Attentional demands and postural control: the effect of sensory context.

BACKGROUND: This study used a dual task design to examine the effect of sensory context on postural stability during the concurrent performance of an attentionally demanding cognitive task in young and older adults with and without a history of imbalance and falls. METHODS: A choice reaction time auditory task was used to produce changes in attention during quiet stance in six different sensory conditions that changed the availability of accurate visual and somatosensory cues for postural control. Postural stability was quantified by using forceplate measures of center of pressure in 18 young

A comparison of different postures for scaffold end-frame disassembly.

Overexertion and fall injuries comprise the largest category of nonfatal injuries among scaffold workers. This study was conducted to identify the most favourable scaffold end-frame disassembly techniques and evaluate the associated slip potential by measuring whole-body isometric strength capability and required coefficient of friction (RCOF) to reduce the incidence of injury. Forty-six male construction workers were used to study seven typical postures associated with scaffold end-frame disassembly. An analysis of variance (ANOVA) showed that the isometric forces (334.4-676.3 N) resulting fr…

Downbeating nystagmus and postural hypotension due to basilar invagination.

Downbeating nystagmus is an involuntary vertical rhythmic eye movement with the fast component in the downward direction. The sign indicates a craniocervical disorder. The most common cause is the Arnold-Chiari malformation, followed by cerebellar degeneration. Basilar invagination is a rare cause of downbeating nystagmus. However, with appropriate treatment its prognosis is good. Here, we report a case of basilar invagination which presented with downbeating nystagmus and postural hypotension. A 31 year-old Thai male patient had a 20 year history of postural hypotension. He had recurrent pneu

Foot and ankle sensory neuropathy, proprioception, and postural stability.

Foot and ankle sensory neuropathy may result from a variety of pathologic conditions, especially diabetes mellitus. Decreased sensation, particularly on the plantar surface of the feet, leads to obvious risks of cutaneous injury. Less obvious are the risks of fall-related injury associated with changes in other sensory systems of the foot and ankle, such as the receptors involved in joint movement and position perception. The results of a number of studies demonstrate that the neuropathic process affects these receptors in individuals with diabetes mellitus. Associated with the decreased senso

Postural vasoregulation and mediators of reperfusion injury in venous ulceration.

PURPOSE: To test the hypothesis that ischemia and reperfusion injury may contribute to the cause or nonhealing of venous ulcers, the effects … was no statistically significant change in effluent concentration of any mediator as a function of posture, ulcer size, or healing. CONCLUSIONS: Postural vasoregulation causes relative ischemia and reperfusion in venous leg ulcers. However, this is not associated with changes in release of mediators known to be related to reperfusion injury in internal …

[Study of postural drainage of children with obstructive lung disease].

According to the abnormal manifestation of the X-ray chest film, 90 cases were classified into regional shadow and diffuse volume increasing parts. The regional postural drainage method was adopted in the former, all directional postural drainage was performed in the latter. Each part of cases was classified into postural drainage plus ultrasonic aerosol inhalation group, ultrasonic aerosol inhalation and the control group. The results showed that there were significant differences among the parameters of the body temperature, coughing, stridor, rales and the chest film (P < 0.01). The therape

Maternal postural challenge as a functional test for cervical incompetence.

We studied the effect of an upright maternal position on the cervix. Of 41 high risk patients (17 to 33 weeks' gestational age), 14 of 16 wh … term delivery was 100%. We conclude that for patients at high risk for incompetent cervix, maternal posture-related cervical shortening can assist in predicting those who will undergo premature delivery.

Postural effects of motion vision in young autistic children.

We present the first assessment of motion vision in childhood autism. Postural reactivity to visually perceived motion was measured in five autistic children and 12 normal controls of the same chronological age. Anteroposterior as well as total body sway occurring on a force platform in response to movements in the visual environment were compared. Autistic children were posturally more unstable than normal children and quite insensitive to visually perceived environmental motion. Some implications of this impairment for sensorimotor and social communication development in infantile autism are

Subjective postural vertical in peripheral and central vestibular disorders.

The perception of subjective postural vertical was assessed in normals and patients with peripheral and central vestibular disorders and spasmodic torticollis. The subjects were seated in a motorized gimbal with the head and torso restrained and their eyes closed. The gimbal executed 7-10 cycles of tilt around the vertical at 1.5 degrees/s in either pitch or roll. Subjects indicated when they began to feel upright and again when they began to feel tilted by an analogous 3-position joystick. Normal subjects felt upright within a sector of 5-6 degrees around vertical in pitch and roll. Five pati

Unilateral congenital ptosis: compensatory head posturing and amblyopia.

