Fetomaternal hemorrhage and abruptio placentae may occur concurrently. A prospective study was done to determine the incidence of fetomaternal hemorrhage in noncatastrophic cases of abruptio placentae consistent with sonographic findings. Significant fetomaternal hemorrhage was found in 75% of noncatastrophic cases. Implications of conservative management of abruptio placentae with fetomaternal hemorrhage are discussed.

Intracellular lipolytic activity and lipoprotein lipase activity were determined in healthy human placental tissue, in placentas obtained from patients with severe pre-eclampsia, and from cases of intra-uterine growth retardation. The level of lipoprotein lipase, which is responsible for the transfer of fatty acids across membranes, was found to be much higher compared with the intracellular lipase and had increased further during pregnancy. Abruptio placentae rarely produces severe maternal complications while the fetus is alive in utero. The advent of fetal death (grade III) indicates a severe form of abruptio placentae and a real risk that an overt coagulopathy might develop (grade IIIB). Overt coagulopathy associated with a live fetus is, however, uncommon. The advent of an overt coagulopathy should be viewed as ominous. Terbutaline was given to five patients with complete placenta praevia in order to prevent massive bleeding while at the same time prolonging the pregnancy. The longest period gained on therapy was 48 days and the shortest was 6 days. Doses of terbutaline used in these periods were 0.71-4.47 mg/dl. No serious complications were observed.

Studied were morphologically 12 aborted fetuses of cows, 12 placentas of cows that miscarried, 6 placentas of cows with normal birth but with certain lesions, and 6 uteri of cows that miscarried, originating from various regions of the country. The use of real-time ultrasound and antepartum nonstress testing in the management of abruptio placentae is discussed. In one patient, fetal distress developed when the retroplacental clot volume reached 480 ml. Two cases in which retroplacental clot was less than 480 ml were managed expectantly with the use of ultrasound and antepartum fetal testing, with excellent results.

Teratoma-like lesions of the placenta are rare and of disputed histogenesis. We describe one such tumour arising in the placenta of a normal full-term male infant delivered to a 33-yr-old woman. This lesion and all previously reported cases appear histologically and clinically benign. The authors favour the viewpoint that the so-called placental teratoma represents an extreme form of fetus amorphus.

Three cases of vaginal bleeding during labor associated with high-frequency uterine contractions (HFUC) in the third trimester of pregnancy are reported. Abruptio placentae was confirmed in all three cases. As HFUC was unrecognized as a sign of abruptio placentae, labor was allowed to continue in the first two cases and fetal demise ensued. The appearance of HFUC with vaginal bleeding in the third trimester of pregnancy should make the obstetrician highly suspicious of abruptio placentae. Labor should be terminated by cesarean section to prevent fetal death in utero.

The main indication for Caesarean section in cases of placental abruption is fetal distress. We report a patient in whom clinical examination, including auscultation of the fetal heart, suggested a mild haemorrhage, which would normally have been managed conservatively, but continuous fetal heart rate monitoring showed severe asphyxia, necessitating emergency Caesarean section at which a large concealed haemorrhage was found.

In a large, prospective study, mothers who smoked during one pregnancy but not another had smaller infants in the pregnancy in which they smoked, irrespective of birth order and many other factors that affect fetal growth. Light cigarette smokers gained an average 90 g less at term than non-smokers, entirely due to the smaller size of the light smokers' newborn infants. Six patients near term were given an intravenous infusion of a fat emulsion (Intralipid) a few hours before normal delivery or Caesarean section. Six other non-infused patients were studied as controls. Maternal venous and umbilical venous and arterial blood samples were taken at delivery and analyzed for individual fatty acid concentrations in triglyceride, free fatty acid (FFA) and phospholipid fractions.
