BACKGROUND: Postoperative platelet hyperaggregability following general anesthesia has been reported in patients undergoing major vascular surgery. In contrast, since anesthetic agents inhibited platelet function both in vitro and in vivo, an increased risk for postoperative bleedings due to prolonged platelet dysfunction has been discussed. OBJECTIVE: To evaluate the possible additional clinical benefit from autologous platelet concentrate (APC) treatment adjunct to non-surgical periodontal therapy (NSPT).
METHODS: Electronic (MEDLINE/Embase/Cochrane/MedNar/CORE) and hand searches were conducted. We herein report the case of a patient with idiopathic thrombocytopenic purpura (Werlhof disease) and coronary artery disease undergoing myocardial revascularization. The use of monomeric immunoglobulins, corticosteroids, platelets transfusion, use of a cell saver, normothermic cardiopulmonary bypass, aprotinine and homologous blood transfusion were combined in order to minimize the risk of bleeding complications in the postoperative period.