Patients with unilateral congenital ptosis often adopt chin elevation to maintain fusion. This compensatory head posturing has been considered a sign of fusion, thus indicating a low risk for amblyopia. To evaluate this sign, we reviewed the charts of 11 preverbal children with unilateral congenital ptosis who had straight eyes and appropriate compensatory head posturing to maintain binocular vision. Four additional patients with unilateral ptosis and no compensatory head posturing were tested for comparison. All patients had undergone an examination and had been tested by pattern Visual Evoke

Postural organization in patients with hereditary motor and sensory neuropathy.

The postural organization in patients with hereditary motor and sensory neuropathy (HMSN) type I or II was studied clinically by means of a … ecreased efficiency of postural control (p less than .01) as well as an increased visual control of posture (p less than .05) in both directions of sway. No loss of balance automaticity was found, which suggested an on-line central adaptation to the slowly developing peripheral impairments. The results provide a starting point for understanding the balanc …

Postural hypotension: pressor effect of octreotide not mediated by norepinephrine.

Orthostatic hypotension of the Shy-Drager syndrome is a chronic incapacitating condition characterized by lack of an appropriate increase in the plasma norepinephrine level in response to standing. Recently, the somatostatin analogue octreotide has been reported to induce a pressor response in patients having this syndrome. We have reported a case of Shy-Drager syndrome in which octreotide was effective, but the rise in blood pressure was not accompanied by an increase in the plasma norepinephrine level. Hence, the pressor effect of octreotide is not mediated by stimulation of the sympathetic

[Radiographic screening of asymmetry and postural changes in orthodontic patients].

The role of OPM and of the teleradiography in the diagnosis of the asymmetries and of the postural problems of the orthodontic patients, is pointed out from this analysis on the group of 204 patients from mixed dentition to the adult age with different types of malocclusion. The evidence of a significant amount of patients with asymmetries and incorrected posture shows the utility of this radiological screening and gives advises for a therapeutic approach more corrected.

Postural control in siblings to scoliosis patients and scoliosis patients.

The etiologic importance of postural dysfunction, as shown by many authors in adolescent idiopathic scoliosis (AIS), has been under great debate. The authors' hypothesis was that a factor that is involved in the development of the scoliotic curvature, would be present also in nonscoliotic siblings to scoliosis patients, as AIS is a hereditary transmitted disease. Postural function in 100 siblings to scoliotic children was investigated by means of stabilometry, and compared with a matched control group of healthy children, as well as a group of scoliotic children. The siblings showed a postural

Mechanism of the postural dependence of vital capacity in tetraplegic subjects.

It is well established that unlike normal subjects patients with cervical cord transection have an increase in VC when changing from the seated to the supine posture. To investigate the mechanism of this paradoxical increase, we measured static lung volumes in both the seated and supine posture in 14 consecutive patients with tetraplegia (C4-C7) and in 4 patients with paraplegia (Th4-Th7). The increase in VC in the supine compared with the seated posture was (mean +/- SE) 0.41 +/- 0.07 L (16.0% of the seated value) in the tetraplegic subjects and 0.40 +/- 0.01 L (11.2% of the seated value) in …

Hypotension with postural syncope secondary to the combination of chlorpromazine and captopril.

A 49-year-old man with severe hypertension and chronic schizophrenia developed marked hypotension with postural syncope following therapy wi … that increased during therapy with captopril and a physiologic catecholamine response to change in posture. These data demonstrate that there is a synergism between captopril and chlorpromazine that may result in marked, symptomatic hypotension that is probably unrelated to the baseline level of plasma renin activity or catecholamines.

Decerebrate posturing with the syndrome of inappropriate secretion of antidiuretic hormone.

Decerebrate and/or decorticate posturing have not yet been described as possible features of the syndrome of inappropriate secretion of antidiuretic hormone. Cerebral edema and loss of brain electrolytes are contributing factors resulting in altered excitability of the neural membrane. We describe a patient who, on the eighth day after a head injury, presented with decerebrate posturing, which coincided with the development of the syndrome of inappropriate secretion of antidiuretic hormone, and which rapidly reversed with the correction of the hyponatremia. We believe that the syndrome of inap
