Decline in pneumonia admissions after routine childhood immunisation with pneumococcal conjugate vaccine in the USA: a time-series analysis.

BACKGROUND: Routine infant immunisation with seven-valent pneumococcal conjugate vaccine (PCV7) began in the USA in 2000. Although invasive pneumococcal disease has declined substantially, the programme's effect on hospital admissions for pneumonia is unknown. We therefore assessed the effect of the programme on rates of all-cause and pneumococcal pneumonia admissions. METHODS: Data from the Nationwide Inpatient Sample, the largest inpatient database available in the USA, were analysed with an interrupted time-series analysis that used pneumonia (all-cause and pneumococcal) admission rates as

Emergence of 19A as virulent and multidrug resistant Pneumococcus in Massachusetts following universal immunization of infants with pneumococcal conjugate vaccine.

BACKGROUND: The long-term effects of selective pressure from conjugate pneumococcal vaccine on the serotype distribution and antimicrobial resistance of carriage and invasive isolates of Streptococcus pneumoniae are unknown. Early changes demonstrate a reduction in vaccine serotypes and an increase in nonvaccine serotypes (NVT) among both carriage and invasive isolates. Ongoing surveillance is necessary to identify emerging invasive serotypes and antimicrobial susceptibilities. METHODS: Enhanced surveillance of invasive pneumococcal disease in Massachusetts began in October 2001 and remains o

Pediatricians' self-reported clinical practices and adherence to national immunization guidelines after the introduction of pneumococcal conjugate vaccine.

BACKGROUND: Little is known about whether pneumococcal conjugate vaccine (PCV) has altered pediatricians' practices regarding well-child and acute care. OBJECTIVES: To (1) describe whether PCV caused pediatricians to move other routine infant vaccines and/or add routine visits; (2) characterize adherence to national immunization recommendations; and (3) determine whether PCV altered pediatricians' planned clinical approach to well-appearing febrile infants. DESIGN AND METHODS: One year after PCV was added to the pediatric immunization schedule, we mailed a 23-item survey to 691 randomly sele

The contribution of specific pneumococcal serogroups to different disease manifestations: implications for conjugate vaccine formulation and use, part II.

To assess whether certain serogroups of Streptococcus pneumoniae are preferentially associated with specific disease manifestations, we analyzed all recent pneumococcal disease studies and assessed the relative frequency of isolation of each serogroup by clinical site (as a proxy for different disease states). In all age groups, serogroups 1 and 14 were more often isolated from blood, and serogroups 6, 10, and 23 were more often isolated from cerebrospinal fluid (CSF); in young children, serogroups 3, 19, and 23 were more often isolated from middle ear fluid (MEF). Serogroups represented in co

Accuracy of ICD-9-CM codes in detecting community-acquired pneumococcal pneumonia for incidence and vaccine efficacy studies.

Studies have used medical record discharge data as coded by the International Classification of Diseases, 9th Revision, Clinical Modification (ICD-9-CM) to estimate pneumococcal pneumonia incidence and vaccine efficacy. However, the accuracy of coding data to identify laboratory-confirmed pneumococcal pneumonia is not known. With the use of information collected in Ohio for a community-based pneumonia incidence study, the authors calculated the sensitivities, specificities, positive predictive values (PPV), and negative predictive values (NPV) of specific codes for pneumococcal pneumonia among

SCID-Hu mice immunized with a pneumococcal vaccine produce specific human antibodies and show increased resistance to infection.

Seventy-eight severe combined immunodeficiency (SCID) mice were administered intraperitoneally 1 x 10(7) to 9 x 10(7) human peripheral blood mononuclear cells (PBL) in five experiments. Human immunoglobulin G (IgG) was detected in 70 to 88% of these SCID-PBL-Hu mice after cell transplantation, and all four subclasses were present. The total concentration of human IgG varied from less than 1 to 10.2 g/liter. The SCID-PBL-Hu mice with high concentrations of human IgG regularly had mono- or oligoclonal human IgG bands in serum, as demonstrated by agarose gel electrophoresis. Of the SCID-PBL-Hu mi

Fatal pneumococcal bacteremia with disseminated intravascular coagulation and Waterhouse-Friderichsen syndrome in a vaccinated splenectomized adult. Case report.

