Efficacy of Patient Activation Interventions With or Without Financial Incentives to Promote Prescribing of Thiazides and Hypertension Control: A Randomized Clinical Trial.

IMPORTANCE: Evidence-based guidelines recommend thiazide diuretics as a first-line therapy for uncomplicated hypertension; however, thiazides are underused, and hypertension remains inadequately managed. OBJECTIVE: To test the efficacy of a patient activation intervention with financial incentives to promote thiazide prescribing. DESIGN, SETTING, AND PARTICIPANTS: The Veterans Affairs Project to Implement Diuretics, a randomized clinical trial, was conducted at 13 Veterans Affairs primary care clinics from August 1, 2006, to July 31, 2008, with 12 months of follow-up. A total of 61 019 patie

Over the past few decades in the Canadian province of Quebec, patient participation and partnership in decision-making has been promoted through successive Mental Health Action Plans (MHAP) and other policies. Evaluation and Treatment of Acute and Subacute Hearing Loss: A Review of Pharmacotherapy.

Among various forms of hearing loss, there are acute (within 72 hrs) or subacute (weeks to months) presentations that may be reversible with early pharmacological intervention. The workup of a patient presenting with hypoacusia includes the usual history and physical examination in conjunction with an audiometric assessment in order to categorize the hearing loss as conductive, sensorineural, or mixed. Sudden sensorineural hearing loss and autoimmune inner ear disease are acute and subacute forms of sensorineural hypoacusia most likely to be reversed with prompt pharmacological intervention. S

Service users' and carers' views on research towards stratified medicine in psychiatry: a qualitative study.

BACKGROUND: Approximately 30% of people with a diagnosis of schizophrenia receive little to no benefit from current medications. There is therefore an urgent need to develop more precisely targeted and effective treatments. Identifying biomarkers to predict response to treatment and stratify patients into groups may be a way forward. However, we know little about service users' and carers' attitudes regarding such a 'stratified medicine' approach for psychiatry-nor how this might impact on their willingness to participate in stratified medicine research. This paper presents psychiatric service

Addressing the nonexercise part of the activity continuum: a more realistic and achievable approach to activity programming for adults with mobility disability?

Participation in physical activity is fundamental for the maintenance of metabolic health and the prevention of major chronic diseases, particularly type 2 diabetes and cardiovascular disease. A whole-of-day approach to physical activity promotion is increasingly advocated and includes not only increasing moderate-intensity physical activity but also reducing sedentary time and increasing light-intensity activity (the "nonexercise" part of the activity continuum). This whole-of-day approach to tackling the challenge of inactivity may be particularly relevant for adults with mobility disabiliti

Physician-patient agreement in the assessment of allergen relevance.

BACKGROUND: The efficacy of patch testing may be enhanced by data allowing the physician to estimate the likelihood that results of a patch-test reading are relevant to a patient's dermatitis. OBJECTIVE: The goal of this study was to compare the rates of agreement between the physician's assessment of relevance at the time of final reading and the patients' report 3 months to 3 years later in regard to whether avoidance of an allergen was needed to remain free of dermatitis. We hypothesize that the agreement rates between the physician and patient relevance assessments will vary based on prop

Cognitive Orientation to (daily) Occupational Performance (CO-OP) with children with Asperger's syndrome who have motor-based occupational performance goals.

BACKGROUND/AIM: Motor difficulties associated with Asperger's syndrome (AS) are commonly reported, despite these not being diagnostically significant. Cognitive Orientation to daily Occupational Performance (CO-OP) is a verbal problem-solving intervention developed for use with children with developmental coordination disorder to address their motor-based difficulties. This paper reports on two case studies of children with AS illustrating the outcomes of CO-OP to address motor-based occupational performance goals. METHODS: A case study approach was used to document how two children with AS e

Surveillance in Barrett's oesophagus.

Barrett's oesophagus is a premalignant condition with an increasing incidence of adenocarcinoma. There remains uncertainty based on the lack of accurate information, not least the necessity for, effectiveness of, and optimal interval for surveillance of known cases. The incidence of oesophageal cancer may not be as high as previously supposed, which could influence both surveillance intervals and cost effectiveness. Issues around patient selection have not been satisfactorily resolved; although most patients at risk are elderly and die of other causes, advanced oesophageal cancer is an unpleas

A personalised Healthcare Information Delivery System: pushing customised healthcare information over the WWW.

Easier and focused access to healthcare information can empower individuals to make 'informed' choices and judgements about personal health maintenance. To achieve 'optimum' patient empowerment, we need to re-evaluate and potentially re-design the processes of healthcare information delivery. Our suggestion is that healthcare information should be personalised according to each individual's healthcare needs and it should be pro-actively delivered, i.e. pushed towards the individual. We present an intelligent Personalised Healthcare Information Delivery Systems that aims to enhance patient empo

[The value of self-assessment in management of patients with benign prostatic hyperplasia].

