BACKGROUND: At birth, infants' lungs are fluid-filled. For newborns to have a successful transition, this fluid must be replaced by air to enable gas exchange. Some infants are judged to have inadequate breathing at birth and are resuscitated with positive pressure ventilation (PPV). Giving prolonged (sustained) inflations at the start of PPV may help clear lung fluid and establish gas volume within the lungs.

Pharmacological and/or surgical closure of a hemodynamically significant patent ductus arteriosus (PDA) in the very preterm infant has been the standard of care over the past few decades. However, the rationale for closure of PDA has recently been challenged. In this article, the factors that have fueled the controversy of the approach to the management of PDA and the gap in our knowledge are reviewed in detail. In addition, the pros and cons of the different treatment strategies applied in clinical care are evaluated with a focus on discussing the available evidence in the literature.

During a 12-year-period, 33 infants 4 days to 1 year old underwent 36 operations for treatment of coarctation of the aorta. Associated cardiac anomalies were present in 27 infants (81.8%). All patients presented with congestive heart failure and underwent cardiac catheterization and aortography. Resection was performed in 11 infants, patch aortoplasty in 6, bypass in 2 and subclavian flap angioplasty in 17.