Fatal pneumococcal bacteremia, disseminated intravascular coagulation and Waterhouse Friderichsen syndrome in a vaccinated, splenectomized adult were caused by serotype 22F (Danish classification), which was not included in the vaccine. Revaccination with a 23-valent pneumococcal vaccine and antibiotic prophylaxis are advocated for patients who have undergone splenectomy.

Safety of a combination diphtheria, tetanus toxoid, acellular pertussis, hepatitis B, and inactivated polio vaccine coadministered with a 7-valent pneumococcal conjugate vaccine and a Haemophilus influenzae type b conjugate vaccine.

The safety of DTaP-HepB-IPV vaccine coadministered with PCV and Hib was compared with separate administration of DTaP, HepB, IPV, Hib, and PCV at 2, 4, and 6 months of age. Healthy 2-month-old infants (N=1008) were randomized to the two groups. Following dose 1, there was no significant difference between the groups in the incidence of fever >101.3 degrees F. After each dose, the incidence of any fever (> or =100.4 degrees F) was significantly higher in the Combination Vaccine Group. The rate of fever >103.1 degrees F was < or =1.4% in both groups after any of the doses. Medical advice visits

Evaluation of the effect of serotype on pneumococcal conjugate vaccine-induced immunity: an exploratory analysis of the PREVENTING PNEUMO 2 trial.

BACKGROUND: Although evidence suggests some direct protection against Streptococcus pneumoniae serotype 3 (Spn3) from the 13-valent pneumococcal conjugate vaccine (PCV13), Spn3 remains a common cause of pneumococcal disease. We aimed to assess whether serotype-specific humoral immune responses to capsule polysaccharide (CPS) 3 and CPS6B after vaccination were associated with protection from experimental pneumococcal carriage. METHODS: We undertook a prospective cohort study nested within the PREVENTING PNEUMO 2 double-masked, randomised, phase 4 human pneumococcal challenge trial conducted at

Accounting for serotypic heterogeneity improves the interpretation of pneumococcal vaccine response using serotype-specific ELISA IgG concentrations.

Assessment of pneumococcal vaccine response by enzyme-linked immunosorbent assay (ELISA) currently relies on a single IgG concentration threshold applied to all serotypes. This approach does not account for the marked serotype variability in the relationship between IgG concentration and functional antibody activity. We investigated whether accounting for serotypic heterogeneity improves the interpretation of pneumococcal vaccine response by establishing serotype-specific IgG concentrations predictive of a positive OPA response (PIC). We analyzed 729 samples from adults referred for assessment

A randomized, placebo-controlled trial of 13-valent pneumococcal conjugate vaccination to accelerate immune recovery after sepsis.

Adults who survive intensive care unit (ICU) admission with sepsis (sepsis survivors) have immune impairments involving concurrent inflammation and immunosuppression that increase their long-term risk of reinfections and mortality. Vaccine immunogenicity could therefore be abnormal in sepsis survivors but has never been examined. Here, in a 1:1 randomized, placebo-controlled trial, we tested the efficacy and immunogenicity of a single intramuscular dose of 13-valent pneumococcal conjugate vaccine (PCV13) in 214 sepsis survivors at ICU discharge. The PCV13 group (n = 104) experienced 43 primary

Improving pneumococcal vaccination uptake in a residency clinic: a case-inspired QI initiative aligned with updated CDC guidelines.

Streptococcus pneumoniae remains a leading cause of preventable morbidity and mortality among adults, particularly those with chronic medical conditions, yet pneumococcal vaccination uptake in adult populations remains suboptimal. Residency-based primary care clinics face unique barriers to preventive care delivery, including staffing instability and competing clinical priorities. After identifying pneumococcal vaccination rates in our internal medicine residency clinic that were lower than the national average, we conducted a quality improvement initiative to increase vaccination uptake. Invasive pneumococcal disease in Latin America: serotype distribution and vaccine coverage among adults in Mexico, Brazil, and Argentina.

