Surgical management of upper- and lower-lobe bronchiectasis without middle lobe involvement: is middle lobectomy necessary?

Postoperative quality of life is a crucial factor in decisions regarding surgical management of bronchiectasis. The goal of surgical treatme … thorax. When the disease involves both the upper and lower lung lobes, surgeons must assess whether pneumonectomy is required. Herein, we describe the case of a patient with bronchiectasis who was successfully treated with upper and lower lobectomy and preservation of the middle lobe.

Postpneumonectomy space infection eight years after mediastinal repositioning procedure.

Postpneumonectomy syndrome is a rare complication following pneumonectomy. Repositioning of the mediastinum via insertion of prosthetic implants into the postpneumonectomy space can provide symptomatic relief. We present a case of a man in his early 70s presenting with empyema necessitans 8 years after the implantation of silicon-saline prostheses for the management of postpneumonectomy syndrome. Excision of the chest wall sinus, re-do right thoracotomy and removal of the infected silicon-saline prostheses and postprocedural intrapleural irrigation led to resolution. There was no evidence of m…

Posterior trans-mediastinal lung herniation in a postoperative Marfan's patient.

Thoracic hernias are defined as the protrusion of contents either into or outside of the thoracic cavity. These hernias can be acquired or congenital and occur in varying locations. Acquired thoracic hernias typically develop after surgery or trauma. The most common form is intercostal herniation due to thoracotomy. Trans-mediastinal herniation of the lung is even less common, typically seen in patients with sequestration, scimitar syndrome, or pneumonectomy, and, when present, commonly occurs across the anterior mediastinum. Here, we present to our knowledge the first known case of posterior …

Prolonged survival after multimodal therapy for pleural mesothelioma: Don't give up the follow-up. A case report.

Malignant Pleural mesothelioma (MPM) is a rare disease which is associated with a poor prognosis. Front line chemotherapy represents the cornerstone in the management of MPM, and the place of radical surgery is controversial and reserve in early-stage disease. However prolonged survival (more than 24 months) can be observed in rare cases and only in the context of multimodal treatment including surgical management. We report the case of a patient suffering from an epithelial MPM with a 14-years progression-free survival after trimodal treatment including extrapleural pneumonectomy followed by …

Marked cachexia in probable invasive pulmonary aspergillosis with bronchopleural fistula.

A 49-year-old man with a medical history of diabetes and heavy smoking was admitted to intensive care with severe bilateral pneumonia associated with marked cachexia. He developed a complex right-sided bronchopleural fistula and was transferred to our tertiary centre for consideration of surgical intervention.Despite escalation of antibiotic therapy, he did not improve and further investigations led to a diagnosis of invasive pulmonary aspergillosis. Definitive treatment plans required a right pneumonectomy; however, given the severity of cachexia, he remained unable to undergo such a large op…

[A case of bronchial carcinoid diagnosed after 21 years of recurrent lung infections].

A case of 59 year old woman with a 21 year history of recurrent pneumonias of the upper left lobe is presented. When she at last agreed for the diagnosis her chest X-rays revealed atelectasis and cirrhosis with bronchiectases of the left lung. Tumor of the left main bronchus and severe inflammation of the surrounding mucosa with purulent secretion were found during bronchofiberoscopy. The patient underwent left pneumonectomy. Histological examination of the specimen revealed presence of carcinoid in the main left bronchus without involvement of the regional lymphnodes, with signs of irreversib…

Severe pneumococcal pneumonia complicated by massive pulmonary gangrene.

Massive pulmonary gangrene is a rare complication of pneumonia, particularly in the postantibiotic era. We report two cases of community-acq … tiple organ failure and the obvious failure of appropriate medical therapy, both patients underwent pneumonectomy with a successful outcome. These cases serve to emphasize the role of surgery in the management of massive pulmonary gangrene.

The VVBA method for thoracoscopic right middle lobe segmentectomy.

The right middle lobe segments (RS4 and RS5) are very small and the technique for segmentectomy is rarely reported. Here, we report 3 cases of thoracoscopic RS4 segmentectomy with VVBA (V, vein; V, ventilation; B, bronchus; A, artery) method. The VVBA method for thoracoscopic segmentectomy is a simple, safe, and effective procedure for RS4 segmentectomy with clear procedural steps. It overcomes the difficulty in manipulation of incomplete lung fissures and simplifies the surgical procedures.

Advances and controversies in the management of early stage non-small cell lung cancer.

Complete resection continues to be the gold standard for the treatment of early-stage lung cancer. The landmark Lung Cancer Study Group trial in 1995 established lobectomy as the minimum intervention necessary for the management of early-stage non-small cell lung cancer, as it was associated with lower recurrence and metastasis rates than sublobar resection and lower postoperative morbidity and mortality than pneumonectomy. There is a growing tendency to perform sublobar resection in selected cases, as, depending on factors such as tumor size, histologic subtype, lymph node involvement, and re…

Main bronchial sleeve resection with pulmonary conservation.

This article aims to present the experience of our clinic regarding 4 cases of primitive bronchial disease for which a main bronchial sleeve resection with pulmonary conservation were performed between 2005 and 2006. The reduced number of cases allows us express the opinion that in well evaluated cases where bronchoscopic exam is essential the main bronchial sleeve resection and full lung preservation is a procedure that can be made with excellent results for the patient. Life quality is superior to that of cases with pneumonectomy which can be a disease by itself. An adequate technique, the r…
