Sleep-disordered breathing (SDB) refers to a group of disorders characterized by abnormalities of respiration or ventilation during sleep. It encompasses obstructive sleep apnea (OSA), central sleep apnea (CSA) syndromes, sleep-related hypoventilation and sleep-related hypoxemia disorders. This review will concentrate on the disorder most prevalent in pediatrics, i.e., OSA, highlighting the most recent developments in our understanding of the etiology, pathophysiology and treatment options of this condition. OSA morbidities primarily involve the neurocognitive, cardiovascular and metabolic sys

OBJECTIVES/HYPOTHESIS: To examine the current body of literature supporting the role of the larynx in adult obstructive sleep apnea (OSA).
DATA SOURCES: PubMed.
METHODS: PubMed search using medical subject headings "larynx" and "obstructive sleep apnea." Original articles in English related to adult patients were initially included, and the bibliographies of pertinent articles were hand-searched for additional articles.
RESULTS: A total of 17 articles were included in the review in the following subgroups: neuromuscular role of larynx (n = 3), anatomic contribution of larynx to OSA (n = 7),

Slow waves during non-rapid eye movement (NREM) sleep are associated with the restorative aspects of sleep. Previous research has suggested reduced normalized slow wave activity (SWA), particularly in the first cycle of NREM sleep, as a feature of hypersomnolence disorder (HD); however, the scarcity of existing research highlights the need for replication studies. Thus, the aim of this investigation was to evaluate normalized SWA in patients with HD, relative to healthy sleeper controls (HSC). Nocturnal polysomnography from 37 unmedicated HD clinical patients were compared against 29 HSC. Six-…

Sleep apnea (SA) diagnosis remains challenging due to the cost and limited accessibility of polysomnography. Most automated screening approaches estimate the Apnea-Hypopnea Index (AHI) from oximetry-derived features and do not provide temporally resolved characterization of respiratory events. This study investigates the feasibility of dense temporal respiratory event profiling and clinically meaningful SA severity estimation from single-channel photoplethysmography (PPG).
Approach: A two-stage framework was developed using single-channel PPG. A fully convolutional neural network operating…

Rapid eye movement (REM) sleep behavior disorder (RBD) is characterized by dream-enacting behaviors occurring in the presence of REM sleep without atonia (RWA) and is most commonly associated with synucleinopathies. Structural lesions of the brainstem have been reported as a potential substrate of secondary RBD, yet polysomnographically confirmed cases remain sparsely documented. We report two patients with focal ischemic brainstem lesions and clinically suspected RBD who underwent detailed neurological evaluation, neuroimaging, and video-polysomnography. In both patients, polysomnography demo…

BACKGROUND AND OBJECTIVE: Obstructive sleep apnoea (OSA) is a heterogeneous disorder. Although excessive daytime sleepiness (EDS) is a hallmark symptom of OSA, many patients remain asymptomatic, and a subset continues to experience EDS despite optimal therapy. The endotypic traits underlying EDS in OSA remain poorly defined.
METHODS: We conducted a cross-sectional study of 694 adults at a Beijing sleep center, including 542 participants with AHI ≥ 5 events/h. EDS was defined as an Epworth Sleepiness Scale score > 10. Four endotypic traits were derived from full-night polysomnography using the…

All patients underwent polysomnography, and OSA severity was classified using the apnea-hypopnea index as mild, moderate, or severe. OBJECTIVE: To assess the expression changes of serum fibrinogen, E-selectin, and tissue-type plasminogen activator (t-PA) in acute ischemic stroke (AIS) patients with varying degrees of obstructive sleep apnea syndrome (OSA), and evaluate their value in diagnosing AIS with OSA.
METHODS: Data were gathered from 80 patients with AIS who were admitted to the First Hospital of Jilin University between January 2023 and December 2023. Out of these, 60 patients completed the NIHSS Scale, ESS Scale, STOP-Bang Scale, and underwent polysomnography within a week of symptom onset. Based on the apnea-hypo…

OBJECTIVES: Respiratory mechanic instability (RMI) is characterized by irregular chest and abdominal movements due to upper airway collapse or obstruction, frequently observed during obstructive apnea or hypopnea events. This study aims to explore the potential of RMI as a determinant of adenotonsillectomy's success in treating pediatric obstructive sleep apnea (OSA).
METHODS: Medical records of pediatric patients diagnosed with OSA via standard polysomnography who underwent adenotonsillectomy with subsequent follow-up evaluations were retrospectively analyzed. RMI parameters were automatical…

INTRODUCTION: The polysomnography is inconvenient and complex in diagnosing obstructive sleep apnea-hypopnea syndrome. Diagnostic potency of more novel contactless monitoring devices are needed to be validated to screen individuals with obstructive sleep apnea-hypopnea syndrome. The study aimed to evaluate the diagnostic value of a novel contactless sleep monitoring device in obstructive sleep apnea-hypopnea syndrome.
METHODS: Demographic features and parameters of monitoring devices were collected from adults with suspected obstructive sleep apnea-hypopnea syndrome from September 2023 throug…

Polysomnography annotations in Sleep Heart Health Study (SHHS), Osteoporotic Fractures in Men Study (MrOS), and Multi-Ethnic Study of Atherosclerosis (MESA) scored apneas and hypopneas solely by flow reduction rather than following the American Academy of Sleep Medicine's (AASM) comprehensive criteria. To address this, we developed a standardized annotation pipeline that integrates sleep staging, oxygen desaturation, and arousal events in accordance with AASM guidelines. This retrospective study analyzed polysomnography data from SHHS1 (n = 5793), SHHS2 (n = 2651), MrOS1 (n = 2907), MrOS2 (n =…

BACKGROUND: Sleep deprivation is common in intensive care units (ICUs) and may alter respiratory performance. Few studies have assessed the role of sleep disturbances on outcomes in critically ill patients.
OBJECTIVES: We hypothesized that sleep disturbances may be associated with poor outcomes in ICUs.
METHODS: Post-hoc analysis pooling three observational studies assessing sleep by complete polysomnography in 131 conscious and non-sedated patients included at different times of their ICU stay. Sleep was assessed early in a group of patients admitted for acute respiratory failure while brea…

STUDY OBJECTIVES: Obstructive sleep apnea (OSA) is common in children with syndromic craniosynostosis (SC). The efficacy of the Le Fort III procedure in managing OSA in children with SC remains a subject of ongoing debate. This study aimed to explore the efficacy of Le Fort III procedure in the management of OSA in children with SC.
METHODS: A retrospective study was performed in children with SC and OSA diagnosed by polysomnography, which was defined as an apnea-hypopnea index ≥ 1 event/h. Patients meeting the inclusion criteria were those who underwent Le Fort III surgery and had both basel…

OBJECTIVE: To explore sleep structure in participants with obstructive sleep apnea (OSA) and comorbid insomnia (COMISA) and participants with OSA without insomnia (OSA-only) using both single-night polysomnography and multi-night wrist-worn photoplethysmography/accelerometry.
METHODS: Multi-night 4-class sleep-staging was performed with a validated algorithm based on actigraphy and heart rate variability, in 67 COMISA (23 women, median age: 51 years) and 50 OSA-only (15 women, median age: 51) participants. Sleep statistics were compared using linear regression models and mixed-effects models.…