BACKGROUND: Invasive pneumococcal disease (IPD) causes substantial morbidity and mortality among adults in Latin America. Despite high pediatric pneumococcal conjugate vaccine (PCV), uptake, adult coverage remains low, and residual IPD burden persists. Among currently available PCVs, PCV21 and PCV20 are the newest higher-valency formulations. PCV21 includes eight unique serotypes to address residual adult disease. This analysis quantified the proportion of IPD cases in Argentina, Brazil, and Mexico attributable to PCV21 serotypes compared with those in other pneumococcal vaccines to estimate p…

Pneumococcal pneumonia in adults - re-emergence of vaccine serotypes: a prospective multicenter cohort study in Germany, 2020-2023.

The main burden of non-invasive pneumococcal diseases in adults is largely due to community-acquired pneumonia (CAP). This study aimed to investigate the distribution and dynamics of pneumococcal vaccine serotypes and to determine the proportion of CAP cases attributable to serotypes covered by 13-valent conjugate vaccine (PCV13), the 20-valent conjugate vaccine (PCV20), and the 23-valent polysaccharide vaccine (PPV23) among adults in Germany from 2020 to 2023. In this prospective multicenter cohort study, we analyzed all adult patients with CAP enrolled between January 1, 2020, and December 3

Cross-sectional investigation of pneumococcal vaccination patterns in HIV patients at a tertiary care hospital in Riyadh, Saudi Arabia.

BACKGROUND: People living with HIV (PLWH) are recognized as a high-risk population for opportunistic infections, including community-acquired pneumonia (CAP). Therefore, the United States Centers for Disease Control and Prevention (US-CDC) and Saudi Ministry of health (MOH) advocate for pneumococcal vaccination among all PLWH as a crucial preventive measure against pneumococcal infections. Hence, this study aims to investigate the prescribing patterns of pneumococcal vaccines for PLWH at a tertiary care hospital. As PCV15 and PCV20 were not available until 2022, this study focuses on the presc…

Pediatric Pleural Effusion and Pneumococcal Vaccination Trends in the Pre- and Post-COVID Era: A Single-Centre Retrospective Study.

Background/Objectives: Pleural effusion represents an accumulation of fluid in the pleural cavity, frequently associated with pneumonia. There has been a gradual increase in cases among children in recent years, with a notable rise during the post-pandemic period, potentially due to immune debt, decreased vaccination coverage, and changes in pathogen dynamics. Methods: We enrolled 66 children with pleural effusion treated at the Children's Emergency Clinical Hospital, Brasov, between January 2019 and September 2024. We analyzed the data on demographics, symptoms, vaccination status, hospitaliz

Hospital-Based Surveillance of Pediatric Invasive Pneumococcal Diseases, 2016-2023 in Korea: Serotype Trends and Vaccination Policy.

BACKGROUND: In Korea, the 10-valent and 13-valent pneumococcal conjugate vaccines (PCVs) were introduced into the national immunization program (NIP) in 2014 for the protection in children. A decade later, in 2024, PCV15 replaced PCV10 and was included in the NIP in April, while PCV20 was licensed for use in October. To inform optimal vaccination policy, this study aimed to analyze the current distribution of serotypes responsible for invasive pneumococcal diseases (IPDs) in children. METHODS: IPD cases from children under 19 years of age were collected from a prospective hospital-based surve

NKT cell-dependent PspA-specific IgA production caused by mucosal administration of a novel nanoparticle pneumococcal vaccine.

Pneumonia is one of the leading causes of death worldwide, with Streptococcus pneumoniae being a major causative agent of community-acquired pneumonia. In immunocompromised infants and the elderly, this pathogen can invade the bloodstream and cerebrospinal fluid, leading to invasive pneumococcal diseases (IPDs), such as bacteremia and meningitis. Currently, vaccines targeting the capsular polysaccharides of pneumococcus are available to prevent pneumococcal infection. However, these vaccines have limitations, including incomplete coverage of all serotypes. Therefore, there is a need for next-g