Since benign prostatic hyperplasia (BPH) is a very common and slowly progressive disease which requires surgical treatment in only one-third of those affected, surveillance of the condition on the basis of subjective symptoms is of considerable importance. For this reason, the WHO developed a prostate symptom score (WHO-PSS or I-PSS), which includes both irritative and obstructive symptoms, and which has become accepted all over the world. This score can be used both in investigative studies as a means of monitoring the efficacy of therapy, and as a very good instrument for patient management

Family history of breast cancer and screening behavior: an inverted U-shaped curve?

There are conflicting reports regarding the association between breast cancer risk factors, especially family history, and early detection/screening behavior. While some studies have reported that women at higher risk for breast cancer engage in more screening behavior than those at lower risk, others have found no relationship between the two variables, and still other studies have found the opposite effect--less screening in high risk women as compared to lower risk women. It is suggested that these conflicting results might be explained by including anxiety or fear as a mediating factor in

Postoperative pain relief by demand analgesia.

Postoperative pain relief is only apparently an easy task. A brief survey of literature investigating the discomfort experienced in the postoperative phase is all but encouraging. One can identify four basic problems in obtaining adequate results by delivery of analgesic drugs: a) the biological variability among individual patients, b) the unpredictable uptake of the drug administered intramuscularly, c) the time lag involved between request by the patient and the subsequent administration of a single dose and d) the lack of knowledge about the nature of the discomfort and its remedies. An al

"There Was a Light Bulb Moment": Using Persona Dyads to Empathise, Explore and Develop Person-Centred Solutions in Mental Health Service Co-Design.

INTRODUCTION: Co-design of health services is an increasingly used method to meaningfully enable patient and public involvement, especially when working with priority populations whose perspectives are often overlooked in traditional top-down approaches. However, differences in power and influence among stakeholders can lead to asymmetry and the marginalisation of certain voices. We used persona dyads (patient and provider) to co-design improvements in Emergency Department care for people presenting with mental health concerns. METHODS: Persona dyads were used in two Experience-Based Co-Desig

An Online Decision Aid for Patients With Metastatic Melanoma—Results of the Randomized Controlled Trial “PEF-Immun”.

BACKGROUND: Treatment decisions in metastatic melanoma (MM) are highly dependent on patient preferences and require the patients' involvement. The complexity of treatment options with their individual advantages and disadvantages is often overwhelming. We therefore developed an online patient decision aid (PtDA) to facilitate shared decision making (SDM). METHODS: To evaluate the PtDA we conducted a two-armed, twocenter, prospective, open randomized controlled trial with MM patients who were facing a decision about first-line treatment. The patients were allotted randomly in a 1:1 ratio to an

Impact of a brief intervention on patient communication and barriers to pain management: results from a randomized controlled trial.

OBJECTIVE: This study examined the impact of a brief pain communication/education intervention on patient outcomes in breast cancer. We hypothesized that our intervention would improve patient communication and reduce misconceptions ("Barriers") concerning pain management, and that patients with lower Barriers, or who perceived their physician as being more facilitative and receptive, would report better outcomes. METHODS: Female breast cancer patients with persistent pain (n=89) were randomly assigned to either a 30-min in-person pain education/communication intervention or control condition

Improving patient information leaflets: developing and applying an evaluative model of patient-centredness for text.

The purpose of this paper is to present an evaluative model of patient-centredness for text and to illustrate how this can be applied to patient information leaflets (PILs) that accompany medication in the European Union. Patients have criticized PILs for sidelining their experiences, knowledge and affective needs, and denying their individuality. The health communication paradigm of patient-centredness provides valuable purchase on these issues, taking its starting point in the dignity and integrity of the patient as a person. Employing this evaluative model involves two stages. First, a Fouc

A framework to amplify the voices of underrepresented children and young people in rare disease research.

Rare diseases predominantly affect children and young people (CYP). Current research involving CYP often fails to adequately represent those from lower socioeconomic backgrounds, contributing to widening health and research inequalities. This concise report proposes a framework to build on successful methods that were able to engage with underrepresented CYP. This was achieved through partnership with social enterprise organisations and utilised connections in the community to provide positive knowledge exchange of local services that may enhance their socioeconomic situation, in turn for enga

Coproduction Without Youth? Closing the Participation Gap in Digital Mental Health Research.

Young people are among the most intensive users of digital and generative artificial intelligence (GenAI)-enabled mental health tools, yet they remain underrepresented in the research and design processes that shape these technologies. Although participatory approaches such as co-design and patient and public involvement are widely endorsed as best practices, youth involvement in digital youth mental health (DYMH) research is often inconsistent, superficial, or limited to late-stage consultation. This participation gap risks producing interventions that are misaligned with young people's lived

"A tool in a toolbox": patient engagement with a gamified and personalised approach bias modification app to reduce harmful alcohol consumption - a qualitative study.

BACKGROUND: Approach Bias Modification (ApBM) is a recommended adjunctive intervention during residential treatment for Alcohol Use Disorder (AUD), and there are also promising findings for ApBM as an adjunctive intervention for people in outpatient treatment. ApBM can be delivered via smartphone apps, yet little is known about how people interact with and interpret ApBM apps. We adopted an ‘evidence-making intervention’ approach to qualitatively explore experiences of an AUD ApBM app, to guide implementation in real-world settings. Whilst evidence-based research is mostly concerned with wheth
