β, γ, and δ genera, encompassing bat, human, and other animal coronaviruses. AVI8122 exhibits low nanomolar potency and favorable pharmacokinetics in mice. Structural studies reveal that AVI8122 forms a covalent bond with the catalytic cysteine and maintains conserved interactions within the active sites of these Mpros. In cellu…

Lessons learnt from one-year cold chain support for COVID-19 vaccination in 5 middle income countries of North Africa.

The COVID-19 pandemic demanded rapid scale-up of vaccine cold chain capacity worldwide. With support from the Government of Japan, five middle-income countries in North Africa; Algeria, Djibouti, Egypt, Libya and Tunisia strengthened their cold chain systems between May 2021 and May 2022. This 12-month initiative added 341,146 litres of storage space across 1238 facilities, trained 2850 health staff, and deployed WHO-PQS-qualified equipment worth USD 2.93 million. There were supply and installation delays linked to the equipment and the vaccines. Despite these delays, the investment improved t

Supporting molecular detection of A(H5N1) through development and characterization of RNA reference materials.

In 2024, the US Centers for Disease Control and Prevention (CDC) and National Institute of Standards and Technology (NIST) initiated an interagency collaboration to bolster preparedness in the event of a pandemic scenario due to high pathogenicity avian influenza A(H5N1) clade 2.3.4.4b viruses that had been circulating in North America since 2021. Multiple spillover events into mammalian species, including an outbreak among dairy cattle and a surge in human cases, spurred the development of RNA reference materials to assist with new diagnostics development and validation. With input from CDC o

Pre-existing and early cellular immune factors correlate with functionally complete protection against primary controlled human SARS-CoV-2 infection.

Identifying host factors that mediate protection against newly-emergent viruses is needed for improved pandemic preparedness. Here, we analysed pre- and early post-exposure immune factors associated with resisting SARS-CoV-2 infection after human challenge in seronegative individuals, using multiplex protein, cytometric and RNA sequencing approaches in the nasopharynx and circulation. Pre-existing cross-reactive antibodies correlate poorly with clinical outcome. Instead, protection is associated with heightened nasopharyngeal CCL13 levels locally produced by conventional dendritic cells and mo…

Exploiting high-energy hydration sites for the discovery of potent peptide aldehyde inhibitors of the SARS-CoV-2 main protease with cellular antiviral activity.

Small-molecule antivirals that prevent the replication of the SARS-CoV-2 virus by blocking the enzymatic activity of its main protease (Mpro) are and will be a tenet of pandemic preparedness. However, the peptidic nature of such compounds often precludes the design of compounds within favorable physical property ranges, limiting cellular activity. Here we describe the discovery of peptide aldehyde Mpro inhibitors with potent enzymatic and cellular antiviral activity. This structure-activity relationship (SAR) exploration was guided by the use of calculated hydration site thermodynamic maps (Wa…

A pox on all our houses: a missing component in the global mpox response is equity.

The global health community has had more than a decade to develop pandemic preparedness programmes and apply lessons learnt to new disease outbreaks. Mpox has now been declared a global public health emergency, but the response appears to be missing important elements of equity, focusing instead on diagnosis and surveillance. This approach leaves vulnerable populations in countries grappling with the outbreak without the preventive and treatment services they need. Based on our experiences managing the current mpox outbreak in Burundi, we outline some limitations of ongoing response strategies…

ANP32B Deficiency Protects Mice From Lethal Influenza A Virus Challenge by Dampening the Host Immune Response.

Deciphering complex virus-host interactions is crucial for pandemic preparedness. In this study, we assessed the impact of recently postulated cellular factors ANP32A and ANP32B of influenza A virus (IAV) species specificity on viral pathogenesis in a genetically modified mouse model. Infection of ANP32A-/- and ANP32A+/+ mice with a seasonal H3N2 IAV or a highly pathogenic H5N1 human isolate did not result in any significant differences in virus tropism, innate immune response or disease outcome. However, infection of ANP32B-/- mice with H3N2 or H5N1 IAV revealed significantly reduced virus lo…

Influenza A virus resistance to 4'-fluorouridine coincides with viral attenuation in vitro and in vivo.

Pre-existing or rapidly emerging resistance of influenza viruses to approved antivirals makes the development of novel therapeutics to mitigate seasonal influenza and improve preparedness against future influenza pandemics an urgent priority. We have recently identified the chain-terminating broad-spectrum nucleoside analog clinical candidate 4'-fluorouridine (4'-FlU) and demonstrated oral efficacy against seasonal, pandemic, and highly pathogenic avian influenza viruses in the mouse and ferret model. Here, we have resistance-profiled 4'-FlU against a pandemic A/CA/07/2009 (H1N1) (CA09). In vi
